Travel sickness is the feeling of nausea and general malaise which may occur when travelling in a car, boat or plane.

The feeling occurs due to conflicting signals between the vestibular system and the brain. The vestibular system is the part of the body which senses balance, orientation and motion.

When the vestibular system detects motion, but no motion is perceived by the eyes, the conflicting signals trigger the vomit centre in the brain. Likewise, if motion is perceived by the eyes but not sensed by the rest of the body, such as with VR headsets, the same result can occur.

Depending on a person's sensitivity to motion, symptoms can vary. Symptoms of travel sickness can include:

If you experience travel sickness, avoid eating heavy meals before travelling. Try to abstain from reading, watching films or using your phone and tablet while in a moving vehicle as these can worsen symptoms.

Avomine contains Promethazine Teoclate, which blocks histamine receptors in the chemoreceptor trigger zone (vomit centre) of the brain, to treat nausea and vomiting.

When the inner ear receives information that the body is in motion, it relays it to the brain which, in turn, sends signals to the visual receptors to determine if motion is perceived.

If these messages do not match - if the eyes can't see the motion that the inner ear is reporting and vice versa - the brain becomes confused and releases chemicals to induce vomiting.

Children over the age of 5 can take Avomine, but it must not be taken for more than 7 days.

For adults, swallow one tablet whole with a glass of water an hour or two before travelling to prevent travel sickness. Avomine is effective for up to 24 hours.

You can also take Avomine to treat travel sickness by taking one tablet as soon as you feel symptoms of travel sickness, and take another tablet later in the evening of the same day.

What should I do if I forget to take Avomine when I should?

If you forget to take Avomine tablets before travelling, take your usual dose as soon as you remember or when you feel symptoms.

Do not take more Avomine tablets to make up for the forgotten dose.

What should I do if I take too many Avomine tablets?

Avomine Theoclate works by stopping histamine from binding to its receptors in an area in the brain known as the vomiting centre. The vomiting centre is responsible for causing feelings of sickness and for the vomiting reflex. It is activated when it receives nerve messages from the vestibular apparatus in the middle ear.

The vestibular apparatus provides constant feedback to the brain about the position of our body. When something disturbs the vestibular apparatus, such as movements of the head when travelling by boat or car, nerve signals are sent from the vestibular apparatus to the vomiting centre. This can cause the symptoms of motion sickness, such as nausea, dizziness or spinning sensations (vertigo) and vomiting.

By blocking the histamine receptors in the vomiting centre, Avomine Theoclate prevents disturbances in the middle ear from activating the vomiting centre.

Avomine is is a histamine H1 antagonist that can be used for it's ability to induce sedation, reduce pain, and treat allergic reactions. Avomine's effects generally last 4-6h but can last up to 12h. Patients should be counselled regarding CNS and respiratory depression, reduce seizure threshold, and bone marrow depression.

Avomine

Avomine Theoclate Tablets are an anti-emetic (anti-sickness drug) which helps to prevent, and treat nausea and vomiting, including travel sickness, and vertigo. Avomine Theoclate contains promethazine which belongs to a group of medicines called phenothiazines. Your doctor may prescribe this medicine for giddiness or light-headedness (vertigo), or for sickness after an operation, and in such cases the tablets should be taken as instructed by your doctor.

How Avomine works

Avomine is a an antagonist of histamine H1, post-synaptic mesolimbic dopamine, alpha adrenergic, muscarinic, and NMDA receptors. The antihistamine action is used to treat allergic reactions. Antagonism of muscarinic and NMDA receptors contribute to its use as a sleep aid, as well as for anxiety and tension. Antagonism of histamine H1, muscarinic, and dopamine receptors in the medullary vomiting center make promethazine useful in the treatment of nausea and vomiting.

Avomine dosage

To prevent travel sickness on short journeys : Avomine Theoclate should be taken at least one or two hours before travelling. Adults and children aged over 10 yearsshould take one tablet. Children aged 5 to 10 years should be given half a tablet.

To prevent travel sickness onlong journeys : Avomine Theoclate should be taken at bedtime each night during the journey, startingthe night before you travel.Adults and childrenaged over 10 yearsshould take one tablet.Children aged 5 to 10 years should be given half a tablet.

To treat travel sickness :Adults and children over 10 years should take one tablet as soon as you feel sick, followed by a second tablet the same evening. Take a third tablet the following evening if necessary. Children aged 5 to 10 years should treated in the same way, but with half a tablet each time.

For sickness and vomiting due to other causes, and for treating vertigo : Adults and children aged over 10 years should take one tablet up to three times aday. Children aged 5 to 10 years should be given half a tablet up to three times a day.Alternativelyyou should follow the instructions given by your doctor.

Side Effects

Medicines and their possible side effects can affect individual people in different ways. The following are some of the side effects that are known to be associated with promethazine. Just because a side effect is stated here does not mean that all people using Avomine Theoclate will experience that or any side effect.

  • Drowsiness or feeling tired.
  • Restlessness.
  • Nightmares.
  • Blurred vision.
  • Difficulty passing urine.
  • Feeling disorientated.
  • Unexpected increase in excitability in children.
  • Increased sensitivity of the skin to sunlight (photosensitivity). If affected you should avoid sun lamps and direct sunlight while you are taking this medicine.
  • Loss of appetite.
  • Awareness of your heartbeat (palpitations).
  • Abnormal heartbeats (arrhythmias).
  • Low blood pressure (hypotension).
  • Abnormal movements of the hands, legs, face, neck and tongue, eg tremor, twitching, rigidity (extrapyramidal effects).
  • Blood disorders.

The intraperitoneal LD 50 in rats is 170mg/kg and in mice is 160mg/kg. The subcutaneous LD 50 in rats is 400mg/kg and in mice is 240mg/kg. The oral LD 50 in mice is 255mg/kg.

Patients experiencing an overdose of promethazine may present with mild central nervous system and cardiovascular depression, hypotension, respiratory depression, unconciousness, hyperreflexia, hypertonia, ataxia, athetosis, extensor-plantar reflexes, convulsions, dry mouth, flushing, gastrointestinal symptoms, and fixed, dilated pupils. Treat overdoses with symptomatic and supportive treatment, which may include activated charcoal, sodium sulfate, magnesium sulfate, controlled ventilation, diazepam, intravenous fluids, vasopressors, norepinephrine, phenylephrine, anticholinergic antiparkinsonian agents, diphenhydramine, barbiturates, or oxygen.

Avomine Theoclate tablets can make some people feel drowsy, dizzy, confused or disorientated and this may reduce your ability to drive or operate machinery safely. Do not drive or operate machinery until you know how this medicine affects you and you are sure it won't affect your performance. If your child is affected in this way they should avoid potentially hazardous activities such as riding bikes. You should not leave your child unattended after giving them this medicine.

Avoid drinking alcohol while taking Avomine Theoclate because this is likely to make the above effects more likely or worse.

Avomine Theoclate tablets may make your skin more sensitive to sunlight than usual, so you should avoid exposing your skin to direct sunlight or sunlamps until you know how your skin reacts. If you can't avoid strong sunlight you should use a sunscreen lotion or make sure your skin is protected with clothing.

This medicine may interfere with some pregnancy tests, causing false negative or false positive results.

If you are due to have any skin prick tests to diagnose allergies you should stop taking Avomine Theoclate at least 72 hours before the tests. This is because antihistamines can prevent or lessen the skin reactions that indicate an allergy, and so can make the test results unreliable.

Interaction

It is important to tell your doctor or pharmacist what medicines you are already taking, including those bought without a prescription and herbal medicines, before you start taking Avomine Theoclate. Similarly, check with your doctor or pharmacist before taking any new medicines while taking Avomine Theoclate, to make sure that the combination is safe.

Avomine Theoclate should not be taken by anyone who has taken a type of antidepressant called a monoamine oxidase inhibitor (MAOI), eg phenelzine, tranylcypromine, isocarboxazid, in the previous 14 days.

You are more likely to feel drowsy if you take Avomine Theoclate with any of the following (which can also cause drowsiness):

  • antipsychotic medicines, eg chlorpromazine
  • barbiturates, eg phenobarbital
  • benzodiazepines, eg diazepam, temazepam
  • sedating antihistamines, eg chlorphenamine, brompheniramine, hydroxyzine
  • sleeping tablets, eg zopiclone
  • strong opioid painkillers, eg morphine, codeine, dihydrocodeine, tramadol
  • tricyclic antidepressants, eg amitriptyline.
  • Avoid alcohol. Alcohol may increase the sedation caused by promethazine.

Avomine Alcohol interaction

[Moderate] GENERALLY AVOID:

Concurrent use of ethanol and phenothiazines may result in additive CNS depression and psychomotor impairment.

Also, ethanol may precipitate dystonic reactions in patients who are taking phenothiazines.

The two drugs probably act on different sites in the brain, although the exact mechanism of the interaction is not known.

Patients should be advised to avoid alcohol during phenothiazine therapy.

The volume of distribution of promethazine is approximately 970L or 30L/kg.

Elimination Route

A 25mg dose of intramuscular promethazine reaches a C max of 22ng/mL. Intravenous promethazine reaches a C max of 10.0ng/mL, with a T max of 4-10h, and an AUC of 14,466ng*h/mL. Oral promethazine is only 25% bioavailable due to first pass metabolism. Oral promethazine reaches a C max of 2.4-18.0ng/mL, with a T max of 1.5-3h, and an AUC of 11,511ng*h/mL.

The elimination half life of promethazine is approximately 12-15h.

The intravenous clearance of promethazine is approximately 1.14L/min. The renal clearance of promethazine is 5.9mL/min and the renal clearance of promethazine sulfoxide is 90.4mL/min.

An intravenous dose of promethazine is 0.64% eliminated in the urine as the unchanged parent drug, 0.02-2.02% in the urine as desmethylpromethazine, 10% in the urine as promethazine sulfoxide.

Pregnancy & Breastfeeding use

If you are pregnant you should not take Avomine Theoclate without consulting your doctor first. Avomine Theoclate should only be used during pregnancy if essential, and only if the expected benefit to the mother outweighs any potential risks to the developing baby. It should not be used during the last two weeks of pregnancy, because if taken during this time it may cause irritability or excitement in the baby after birth. Seek further medical advice from your doctor.

Small amounts of Avomine Theoclate may pass into breast milk. This is not expected to be harmful to a nursing infant with occasional short-term use of Avomine Theoclate, but there is a risk that it may cause drowsiness, irritability or excitement in a newborn baby. If you are breastfeeding you should not take Avomine Theoclate without getting medical advice from your doctor first.

Contraindication

  • Children under five years of age.
  • People with reduced awareness, slow reactions or extreme drowsiness caused by medicines or illnesses that reduce activity in the central nervous system.
  • People who have taken a type of medicine called a monoamine-oxidase inhibitor antidepressant (MAOI) in the past 14 days.
  • Children and adolescents who have signs and symptoms suggestive of Reye's syndrome.
  • People who are allergic to any ingredients of the tablets.
  • Avomine Theoclate tablets contain lactose and should not be taken by people with rare hereditary problems of galactose intolerance, the Lapp lactase deficiency or glucose-galactose malabsorption.

Avomine Tablets 25mg contains the active ingredient Promethazine theoclate, which is an antihistamine mostly used for preventing travel sickness and nausea.

What can you use Avomine Tablets for?

It can also be used in the treatment and prevention of:

  • Vestibular disturbance

How does Avomine Tablets Work?

Avomine Tablets 25mg can provide up to 24 hours relief in one dose and is generally well tolerated and can be used by children from 5 years. The active ingredient, promethazine, works by blocking histamine from binding to receptors in the body. It blocks histamines in a part of the brain known as the “vomiting centre” which is activated when it receives nerve messages from the ear.

The vestibular apparatus in the middle ear is constantly providing feedback to your brain about the position of your body, especially when you are travelling by boat or car. This is what causes symptoms of motion sickness when travelling and leads to nausea, vomiting, dizziness, and vertigo (spinning sensations). In order to prevent this, you can take Avomine tablets one to two hours before travelling. If you are going to go for a longer journey, you should take Avomine tablets at bedtime, the day before starting out.

  • This product should not be used in children under 2 years of age because safety of such use has not been established.
  • This medication may cause drowsiness. If affected do not drive a vehicle or operate machinery.
  • Avoid alcohol.

Usage / Instructions

How to take Avomine Tablets:

  • Adults and children aged over 10 years: Take one tablet.
  • Children aged 5-10 years: Give half tablet.
  • Children aged 2-4 years: Give quarter tablet.

This product should not be used in children under 2yrs of age because safety of such use has not been established. For the relief of travel sickness the above dosage should be taken promptly and repeated nightly if required.

  • For long journeys: Take tablet at bedtime on the night before you travel.

Nausea and vomiting due to other causes

One 25mg tablet at night is often sufficient, but two or three tablets are sometimes necessary. Alternatively, more frequent administration such as 25mg two or three times a day may be required for some patients. It is often not necessary to give more than four of the 25mg Avomine Tablets in 24 hours.

In the above indications children over 10 years of age may be given the lower adult doses described above. Children between 5 and 10 years may be given half the adult dose. Tablets are not suitable for administration to children aged between 2 and 5 years. An oral liquid preparation is recommended in this age group. Not for use in children under 2 years of age.

One 25mg tablet at night is often sufficient, but two or three tablets are sometimes necessary. Alternatively, more frequent administration such as 25mg two or three times a day may be required for some patients. It is often not necessary to give more than four of the 25mg Avomine Tablets in 24 hours.

In the above indications children over 10 years of age may be given the lower adult doses described above. Children between 5 and 10 years may be given half the adult dose. Tablets are not suitable for administration to children aged between 2 and 5 years. An oral liquid preparation is recommended in this age group. Not for use in children under 2 years of age.

Avomine is a medicine which helps to prevent and helps to stop travel sickness. It is believed that travel sickness is caused when the brain receives different messages from the eyes, the stomach and the ears which seem to be inconsistent or conflicting.

As a result, you feel sick (nausea) and may vomit. Avomine works on the brain by blocking those messages which make you feel sick and which cause vomiting.

Your doctor may, however, prescribe Avomine for giddiness or light-headiness or for sickness after an operation, and in such cases the tablets should be taken as instructed by your doctor.

One 25mg tablet at night is often sufficient, but two or three tablets are sometimes necessary.

Alternatively, more frequent administration such as 25mg two or three times a day may be required for some patients.

It is often not necessary to give more than four of the 25mg Avomine Tablets in 24 hours.

If you suffer with asthma, bronchitis, glaucoma, epilepsy or heart, liver, kidney or bladder problems tell your doctor or pharmacist before taking these tablets.

If you have severe headaches with blurred vision, severe stomach pains with vomiting, dizziness, difficulty with keeping your balance, ringing in the ears or other ear problems as well as feeling sick, you should telI your doctor or pharmacist before taking these tablets. This is because the effects of the tablets may make it more difficult for them to find at why you are unwell.

If you have a bad cough, please consult your doctor or pharmacist before taking Avomine tablets.

If you are pregnant, think you may be pregnant or are breastfeeding your baby, you should not take Avomine tablets unless they have been recommended for you by a doctor. If you are breastfeeding your baby, Aventine tablets may cause your baby to be irritable and excited.

If you are taking medicines to help you sleep, or for depression, mental illness, anxiety, crampy stomach pains, Parkinson's disease, or bladder problems, Avomine may not be suitable for you. Ask your doctor for advice. You should tell your doctor or pharmacist if you are taking any other medicines or if you are seeing another doctor or attending hospital for any reason.

If you are having a pregnancy test or skin tests for allergy, do not take Avomine tablets as they interfere with the results; if you are having a skin test you should stop taking the tablets at least 72 hours before the skin tests.

These tablets contain sodium metabisulphite which can cause allergic reactions in some people. Sodium Metabisulphite is found in some medicines, foods and drinks. If you get a rash, itching or difficulty breathing or worsening of your asthma after taking something known to contain this chemical, please consult your doctor or pharmacist. These reactions are mom common in patients with a history of allergy or asthma.

While taking these tablets, you should avoid drinking alcoholic drinks as the tablets will add to the effects of alcohol.

These tablets may make you feel dizzy, sleepy, disorientated or confused or cause you to have a headache. You should not drive or operate machinery for the first few days, and until you are sure that you are not going to be affected by these unwanted effects.

A trip to Moscow

I'm going to organize a trip this summer. I want to plan everything. I’ll fly from London to Moscow, and I also want to visit St. Petersburg. Could you, please, advise me what places I should visit? I’ll stay in Moscow for about 10 days before going to St. Petersburg. Thank you!

I don't really see the point of shunning a country because you don't agree with its leader and/or its government. Would you go to Italy? China? Tourist dollars are more likely to get into the hands of the people rather than any corrupt bureaucracy and meeting friendly tourists might just change the locals opinions of foreigners and even give them some information they don't get from local news media.

Hopefully , I won't be starting a war here , but I must say I agree with Chani's comments . Having taken three year visas in 2015 , we have been to Moscow and St; Petersburg ( once to Moscow , and twice to St Petersburg ) . The History , Art , Music ( I am a classically trained musician ) and Culture , fascinate us . And while leaders of such countries can point to foreign visitor numbers as a sign of support ? I also believe that personal safety for certain travellers, especially obvious minorities, cannot be guaranteed, as fervent nationalism increases, and tolerance levels evaporate. Just as I may decide against a purchase at the supermarket because of a company’s behaviour, ethics will always be a consideration.

Well, I'd argue supermarket ethics does not readily translate to travel ethics - the latter is far more complex. Personally, I see the whole "vote with my hard-earned travel dollar" approach as overly simplistic, but if it's your position - I totally respect it.

What I don't understand is members volunteering ethical guidance in response to what seems an innocuous request for travel-related information.

I appreciate Rick Steves' point of view in his "Travel As A Political Act". That said, Putin's Russia is not a place I would support. But, to each his or her own....

From a practical point of view, travel between the two countries can be difficult. I understand the Russians who traveled in the US this past winter had to go to the US Embassy in the country of Georgia to get visas because the Russians expelled American embassy personnel. Due to the Russian government-imposed reduction on U.S. diplomatic personnel in Russia, the U.S. government has reduced ability to provide services to U.S. citizens.

We have traveled to Moscow and St. Petersburg over the past few years. As in the US, most of the people we encountered helpful and not particularly political. In Moscow, we stayed at The Garden Ring Hotel which was a close walk to the metro. The hotel was very comfortable and quiet for being in the city. We loved the art museums and the Georgian restaurants. In St. Petersburg, we stayed at both The 3Mosta hotel near the Hermitage and the Art Hotel Demetra.. In St. Petersburg, we especially liked the Hermitage and Russian museum. Learning the Cyrillic alphabet and having a good map is a must.

Either read a good travel book or book a guide who can personally take you to the best places that interest you and tell about the sights you're seeing. We used Dan Petrov on 2 different trips to Moscow and found him to be wonderfully friendly and witty not to mention super knowledgeable.

Latest advice, Australian Governmemt, 1 April 2018. “Due to heightened political tensions, you should be aware of the possibility of anti-Western sentiment or harassment. While the Australian Government is not aware of any increased difficulties for Australians travelling in Russia at this time, you should follow the security and political situation closely and keep up to date with this travel advice. Remain vigilant, avoid any protests or demonstrations and avoid commenting publicly on political developments.”

We very much enjoyed our group trip to Moscow and St Petersburg last year. excellent guides and accommodations. While Steven who has replied here has excellent advice, ( and we took advantage of it), we felt an organized tour really facilitated our enjoyment of the country. You may want to consider a combination of an organized tour and than adding independent days . Some of the Australians on the tour had arrived a couple of days earlier than the tour stated and then some stayed later. If we were to go again (which I would), that"s how I would do it. And I must say , it was enlightening to have conversations with the Russian tour guides and the non Americans about the current world situation. Not that we went into any great depth, but I think we all went away with a little more understanding. Top places: allow lots of time in St Petersburg at the Hermitage- take a separate day for the Impressionist building. In Moscow, the Russian Art Museum was amazing.

Solid advice from Australian government quoted above, especially the keeping up to date part.

I checked the current version (Sep 18) of that advisory - and it hasn't been updated much. In fact, some sections don't seem to have changed since 2005 - their recommendations for importing/exporting currency and financial instruments, for example, are at least five years behind.

I do like reading governmental travel advisories for entertainment purposes - for instance, up until very recently the US one sternly advised potential travelers that Moscow was largely a cash-based economy (couldn't be further from the truth) and warned about intrinsic dangers of gypsy cabs (which had all but disappeared by 2015).

Is the Trans-Siberian Railway expensive?

Before starting on your Trans-Siberian Railway adventure you naturally want to know what the entire trip will cost. Although this sounds like a simple question, it is pretty difficult to answer. The Trans-Siberian Railway price of travel depends on the following factors:

  • Which travel class do I want to use? The price for a first class ticket is about three times the price of a 3rd class ticket
  • Am I willing to buy the tickets myself and assume responsibility for the organisation of the trip?
  • How many stopovers do I want to make? The more breaks, the higher the total price.
  • What sort of accommodation do I want? Will it be a luxury hotel or will a hostel dormitory be sufficient?
  • What tours and excursions would I like to go on?
  • What is the current exchange rate for rubles?

Basically, everything from a luxury to a budget holiday is available. If you buy yourself a 3rd Class nonstop ticket at the counter, a few hundred Euros will cover the price. All you will experience is a week on the Trans-Siberian train and will see nothing of the cities on the way. There is, however, any amount of room for upward expansion. Everyone makes different choices about which aspects they are willing to spend money on. I personally prefer to save money on accommodation and railcar class, visit as many cities and do as many trips as possible. To enable better classification of your travel expenses I have contrasted two typical traveler types. In the third column you can calculate the total cost of your own journey on the Trans-Siberian Railway. Please keep in mind that these are only rough estimations and not exact prices.

The all-in costs seem fairly high at first. However, they cover everything and it is quite a long journey taking four weeks. Many people forget to consider that when looking at the list. We should also deduct the running costs for food and leisure at home. I think most visitors to this page will classify themselves somewhere between the two categories, that is around the € 2,000 – € 2,500 range. When comparing these prices with other travel packages, you get the impression that it is hardly worthwhile travelling individually on the Trans-Siberian Railway. Please keep in mind that most packages last no more than 14 days and you are herded like cattle through the most beautiful locations.

If you spend less time on the Trans-Siberian Railway you will, of course, pay less. I chose this particular travel length because I prefer not to do things by halves. If you fulfill your dream of travelling on the Trans-Siberian Railway, enjoy it and don’t rush things. But it’s up to you, of course. Try playing around with the form a bit to find the appropriate price for your trip.
