Caregivers You Can Trust

  Journey Homecare Services provides a wide range of homecare services including adult and pediatric skilled nursing, non-medical companion and personal care, as well as medical staffing. Our professionals partner with our clients and their families to provide services specific to you or your loved one’s individual needs. When needed, our clinical team will coordinate with your physician to develop a personal care plan. All services provided by Journey Homecare Services are supervised regularly to ensure client satisfaction.  

(Coming Soon) Skilled Nursing Services

Journey Homecare provides nursing care for both children and adults with high-tech nursing needs at home, school or anywhere in the community. Our Team of RNs and LPNs can provide up to 24 hours of care each day, 7 days a week depending on individual needs and physician orders. An individual nursing plan of care is developed for each patient and carried out by skilled nursing staff.

Companion Care (CNA/PCA/PCS)

Our CNAs/PCAs will visit you at home or in the living environment of your choice, provide custodial care and assist with activities of daily living (ADL’s). Our screened and certified care professionals will work under the direction of a Registered Nurse and follow a care plan that is recommended and approved for you by your attending physician.

Our In-Home Personal Care Services Include, but are not limited to:

• Showering / Bathing
• Assistance with Ambulation
• Meal preparation
• Feeding assistance
• Medication reminders
• Light housekeeping
• Transfer/positioning assistance
• Post Hospital Assistance

Care Partners are thoroughly screened and must meet our strict guidelines for employment. We always perform a comprehensive investigation that involves reference checks, state and federal background checks, drug screen and DMV record checks. We train, lead, and supervise all Care Partners, and cover all government mandated taxes and worker’s compensation for our employees. Only Care Partners who pass our in-depth screening process, orientation, and training are employed by Virginia Home Care Partners.

How quickly can I arrange for service?

Our personal care service is nurse managed and the development of a comprehensive care plan precedes the start of care.  We partner to ensure your care plan meets your needs and we seek to provide the best Care Partner match possible. We can usually begin services in a few days.

How much does the service cost?

We bill on an hourly basis for most services. The hours required to properly care for your loved one will be determined during the initial assessment and care plan development and reviewed on an ongoing basis. Please contact us to learn more about our pricing.  Our services are available for private pay and private insurance.  We accept cash, checks and offer bank draft service.  For more details, please contact us today.

What is the purpose of Journey Together Care Home?

Journey Together Care Home is an adult residential facility providing 24-hour non-medical care and supervision to adults with intellectual/developmental disabilities.

We are partnered with Eastern Los Angeles Regional Center and are fully funded by the regional center. 

Our purpose goes beyond the scope of our mission statement. Journey Together Care Home is here to make a positive impact on the lives of our persons served. We are here to create a home and a family. Journey Together Care Home’s goals for the program are to increase the people's access to the day to day and special activities in their community, to increase participation in activities with non-disabled individuals, increase and improve positive behaviors within the home and in public, and to increase self-care and independent skills of persons served in the home and in the community. Individuals will be given choice and inclusion in all aspects of their daily lives.
