Health Self-Assessment for International Travel

To prepare for international travel, carefully consider your existing health conditions in consultation with your medical providers.

Pre-existing health conditions may be exacerbated by changes in elevation, diet, social life, jet lag, time changes, air quality, climate, living arrangements, etc. To help you consider your medical history and create a plan to address your needs while abroad, Global Cornell has created a Health Self-Assessment for International Travel.

Below, you will see a number of health-related questions, which you should answer honestly. You may want to take notes as you go through the list. Later, you can use your self-assessment to facilitate a discussion with your physician. In addition, you may want to discuss your health with your traveling companions or with those familiar with your upcoming travel so they can help you more readily if you should need assistance while abroad.

Do you have any disabilities for which you will need accommodations abroad?

If yes, consider the information found on Cornell's Student Disability Services site and the Department of State's Travelers with Disabilities page or contact [email protected] to discuss options.

Do you have any serious food, drug, animal, or other allergies? If yes, are your symptoms life-threatening?

If you answered yes to either question, consult with your physician to determine what precautionary steps you should take to address your condition. Consider wearing a medical alert bracelet or necklace with details of your conditions. Contact [email protected] if you’d like to discuss the implications of living with this condition in your host country.

Are you on a medically restricted diet?

If yes, it is advisable that you speak to your physician, your program leader/advisor, and any in-country hosts arranging meals. You may also contact [email protected] to discuss options.

Do you plan to take prescription medications while abroad?

If yes, note that not all medications prescribed in the United States are legal or available in other countries. Alternate medications may need to be considered by you and your physician. The Cornell Health Travel Clinic is a great resource to discuss your options and expectations.

Have you been treated in the last five years, or are you currently being treated for any of the following conditions?

  • General: Alcohol/Substance Abuse, Eating Disorder, Immunodeficiency, Severe Migraine, Seizure Disorder
  • Respiratory: Asthma, Tuberculosis
  • Gastrointestinal: Crohn’s Disease, Ulcerative Colitis
  • Infectious diseases: HIV/AIDS, Hepatitis
  • Endocrine: Diabetes
  • Mental health: Anxiety Disorder, Bipolar Disorder, Depression, Obsessive-Compulsive Disorder
  • Other chronic physical or mental health condition(s)

If you answered yes to any of these, please consult your physician to discuss your condition and consider a continuation of care plan.

If you are traveling to a non-English-speaking country, and you have a medical condition, do you know how to describe your condition in the local language?

If not, consider taking with you a written description of your condition in the local language to present, if needed while abroad. If you need assistance in finding a translator prior to departure, contact [email protected] . Also, note that when calling International SOS (ISOS) for a medical referral you can request an English-speaking physician if available.

Are any immunizations required or recommended by the CDC for travel to your destination country?

Find your destination country on the CDC Traveler's Health website . If yes, consult with your physician or make an appointment at the Cornell Health Travel Clinic .

Additional Issues to Discuss with your Physician(s)

  • The advisability of traveling outside of the United States given your overall health and/or medical condition(s).
  • The specific destination(s) and activities you have planned throughout your travel.
  • The effects on you while you are abroad of jet lag; time changes; and potentially dramatic changes in elevation, diet, social life, air quality, climate, living arrangements, etc.
  • Arrangements you should discuss with in-country hosts, program leaders, fellow travelers, advisors, and International Health and Safety, and, Security  prior to departure.

The Travel Clinic

Douglas MacQueen, MD, is the Travel Clinic’s medical director and is board certified by the American Board of Internal Medicine in internal medicine and infectious diseases.

While planning for a three-week trip to India this past fall, one of the first stops Pat Malvik of Ithaca made was at the Travel Clinic operated by Cayuga Medical Associates to learn what vaccines and medical issues to address before beginning her travel.

“I was happy to find this service offered locally,” says  Pat, who thought she might have to make an out-of-town trip to get her vaccinations. “It was very convenient and fast. I got an appointment within a few days and got all the shots I needed in one day.”

The Travel Clinic serving the Ithaca area opened about two years ago at 1301 Trumansburg Road, Suite R, in the medical offices in front of Cayuga Medical Center’s main campus. Douglas MacQueen, MD, is the Travel Clinic’s medical director and is board certified by the American Board of Internal Medicine in internal medicine and infectious diseases. He and the Travel Clinic’s nurses have experience in travel and tropical medicine, wilderness medicine, parasitology, and immigrant medicine, in addition to internal medicine and infectious diseases.

The team provides routine and travel vaccines, malaria prevention, traveler's diarrhea prevention, management, and general advice about keeping healthy during travel.

“When we see patients planning to travel to certain areas, we first review their vaccination history to see if they are up-to-date on routine immunizations for diseases like tetanus, diphtheria, and flu. Then, we’ll check several sources such as the U.S. Centers for Disease Control, the World Health Organization and consulates to see what vaccines are recommended for the countries on their trip schedule,” MacQueen says.

The team can provide individualized services to any travelers who are 16 or older, those with special travel health needs, immunocompromised travelers, pregnant travelers, those preparing to work or live overseas for prolonged periods, and business and adventure travelers.

Among the more common immunizations travelers may need are vaccines for: hepatitis, typhoid (in oral and injectable doses), yellow fever, Japanese encephalitis for travel to south and east Asia, and rabies for travelers who expect to work with animals. Oral medications are prescribed to prevent malaria.

“We also advise travelers on preventing diseases transmitted by mosquitoes and other insects, such as malaria, dengue fever, and Japanese encephalitis, and avoiding insect bites,” MacQueen says.

Common concerns such as prevention and treatment of traveler’s diarrhea and travel-related health problems such as traffic accidents, altitude illness, motion sickness, and jet lag are also reviewed.

“Special care for those traveling during pregnancy, healthy travel for individuals with chronic diseases such as diabetes or  immunosuppressive disorders, and health topics for long-term travelers including adjustment issues, tuberculosis testing, and health-care options abroad are also discussed,” MacQueen says.

Most health insurance plans do not cover the cost of the appointment or vaccinations at the Travel Clinic. Patients can expect an out-of-pocket expense of about $300 for an office visit and routine travel vaccines. The fee can be higher depending on the number and types of vaccines a patient might need.

When travelers return home, the Clinic also provides diagnosis and treatment of health problems related to a trip such as acute and chronic diarrhea, skin diseases, and parasitic diseases including malaria, schistosomiasis, filarial diseases, and Chagas disease.

Getting an appointment

The Travel Clinic schedules most appointments within a few days after a patient calls the office. Patients should bring a list of their current prescriptions, medical and vaccine history, and travel plans to an office appointment. For details, call (607) 241-1118.

Especially for Students Outside of Ithaca

Cornell Health provides services for students in Ithaca, and for Cornell AgriTech students studying in nearby Geneva. 

Students who are studying outside of Ithaca – at Cornell Tech, Cornell in Washington, study abroad programs, etc. – may use our services (with some restrictions) when visiting the Ithaca campus.

Cornell students who are registered in programs outside of Ithaca – but are in Ithaca for an orientation program, or just a visit – may come to see us for in-person care, should you need it.

All Cornell students may use our services when in Ithaca, including:

  • Registered undergraduate, graduate, and professional students – no matter where you will be studying
  • Continuing Education students taking summer courses, or taking 12+ credits during the academic year
  • High school students participating in Cornell’s Summer College program

Restrictions include:

  • Refer to our Insurance & Billing page for information about which insurance plans we accept and paying for care.
  • During the summer, students who are not enrolled in the Summer Session but plan to enroll for fall semester may receive medical care on a fee-for-service basis. They are not eligible for counseling services. 
  • Students on leave from the university are not eligible to use our medical and counseling services, but may use pharmacy, physical therapy, and travel clinic services.

Cornell AgriTech students in Geneva, NY

As a registered student, you are eligible for Cornell Health's full range of medical and mental health services when you’re on Cornell’s Ithaca campus.

  • Cost: Students enrolled on the Geneva campus pay Cornell's Student Health Fee , and have a $10 visit charge for nearly all services at Cornell Health.    
  • Transportation and parking: The inter-campus shuttle provides regular transportation between Geneva and Ithaca. Should you choose to drive, please note that patient parking is limited, so you should leave extra time in case you need to find other parking on campus .

  • Finding a medical / mental health provider that participates with your insurance:  Most Cornell AgriTech students are enrolled in Cornell’s Student Health Plan (SHP) . SHP members will get the best coverage by seeing an Aetna "participating provider" (i.e., "in-network provider") for medical or mental health care – in Geneva, or anywhere in the United States. Refer to the  Using Your Student Health Plan page on the Student Health Benefits website to find local providers that participate with your insurance. If you have private insurance, consult your plan information for instructions on finding providers that accept your insurance. 
  • Urgent care:  If you need immediate care, visit WellNow Urgent Care (1 White Springs Road, Geneva; 315-230-4074 ) or another urgent care center.
  • Emergency care:  In a medical or mental health emergency, call 911, or go to the Geneva General Hospital (196 North Street, Geneva; 315-787-4000 ).
  • 24/7 mental health support:  You can speak with a licensed therapist from Cornell's on-call service any time, day or night, by calling  607-255-5155 . We also recommend these free  hotlines and text services .

Our services include.

Urgent Care Treatment of acute illnesses and injuries. 

Primary Care Preventative care including physical exams, gynecologic exams, contraceptive counseling and treatment, and STI counseling, testing and treatment. This also includes management of chronic health problems including asthma and allergies. 

Allergy Shot Administration Once you have been evaluated and treated by an allergist, we can assist you in administering allergy injections that have been ordered by your specialist. 

Routine and Travel Vaccinations (including flu shots) Flu, Hepatitis, Tetanus and specialty travel vaccines on a case by case basis. 

The Health Center currently does Not offer the COVD-19 vaccine or booster shots.

Mental Health Evaluation Our integrated behavioral health model includes two behavioral health consultants (onsite) and one behavioral health/collaborative care manager (remote).

If you are traveling internationally or returning from a long trip, it’s important that you are up to date on vaccinations and knowledgeable about travel-related health concerns. Access travel medicine services at Emory’s TravelWell Center.

Pre-Travel Vaccinations and Consultations

Whether traveling abroad for business, service or pleasure, taking the proper precautions will help you have a safe, trouble-free, and successful journey. Our providers are recognized leaders in travel medicine. They can provide the appropriate travel vaccinations and prescriptions for your journey. They can also address any health-related questions or concerns about your trip.

Post-Travel Care and Tropical Diseases

Infections acquired overseas can be unusual and unfamiliar to physicians in the U.S. It’s critical—and often life-saving—to be evaluated by an expert in tropical diseases. Our infectious disease and tropical medicine-trained physicians have years of experience providing state-of-the-art care for those not feeling well after travel. Our affiliation with Emory University and collaborations with the CDC give us access to the country's most advanced diagnostic testing available.

Pre- and Post-Travel Physical Exams

Many international programs require physical exams and testing before travel. We provide these services and test for various infections among travelers returning from a long international trip or immigrants traveling from other countries.

How to prepare for your Travel Clinic visit

Schedule your visit six to eight weeks before your travel departure to allow enough time to receive any needed vaccines and allow for immunity to develop before your trip begins. 

Please arrive 15 minutes in advance of your appointment time to allow for check-in. 

Bring your travel itinerary, including dates of travel, regions of the countries you plan to visit, expected accommodations, and scheduled airport layovers.

Bring your current medication list and vaccination records that are not already part of your medical record. If you are a UI Health Care employee, please print your vaccine/immunization record from ReadySet and bring it with you. 

The clinic provider will not be able to determine insurance coverage for your visit, vaccinations, or prescribed medications. We recommend you review the Centers for Disease Control and Prevention (CDC) Travelers’ Health webpage in advance to learn what are likely vaccines and medications recommended for your destination. Use the drop-down menu under the “Destinations” heading to select the country you’ll be visiting and click “go.” You will need the CPT (Current Procedural Terminology) codes listed in the tables below for vaccine information and some other fees when you contact your health insurance carrier to inquire about coverage. Medications commonly recommended for travel are also found in the tables.  

A cost estimator tool for a travel clinic visit can be found within MyChart . However, the tool allows you to only inquire about one vaccine per inquiry. The CPT codes listed below are needed to use the tool.  Note that the estimator automatically includes a flat physician fee for a new patient visit and a hospital fee when a vaccine CPT code is entered. However, the provider charge submitted varies with the complexity/duration of the travel assessment/counseling.

To access the estimator from MyChart, click “Menu” in top left corner, scroll down to “Billing,” and then click “Estimates.” Click “Create A New Estimate,” then type in the desired vaccine or its associated CPT code into the search bar. If you have not signed up for a MyChart account, you can use the estimator as a guest. 

Vaccines that may be recommended and provided during Travel Medicine Clinic visits

Medications that may be recommended/prescribed during travel medicine clinic visits, what to expect during your travel clinic visit.

  • Your provider will review your travel itinerary, planned activities, medical history, medication list, and prior vaccinations. They will also discuss precautions to help you stay well, including recommended vaccines and prescription medications to prevent or treat common or important travel-related illnesses. The vaccines that you want or are required then will be administered, and medications will be prescribed.
  • The visit may last up to 75 minutes, including the travel assessment and counseling and administration of vaccines. If more than one person is sharing an appointment, the total visit may require 90 minutes. If a yellow fever vaccine is administered, 15 minutes of monitoring in the clinic is required.
  • We see adults and children who are age 15 and older. We also see 14-year-old patients who weigh more than 100 pounds.

What to Know When Getting Ready to Travel

ThedaCare Nurse Practitioner Offers Advice to Help Keep Your Summer Vacations Healthy and Safe

Maybe you’ve been planning your summer getaway since winter — you’ve purchased your plane tickets, you’ve reserved a place to stay, and you’ve got your swimsuit packed. There may be a few things remaining on your “to do” list to ensure you have a healthy and safe vacation.

“One of the most important things for people to remember while planning to go on summer vacations, particularly if you’re going out of the country, is to make sure you have with you the prescription drugs that you need,” said  Ashley Crowe, a Family Medicine Nurse Practitioner at Theda Care Physicians-Neenah . “Ensure that you have any needed prescriptions refilled so you have enough while away, and always carry them in their original containers. Medications without labels may be confiscated by customs.”

She also noted that it is recommended to carry prescriptions in a carry-on bag, never pack them in your checked luggage, as sometimes checked bags get lost or delayed.

“You don’t want to be without your prescription medication, particularly if you’re going abroad, where it may be more difficult to refill those medications,” Crowe said.

Preparing Ahead of Your Vacation

Leading up your vacation, it’s also a good idea to schedule an appointment with your health care provider at least a month or two before you travel to ensure you have any needed vaccinations or oral medications for the region where you’re vacationing, Crowe said.

“If you haven’t already received the COVID-19 vaccine, you should get it or the most recent booster whether you’re planning to travel domestically or abroad,” Crowe said.

While most airlines have dropped COVID-19 pandemic masking requirements, the virus is still present and active. Some countries may still have pandemic-related restrictions and require proof of vaccination or negative COVID-19 tests before entry. Travelers can check the  State Department’s website  on country-specific COVID-19 information before traveling.

Depending where you’re traveling, you may need to receive other vaccines or boosters such as Tdap (tetanus, diphtheria, pertussis), MMR (measles, mumps, rubella), influenza, chicken pox, cholera, Hepatitis A/B, rabies, typhoid or yellow fever.

“It’s really dependent on the country where you’ll be traveling and the areas you’ll be visiting,” Crowe said. “That’s why it’s important to let your health care provider know if you’re planning travel abroad.”

Children who are traveling should be updated on all their routine vaccinations, and they should be watched carefully for any changes in their health while traveling, particularly if they’re too young to talk about how they’re feeling, Crowe said.

“The most common health issue kids experience while traveling is diarrhea, and that can lead to dehydration,” Crowe said. “Ensure your kids get plenty of fluids, and if they seem dehydrated, develop a fever or have bloody stools, you should seek local medical attention.”

Travelers, especially those vacationing with kids, should have a plan for where and how to seek medical attention should the need arise. Cruise ships have medical facilities aboard, and international resorts and hotels should have local contacts for medical providers nearby. Know the emergency number for the country in which you are traveling.

Traveling with a health kit that includes your prescriptions, over-the-counter medications, sanitizer and wipes, extra masks, band-aids, electrolyte drink powder, and other items is a good idea, she said. Insect repellent is important to have on your trip as mosquitos and other pests can carry diseases including West Nile virus, encephalitis, malaria and the Zika virus. Sunscreen also is a must.

“People living in Wisconsin may forget that if they travel south, it’s easier to get sunburned faster as the sun’s rays become more intense,” Crowe said. “You should use sunscreen outdoors no matter where you’re traveling.”

Bringing sun-protective hats and clothing can help you avoid sunburn, as can wearing lightweight clothing with long sleeves and pants. If sunburn occurs, cooling the skin with cold compresses or cold water can help, as can over-the-counter gels, moisturizers or lotions, Crowe said.

“Getting a bad sunburn or stomach bug while traveling can ruin an otherwise beautiful vacation,” Crowe said. “Taking a few extra precautions and planning ahead can help you avoid a lot of the common issues you can encounter on a trip.”

Another thing to check if you’re leaving the country: Your health insurance may not cover your medical expenses while traveling abroad, so it’s important to call your insurance provider to see what your health coverage allows. If your insurance provider doesn’t offer a travel option, there are plenty of providers offering short-term travel insurance policies, usually with a one-time premium based on benefits and length of time you’re traveling.
