Tour bus company had safety violations before fatal crash at Grand Canyon

tour conventionnel accident

Federal inspectors over the last year found numerous safety violations on buses operated by the California-based company involved in a rollover crash near the Grand Canyon, killing one passenger and seriously injuring eight others, records show.

Arizona Department of Public Safety’s preliminary report, from the August 1 crash, said the tour bus was operated by American Transportation Services and had 56 passengers on board.

According to the report, the bus was getting ready to let passengers off at the parking lot near the Grand Canyon Skywalk, a popular tourist destination, when the 68-year-old driver drifted off the roadway into a drainage ditch.

The tour bus continued out of the ditch into the parking lot and rolled over, striking three parked cars. One of the cars intruded into the passenger compartment of the bus, killing a female passenger on board. Eight other passengers were seriously injured, and all the other remaining passengers were taken to local hospitals.

Records show the tour bus company, American Transportation, based in Long Beach, CA, had enough safety violations in the last year to exceed federal safety thresholds subjecting the company to possible prioritized intervention action and roadside inspection.

Before the most recent crash, Federal Motor Carrier Safety Administration records show the company’s drivers were involved in two other injury crashes in the last two years. Federal inspectors recently cited five operators for driving over their hour limit or for not keeping proper logs. Records also show the company has more than 100 buses in its fleet and had 107 maintenance violations in the last two years.

The 68-year-old bus driver involved in last week’s crash was arrested on suspicion of manslaughter, a class two felony, and booked into a Mohave County Detention Center. The driver has since been released after the county attorney chose not to file charges while the investigation into the crash continues.

ABC15 spoke with the bus driver, and he directed us to his attorney who had no comment. We have not heard back from the tour bus company.

A tour bus on its way to the Grand Canyon was speeding when it flipped on its side, killing one passenger while injuring dozens, according to an investigative report that stops short of drawing conclusions about the cause of the crash.

Four dozen people from across the country were on the bus operated by Las Vegas-based Comedy On Deck Tours when it veered into a dirt embankment, over ruts and rocks, and hit Joshua trees on Jan. 22. At one point, it rode the face of a small hill and was airborne before coming back on to the road and flipping on its side, according to records.

The passengers suffered injuries ranging from abrasions and ankle sprains to fractures, blunt force trauma and broken ribs. Shelley Ann Voges from Booneville, Indiana, was partially ejected and died. Her husband and son, who recently had moved west, also were on the trip.

The bus was heading to Grand Canyon West, about 2.5 hours from Las Vegas and outside the boundaries of the national park. The tourist destination is on the Hualapai reservation and is best known for the Skywalk, a glass bridge that juts out 70 feet (21 meters) from the canyon walls and gives visitors a view of the Colorado River 4,000 feet (1,219 meters) below.

The Mohave County Sheriff’s Office released the crash report, body camera footage, photographs and 911 call recordings to The Associated Press in response to a public records request. The office didn’t respond to additional questions about the report Wednesday.

Passengers told authorities that it appeared the driver possibly had fallen asleep and was driving too fast. The records also call into question whether the automatic engine brake was engaged as the bus traveled on a curve and slightly downhill.

Mohave County sheriff’s Sgt. Dave Coffin noted speed limits of 45 mph posted along the road. The driver, Gary Griep, told authorities he was going no more than 40 mph, but the records say the GPS on the bus put the speed at 62 mph.

Griep said the engine brake, which helps slow and control the bus, was engaged, but two other bus drivers who were on tours in the area that day and stopped at the crash site said that didn’t appear to be true.

The crash report also said there were no visible marks on the tires or in the embankment to indicate heavy braking or steering for at least 700 feet (213 meters). Only when the bus hit a raised embankment did it veer from its path, the report said.

The investigation has not been turned over to prosecutors, said Cara Engstom of the Mohave County Attorney’s Office. The sheriff’s office is awaiting the results of a toxicology test on Griep and an autopsy report on Voges.

Passengers who had to crawl out of a rooftop hatch, the back door or large windows said they thought Griep might have fallen asleep. Griep told authorities he got at least eight hours of sleep the night before, doesn’t drink alcohol and wasn’t under the influence of any drugs. He said he sometimes coughed so hard his head hung low but he always kept his eyes on the road.

He attributed the crash to gusty winds that the National Weather Service said were 13 to 19 mph at the time.

Griep picked up passengers at hotels along the Las Vegas Strip that morning, took them to breakfast and the Hoover Dam and then headed toward the Grand Canyon Skywalk. As he came around a corner, he said the wind pushed the rear of the bus off the shoulder and into the embankment.

“Once it was in, I was fighting to get it back out, but once I did, it rolled over,” he said.

He told authorities he knew the road well and had made the trip hundreds of times. In his 20 years of driving buses, he said he never had another accident. He hung up abruptly Wednesday when reached by the AP.

Voges husband, Hubert, told authorities that passengers were tossed around the bus in the moments before it flipped, struggling to hold on, and screamed and cried. He said the trip was on the “bucket list” for his family, and he and Shelley Voges planned to return home the following day. He declined Wednesday to say more by phone.

Friends have said Shelley Voges, 53, was a sweet and compassionate woman.

Sheriff’s officials noted that all of the seat belts were in the locked position and didn’t appear to be worn. The driver was wearing a seat belt.

Two lawsuits have been filed against Comedy On Deck Tours over the fatal crash.

Four surviving passengers allege negligence on the part of the tour bus operator and Griep. The company has denied the allegations, according to court records, and didn’t return a call Wednesday from the AP.

Justin Zarcone, a Las Vegas attorney who represents the tour bus company in one of the lawsuits, declined comment Wednesday.

Tour Bus Crashes On New York State Thruway, Injuring 57

NEW YORK (AP) — A tour bus bound for Niagara Falls ran off the New York State Thruway and rolled over in central New York, sending more than 50 people to hospitals, state police and a hospital said.

The bus landed on its side in the grass around 12:40 p.m. along the highway’s westbound side near Weedsport, a village about 25 miles west of Syracuse. Some 57 people were aboard, and all were taken to hospitals for injuries ranging from minor to serious, state police said.

It’s unclear why the bus veered off the road, troopers said in a news release. Police photos showed papers and other items strewn around the white bus as it rested on the road shoulder.

Authorities said that the driver of the JTR Transportation bus has been identified as Fermin Vasquez, 66, of Wingdale, New York. JTR is based out of Poughkeepsie, New York. State police said Vasquez was among those taken to a hospital but didn’t elaborate on Vasquez’s injuries.

The bus was headed to Niagara Falls from Poughkeepsie in the Hudson Valley, local reports said.

Juan Gil-Urrego told WSYR-TV that his mother, Ximena Urrego, had arranged the trip, one of many she has planned to various destinations as affordable excursions for residents. The bus left around 6 a.m. on one of the last trips Urrego had organized for the summer, he said.

A few children were aboard, one possibly just a month old, Gil-Urrego said.

His mother was taken to a hospital with glass in her eye and needing back surgery, her son said, but he was just relieved she had survived.

“It’s a miracle,” especially since another relative survived a bus crash in the past, he said. “This time around, it hits me twice as hard to know that it happened to both of them already, and both of them made it.”

Auburn Community Hospital received about 27 patients, including Vasquez, and was assessing their conditions, spokesperson Matthew Chadderdon said. Three were transferred to Upstate University Hospital in Syracuse. Upstate is the area’s top-level trauma center, a hospital especially equipped to handle serious injuries.

Upstate received 25 patients in all, spokesperson Darryl Geddes said. He couldn’t immediately provide information on their conditions.

One westbound lane was closed until early evening, and traffic was backed up in one direction for eight miles (nearly 13 kilometers) at one point, the state police said. Authorities issued a request on social media for anyone who witnessed the crash to contact investigators.

Associated Press news associate Joshua Housing in Munster, Indiana, contributed to this report.

At least 1 dead, nearly two dozen injured after tour bus rolls over on highway

At least one person is dead and nearly two dozen others injured after a tour bus traveling from Canada to New York City rolled over on a highway, officials said.

The incident occurred shortly before 1 p.m. Friday on Interstate 87 between Lake George and Warrensburg in upstate New York, officials said.

One person was killed in the crash, a state official confirmed to ABC New York station WABC .

Approximately 22 people with various injuries were treated by emergency responders, including air medical services, according to Lake George EMS .

FlixBus, the bus operator, said one passenger had serious injuries and 10 other passengers and the driver sustained non-life-threatening injuries.

There were 23 people on board the bus, including the driver, FlixBus said.

The tour bus originated in Montreal and most of the passengers were Canadian, according to Warren County Administrator John Taflan. Most passengers are believed to be adults, he said.

FlixBus said the long-distance bus was traveling on its scheduled route from Montreal to midtown Manhattan when the crash occurred.

New York State Police are investigating the cause of the rollover, Taflan said.

FlixBus said it is working closely with authorities and its transport partner to "determine the exact circumstances of this accident."

"At all times, the safety of passengers and drivers is of the highest importance to FlixBus," the company said in a statement. "FlixBus is deeply shocked about what happened and sends its deepest condolences to the victim's relatives and friends and express its full support to the passengers and drivers affected by this accident."

I-87 southbound is closed between exits 23 and 22 due to the "serious crash," state police said.

"I join New Yorkers in praying for all involved in this horrific incident & am grateful for the heroic efforts of our first responders," New York Gov. Kathy Hochul said in a statement on social media.

‘The loudest noise:’ Musician speaks after wrong-way driver crashes into tour bus on Loop 101

PEORIA, AZ (3TV/CBS 5) — Victims on board a tour bus spoke to Arizona’s Family after being struck by a drunk driver on the Loop 101 in Peoria early Sunday morning.

DPS troopers tell Arizona’s Family that officers received a report around 3:45 a.m. of a wrong-way driver headed southbound in the northbound lanes of Loop 101 between Thunderbird Road and Bell Road. Within 2 minutes, a head-on crash was reported between a Chevy Silverado and a band tour bus hauling a trailer. Authorities say the wrong-way driver, later identified as Felix Perez, 27, of Jurupa Valley, California, had serious injuries and was rushed to a hospital, where he later died. Meanwhile, two people on the bus were rushed to the hospital with moderate and minor injuries, while five others were treated on the scene.

Arizona’s Family learned the bus carried members of the band Piano Men: Generations, who perform in Billy Joel and Elton John tribute concerts. Terry and Nick Davies, the father and son duo said they had just wrapped up a three-day tour in California.

Terry hit his head, blacked out for a couple of seconds and woke up at least eight feet away from where he was before the crash. He was taken to the hospital and needed seven stitches to close up a cut above his right eyebrow. He was released a couple of hours later.

The other person who was hospitalized is a close friend of Terry and Nick’s. They’re also an artist but not a part of the band. That person was ejected from the bus and is in a hospital with a concussion. We’re told he is expected to be okay.

“Family is everything. Please don’t take it for granted,” said Nick. Like the majority of the passengers, he was asleep and woke up when the driver suddenly hit the brakes. “Then I heard the horn and felt the crash. It was the loudest noise. The loudest noise,” he said. “Then my first thought is, ‘Where’s my dad? Is he okay? And I just heard him yell ‘Help!’” said Nick.

“It could’ve been a very different story, you know? We’re just so grateful,” said Terry. Both Terry and Nick said they are keeping the wrong-way driver’s family in their thoughts. “They’re going through a different kind of hell than we are, much worse. We are hoping they can find some peace with it,” said Terry.

Double-decker bus and city bus collide in Manhattan, sending 18 people to hospitals

NEW YORK — A crash involving a double-decker tour bus and a city bus in Manhattan Thursday evening sent 18 people to hospitals for treatment, fire and EMS officials said.

Reports of a major vehicle crash came in just after 7 p.m., Deputy Chief Kevin Murphy with the New York Fire Department’s Division 1 Unit said. Emergency personnel found what he described as a serious accident, with the larger bus adding complications.

“We’ve had a few minor challenges in the double decker bus — going through the windows, taking people out,” New York EMS Division 1 Deputy Chief Paul Hopper said.

Ladders and ropes were required to get some people down because of crash damage to the bus, Murphy said.

Many of those injured have cuts, bruises, scrapes, along with suspected fractures and head and neck injuries, Hopper said.

“None have any life-threatening injury,” said Hopper, who added that about 63 other passengers requested evaluations by a doctor who was at the scene.

Hopper and Murphy said they couldn’t speculate about what led up to the crash.

“I heard the lady next to me scream, so I looked up and I saw this bus barreling towards us,” Ishrak Jahan, a passenger on one bus, told CBS News New York. “I just saw glass everywhere for a second. It was honestly like I was in a movie ... I saw blood. I immediately called 911.”
