How to Keep Track of Your Health Information

Methods of tracking your health range from patient portals and mobile phone apps to paper copies.

How to Track Your Health Information

In a perfect world, your health information would start following you from the moment you’re born. Every documented detail – vital signs, doctor visits, medical test results, diagnoses, prescriptions and hospital stays – would trail you like a magnificent magnetic cape, a vast health history attached to you and accessible in one convenient spot.

Unfortunately, the world has yet to develop a system capable of tracking the volumes of health data you generate. You’re left to gather it on your own from each provider, stitching together a patchwork of medical minutiae and milestones stretching across the decades.

“It can become overwhelming, and for some it’s seen as an additional burden of disease or care,” says Kelly Batista, executive director of the Brian D. Jellison Cancer Institute at Sarasota Memorial Health Care System in Florida.

How do you tackle the challenge? It boils down to four methods.

Use Patient Portals

Patient portals are secure websites that allow you to access your medical information with a particular provider – such as a doctor’s office or a hospital – 24 hours a day.

“Those portals include information about your medical history, test results and interactions with the provider. They also allow you to communicate with providers, ask questions and schedule appointments,” Batista says.

Portals also enable you to:

  • View notes your doctor has written during or after your visit.
  • Download copies of your medical records.
  • See which medications your provider has prescribed and request refills.
  • Make payments to your provider.
  • Update your insurance information.
  • Take part in telehealth visits (depending on the provider).

On some portals, you can even add your own health observations. “It’s a private space for consumers to document information that isn’t accessible to the care team. For example, you can say what your mood is or add information you obtained from your fitness tracker . It’s a repository where you can keep all of that information in one location,” says Rema Padman, a health care informatics, analytics and operations researcher and Trustees Professor of Management Science and Healthcare Informatics at Carnegie Mellon University .

Use Mobile Health Apps

You can also keep track of your health information with mobile health apps – applications or programs that you download to a smartphone. “There are more than 300,000 health-related apps to track walking, sleep, fitness, diet, chronic disease like diabetes and much more. These apps become useful not only to track health conditions, but also to manage day-to-day events,” Padman says. “We’re also seeing apps and wearables provided by health care providers to monitor patients at high risk for adverse events, so they can intervene early and prevent emergency visits or hospital admissions.”

Many apps enable you to access your entire patient portal, including the same information you’d find on the website version and the same ability to communicate with your medical team, schedule appointments and update information. That can be handy if you’re on the go or you’re at a doctor appointment and need to share information with a provider who doesn’t have access to it.

You may also be able to download your patient portal information to other mobile health apps, such as those that help you manage pregnancy or chronic disease. That puts even more information in one easily accessible place.

Keep Hard Copies

The old-fashioned way to track health information is with hard (paper) copies of your medical records. You’ll need to request them and then pick them up in a provider’s office or have them sent by U.S. mail.

Once you obtain paper records, it helps to keep them organized in folders, boxes or three-ring binders. “For example, a three-ring binder with tabs can easily organize a patient’s medical information in categories such as allergies, medications, diagnostic scans, and lab work. Assembling their own binder allows for patients to easily reorganize and add information as needed. A helpful addition to binders is adding a calendar, which allows patients to keep track of their upcoming appointments and events,” says Alexis Eastes, a patient navigator based in Venice, Florida.

Like health apps, you can bring hard copies of your records to doctor visits for your own reference or a doctor’s review.

Keep Soft Copies at Home

Another way to track health information is by obtaining soft copies – files that exist only on a computer screen.

These files can include copies of medical records that are provided to you by:

  • A CD (an older format still commonly used in medical offices, especially for imaging tests ).

Note that health care providers are required by law to deliver records in an electronic format whenever the provider has the ability to produce that type of copy (not just if it’s company policy). You can also create your own soft copies of medical records by scanning paper records into a digital format. Once they’re scanned, name the files and organize them on your hard drive.

Still another type of soft record is a spreadsheet. Create one on your computer to help you track doctor or hospital visits (include the dates), medications, diagnoses and tests (include the results).

All soft medical records can be loaded onto a USB drive and taken to a doctor appointment for review.

Downsides to Consider

Tracking your health information can have downsides – some minor and some major.

For patient portals and mobile health apps:

  • They aren’t accessible to everyone . You’re out of luck if you’re not tech-savvy or you don’t have internet access via computer or smartphone.
  • They may charge fees. Some health apps charge annual or monthly subscription fees.
  • They aren’t all linked. Unless all of your providers are in one large health care system, it’s unlikely they’ll be able to see information in all of your different portals and apps. “These are piecemeal solutions,” Padman says. “What you do with an app is captured there, but it’s probably not the information that your health care provider or pharmacy has.”
  • They may be vulnerable to hackers. “If there’s a data breach, what are the policies to address those consequences? Would you be alerted?” Padman asks.
  • They may own your information. Companies spend billions of dollars every year analyzing consumer health data, and it’s unclear which organizations do it and who cashes in on your information. “It’s a murky area,” Padman says. “What will they do with your information? Will it be distributed or sold to marketing organizations?”
  • They require a little homework before use. Make sure any health app or patient portal you use is subject to federal privacy rules (it’s more likely among apps and portals from large health organizations and physician offices); proves that it can securely store your data; and states clearly that it won’t use, sell or share information without your permission.

For hard or soft copies of your medical records:

  • Collecting them takes a lot of work. “We recommend that patients get a printout of their visit notes, their lab results or any other diagnostic testing before they leave the office. At least that gives them a running start,” says Betty Long, president and CEO of a nurse advocate group in Flourtown, PA.
  • You may be charged for copies. Fees are allowed to cover only the cost of supplies, labor and postage.
  • You may have to wait for copies. A provider has 30 days to comply with a records request, and in some cases that can be extended to 60 days.

And any type of medical record (even electronic records) can get lost or develop errors if information is entered incorrectly. Stay on top of it by regularly checking and updating your records.

Final Advice

It’s not enough to have medical records stored where you can find them; you must understand what they mean. “It’s a lot of information,” Batista says. “That’s where you have the option through portals to communicate with your provider, ask questions and get feedback. Also, in our program, patient navigators step in with education.”

No matter which method you use to track your information, experts agree that it’s worth the effort. “Everyone has a different way of consuming the data. What’s important is that you find a way that works best for you,” Batista notes. “It’s empowering you as a patient.”

CareZone is a medical record app for your entire family. Unlike MTBC PHR, it doesn’t let you store lab records. However, you can still store everything else from your medical prescriptions to medicines, dosage, and refill information. Instead of typing down medicine name, you can even scan your meds. Additionally, you can also set up reminders to take your meds.

My most favorite feature is the option to store multiple doctor contact information. Along with it, you can even configure your doctor’s appointment dates and refill dates. Moreover, it also lets you take quick journal notes during these appointments and keep track of your progress.

7. CapzulePHR

Similar to CarZone, CapzulePHR is an iOS exclusive medicine recording app for your family. But, in addition to medicines, it also lets you record your blood pressure level, temperature, and weight. It maintains a graph based on these data. Additionally, it’s the only third-party app in this list that can use biometrics. Basically, you can use Face ID to lock/unlock your data.

The most novel feature is a QR Code scanner which creates a summary of your entire medical report. So, anyone with a cam scanner can get your medical summary in seconds. Moreover, the app also supports backing up data to Dropbox and Google Drive.

Health apps track vital health stats for millions of people, but doctors aren’t using the data – here’s how it could reduce costs and patient outcomes

Health-tracking devices and apps are becoming part of everyday life. More than 300,000 mobile phone applications claim to help with managing diverse personal health issues, from monitoring blood glucose levels to conceiving a child.

But so far the potential for health-tracking apps to improve health care has barely been tapped. While they allow a user to collect and record personal health data, and sometimes even share it with friends and family, these apps typically don’t connect that information to a patient’s digital medical chart or make it easier for health care providers to monitor or share feedback with their patients.

As a professor and a researcher in the field of operations management, my current research focuses on improving the efficiency and effectiveness of health care delivery. My colleagues and I recently published a multiyear study showing that integrating a mobile health app with ongoing medical care can significantly improve the health of patients with hypertension – a widespread, serious and potentially deadly chronic medical condition.

But it’s not easy to use health apps this way as a regular part of medical care in the U.S.

Case study: Hypertension

Hypertension, better known as high blood pressure, is one of America’s leading chronic health problems. According the U.S. Centers for Disease Control and Prevention , hypertension was a primary or contributing cause to nearly half a million deaths in 2018 and affected nearly half of U.S. adults – nearly 110 million people. Left uncontrolled, high blood pressure can also permanently damage the heart and other parts of the body.

Simple changes in diet, exercise, smoking and drinking can help prevent or postpone the onset of hypertension. Once a person has high blood pressure, the focus of medical care is treatment and management. But patients typically see their doctors only three to four times a year, making it difficult for physicians to track, assess and address the root causes of their hypertension. These visits also tend to be brief, because the fee-for-service insurance payment model motivates doctors to see more patients in a given day.

Prior studies on the effectiveness of mobile health apps for self-management of hypertension were done in controlled settings in which all the participants agreed to use the app, rather than in clinical settings where patients had a choice of whether to engage with care providers using the app. We wanted to see how a hypertension patient’s use of an app played out in a real-world setting. So one of my study co-authors, a practicing endocrinologist , developed a proprietary web-based smartphone app to help monitor and treat hypertension between office visits.

Patients who received this app free of charge measured and entered their blood pressure and pulse readings. The physician reviewed these readings once a day and, if needed, recommended interventions such as new medications or changing doses of existing medications, or advised on diet and exercise. My co-author and his medical assistants weren’t paid to monitor these patients.

Patients and staff could also talk directly with one another through the app. This enabled regular communication and joint decision-making between providers and patients on how to best treat their hypertension, which in turn encouraged patients not to abandon the app after only a few uses.

In tracking the condition of 1,600 hypertension patients over the course of four years, we found that a typical app user reduced her systolic blood pressure – the upper value in a blood pressure reading, indicating the pressure while the heart muscle contracts – by 2 “millimeters of mercury,” or mmHG, compared with someone not using the app. For patients with systolic blood pressure greater than 150 mmHG, the reduction was more than 6 mmHG. These were significant decreases. A reduction of 10 mmHG in systolic blood pressure lowers overall mortality risk by 13%.

Our study bears out the findings of other researchers who have found that using mobile health apps is beneficial for managing chronic conditions.

Barriers to connection

These finding were promising, but there’s a catch: Many patients can’t afford to buy a mobile device and pay for its service.

Further, medical practices need to make a profit to survive, and there is currently no clear way for a provider to charge for time spent providing medical care via an app. However, the massive switch during the COVID-19 pandemic from in-person care to telehealth has driven many insurers to figure out how to bill for services delivered remotely . These solutions might eventually apply to medical health apps as well.

Developers also have few financial incentives to create apps that integrate with professional health care. Apps commonly earn revenues by converting users into paying customers or ongoing subscribers, or by selling advertising space within the app. The most profitable apps tend to be mobile phone games, which use psychological techniques to increase sales .

But these would be inappropriate in the medical setting. Selling to a patient – or selling the patient’s attention to advertisers – raises questions like: Is my data safe? Is this a medically necessary sale? Does my doctor earn money from this? These concerns could reduce the trust between physician and patient that is essential to treating chronic health conditions.

And the tech has to work. Patients care about how easy it is to use an app, and whether it has the tools that they are looking for. Providers are unhappy when their patients have bad experiences with technology. Further, there are no established standards ensuring that mobile health apps are collecting or delivering accurate information, as there are with most professional medical monitoring equipment.

But as the recognition grows that properly designed health care apps can play a big role in enabling more effective medical care, we could see a major stakeholder step in and provide a monetary incentive for app development and use: the insurance carriers who benefit from lower costs of care.

Ultimately, the key part of treating hypertension and other chronic health problems is the interplay between patient and doctor. Our research shows that a well-designed health app that talks not just to the user but also to the user’s care providers increases the likelihood of that engagement and leads to better treatment and better health.

Sharing is caring! CaringBridge lets you set up your own website to keep people updated on your loved one’s condition in one centralized, secure place. You may have seen families use CaringBridge to keep others abreast of a cancer patient’s condition, for instance. This way, you don’t have to contact everyone individually and can focus on more pressing matters. Don't just track your steps. Here are 4 health metrics to monitor on your smartwatch, according to doctors.

From smart wearables like the Apple Watch or Garmin trackers to the celebrity-sported Oura ring and trendy WHOOP strap , health tech has come a long way from just tracking your steps.

"There's lots of different metrics now that we can begin to look at," says Dr. Davin Lundquist, family physician and chief medical officer at Augmedix. "Anytime that we can have a greater awareness of our health and paying attention to it, it tends to influence behavior in a positive way."

Here are four that doctors say can be useful to monitor:

Sleep 

Most healthy adults can benefit from sleep tracking to some level, says Dr. Carlos M. Nunez, chief medical officer at medical device company ResMed.

"Many users aren't tracking the right information and can end up fixating or misinterpreting the data rather than observing the larger trends that the trackers can help to indicate," Nunez says. "Users should start by tracking their sleep-wake cycle to establish a consistent routine of quality sleep , which research has shown can lead to improved concentration, increased productivity and feeling overall more positive."

Tracking your heart rate can give you a picture into your heart health. The lower your resting heart rate, within reason, the healthier your heart is, explains Lundquist. 

"If you're doing more aerobic exercise, over time, your resting heart rate should decline. And that would be an indication that your heart is getting healthier," he says. 

Respiratory rate

"Certain devices can also provide insight into potential key health indicators - such as your respiratory rates, activity level and more," Nunez says. "For some users, the data can also indicate how your body is responding to stress."

Respiratory rate is a metric that may alert someone to other health issues, too. That's something Michael Snyder, a Stanford School of Medicine professor who has studied smartwatches, experienced firsthand after coming down with COVID-19 . Though he took a COVID test that came back negative, his own research app alerted him to sudden changes in his breathing and heart rates.

"I listened to my COVID test, and I should have listened my smartwatch," he told CBS News  in a 2022 interview.

Cardiac rhythm

Tracking metrics like cardiac rhythm may help alert patients to a bigger problems.  

"I had a patient whose Apple Watch told them that they had a run of atrial fibrillation," Lundquist said. "We got this person into a cardiologist — sure enough, it was confirmed and the patient was adequately treated."

"Afib (atrial fibrillation) is a big deal," cardiologist Dr. Tara Narula told "CBS Mornings" in 2018 as Apple rolled out electrocardiogram technology in its smartwatches to help detect the heart issue. "It affects millions of Americans, increases hospitalization rates (and) increases death and heart failure."

It also increases risk for potentially debilitating strokes by five times, she added.

"The problem with afib is that it can be asymptomatic, so you can be walking around and not know you have it while you're at increased risk of stroke," she explained, noting that a tracking device could help empower patients, but could also lead to false alarm: "Anxiety, false positives, flooding doctors' offices with calls. There are definitely downsides, but I think this has the potential to really be very helpful down the road,"

How accurate is the data?

Health trackers have come a long way, doctors says, but you shouldn't rely on these devices for 100% accuracy or diagnosis. The FDA has specifically warned against any device that claims to measure blood sugar without needles, since inaccuracies could lead to serious health consequences.

For other types of tracking, "The accuracy of some of the smartwatches is still a little bit in question, although with each generation they're getting better. So I think overall, physicians are getting more confident in trusting these devices," Lundquist says. Plus, as he points out, users also have to take off the device to charge, meaning data won't be recorded 24/7. 

While trackers can be a "valuable tool for many," Nunez says the data shouldn't be used to diagnose serious sleep or health issues.

"Ultimately, sleep tracking devices can help to empower users to set and achieve health goals but are not a substitute for formal diagnosis or professional medical care," he says. 

They can also help physicians partner with their patients, Lundquist says.

"As these applications become more mainstream, the ability to show up in your with your doctor's appointment, pull up your phone and show them your metrics would be a great way for us to partner with our patients and help them see where there's potential opportunities or problems," he says.
