The government of India has introduced medical visa for foreign nationals across the world seeking specialty medical treatment in India for extended periods. A visa-on-arrival system for tourists from few selected countries has been instituted by government of India, which allows foreign nationals to stay in India for 30 days of medical procedure.

Maximum of two attendants who are blood relatives are permissible to accompany the patient under separate Medical Attendant visas, the validity of the Medical Attendant visa will be the same as the patient Medical Attendant Visa. The preliminary duration of medical visa is up to a year of duration of the treatment, whichever is less. The visa allows maximum of 3 entries during 1 year.

A medical category visa may be issued with the following conditions:

  • Every medical document is checked by Indian Missions and posts that are placed abroad to confirm whether the request to obtain a medical visa is genuine or not.
  • One may also need to provide satisfactory details to The Missions that they have sought initial medical advice in the country of residence/origin and consequently has got an advice to go for special treatment.
  • One can get a Medical Visa to India only for treatment in reputed and recognized medical centers. Although a medical visa to India is given for a number of treatments but there are prime ones including radio therapy, neuro-surgery, heart treatments, renal disorder, plastic surgery, organ transplant, congenital disorders, plastic surgery and many more.
  • This type of visa should be granted for seeking medical attendance only in reputed/recognized specialized hospitals/treatment centers in the country. Although non-exhaustive, following illustrative list of ailments would be of primary consideration, serious ailments like neuro-surgery, ophthalmic disorders, organ transplantation etc. the basic idea would be that the Mission may satisfy about the need of the foreign national to come to India for medical treatment/health enhancements.

Eligibility

The medical visa is based on certain conditions that are outlined above, but it is required to ensure that people who are immigrating to India are genuinely doing so for the purpose of receiving treatment. Indian Immigration Authorities require the following criteria to be satisfied due to the above mentioned reasons:

  • It should be clear that the initial medical guidance has already been made by the applicant in their own country and they are then recommended to pursue specialist treatment.
  • It should be demonstrated by the applicant that he/she is seeking medical attention only in a recognized institution which specializes in the treatment of the conditions.
  • High priority is given to ‘M’ visa applications in India that are associated with several medical conditions. One serious ailments are given primary considerations such as neuro surgery, heart problems, ophthalmic disorders and organ transplantation.

Validity of Visa and Extension of Visa

The initial period for such a visa may be up to a period of one year or the period of treatment whichever less, which can be extended for a further period up to one year by the State Government/FRROs on the production of medical certificate/advice from the reputed/recognized/specialized hospitals in the country. Any further extension will be granted by the Ministry of Home Affairs only on the recommendations of the State Government/FRROs supported by appropriate Medical documents. Such visa will be valid for maximum of three entries during one year. State Government/FRROs may be permit one additional entry in emergency situation if required.

Registration

Foreigners coming on ‘M’ Visa will be required to get themselves registered mandatorily well within the period of 14 days of arrival with the concerned FRROs/FROs.

Visa to Attendant/Family Members

Attendant/family members of the patient coming to India for medical treatment shall be granted miscellaneous visa co-terminus with the ‘M’visa of the patient. Such visa may be granted the spouse/children or those who have blood relations with the patient. However, not more than two attendants may be allowed at a time for grant of miscellaneous visa. Such visa will be called ‘MX visa’. Such foreigners are also required to get themselves registered with the local FRROs/FROs well within 14 days from the date of arrival.

  • The medical Indian visas only provide entry for a particular purpose of obtaining medical attention.

  • The Government of India permits a period only up to 12 months but it can be further extended to 12 months if required. Physicians as medical tourism facilitators in Nigeria: ethical issues of the practice

Globalization through the internet provides an opportunity to market services, including health care, across the world. Marketing health care services over the internet and social media is an increasing practice carried out by medical tourism facilitators (MTFs) so as to reach a wider audience ( 1 ). Traveling from one country to another to seek health care services has been termed medical tourism. In addition to marketing health care services, MTFs mediate between medical tourists and potential caregivers at medical tourist destinations ( 2 ). The internet remains the medium by which potential medical tourists learn about the services offered at possible medical tourist destinations several miles away. Occasionally, advertisements by MTFs contravene the laws of their target countries. An earlier paper from Nigeria identified that the code of medical ethics of the country did not allow in-country medical practitioners to advertise the services rendered at their practices ( 3 ). However, this code of ethics was regularly violated by MTFs marketing health care services over the internet and social media.

Medical tourism is thriving in some low- and middle-income countries, which serve as destination countries for medical tourists. Particularly, India, Thailand, Malaysia, South Africa, and Turkey are among the world’s most prominent medical tourist destinations ( 4 - 6 ). The growth of medical tourism has been a result of the cost of service and lengthy waiting times in developed countries and the absence of specialist services, elitist ideas, and loss of faith in the quality of health care services rendered in developing countries ( 3 , 7 - 9 ). The global medical tourism industry has been estimated to be worth over 100 billion dollars and projected to grow at an annual rate of between 20% and 30% ( 10 ).

Nigerians with an estimated annual spending on medical tourism of between one and 20 billion US dollars are undoubtedly major contributors to this industry ( 11 , 12 ). Many Nigerians travel to countries such as India and the UK for various treatments including cardiac surgeries, neurosurgeries, cosmetic surgeries, orthopedic surgeries, and renal transplant surgeries ( 13 , 14 ). In 2005, the wife of a serving Nigerian president died after undergoing cosmetic surgery in Spain ( 15 ). The current president of the country traveled to the UK in June 2016 in order to attend to “a persistent ear infection” ( 16 ). This was despite the availability of over 250 ear, nose, and throat specialists and a national ear hospital in the country ( 16 ). Several other prominent Nigerians have reportedly received health care services in various countries and for various ailments.

While medical tourism could provide access to health care services that are not available in departure countries, several issues such as cost of service, follow-up after surgery, quality of care, and adverse outcomes are challenges that have plagued the industry ( 2 , 13 ). Not all services provided to medical tourists are of the quality advertised. About 25% of medical tourists who presented in an Indian hospital for care regretted seeking care at this health facility and were unlikely to recommend the practice to their peers ( 14 ). Some of the services that medical tourists seek are not ethically allowed in their home countries where they will return upon completion of a procedure, thereby creating a follow-up conundrum. In addition, the risks associated with services are toned down or never mentioned to the medical tourists by MTFs ( 2 ). Such observations are not particular to Nigeria as a source country. A study recently reported that nine (39%) of 23 patients who presented at a health facility in Nigeria after receiving neurosurgical care outside the country died from complications of the procedures they had undergone. Upon return to the country, over a quarter of these patients presented with infections necessitating follow-up care that was not initially planned ( 13 ) and that incurred unplanned expenses, which pushed the cost of care to astronomical levels. Medical tourists, in their bid to seek care, are exposed to infectious microbes that are uncommon in their native environments, thereby facilitating the transfer of these infectious agents across geographic boundaries ( 17 ). Nosocomial infections they acquired abroad can cause devastating outbreaks in their home countries where inherent immunity to such microbes is non-existent.

Patients with medical conditions that are treatable in Nigeria are being referred abroad because of better information on availability of services over the internet and social media, whereas in-country health facilities are held back by the code of medical ethics, which prohibits physician advertisements ( 18 ). A report highlighting a young lady who lost her life in India where she had gone to seek orthopedic care that could have been provided locally was recently published ( 19 ). Though it was noted that the procedure could have been carried out in the country, it is possible that the late patient and her family were unaware of it because of limited information and the absence of advertising opportunities by the local health practices.

The wording of the code of medical ethics as understood by the author suggests that alongside physicians who are restricted from advertising their services, health practices are similarly restricted, although an Assistant Registrar of the Medical and Dental Council of Nigeria stated in his public lecture delivered in 2015 that, unlike individual physicians, health facilities were not barred from advertising their services.

Recognizing the financial viability of Nigeria as a source of medical tourists, MTF agencies have moved into the country to have direct access to the population. The trend now also includes physicians, who are approached with a request to refer patients to specific practices outside the country and in return receive some financial inducements ( 20 ). Little has been documented about physicians acting as MTFs or their agents around the world. While referrals to more advanced centers have been a worldwide practice of service delivery, international referrals and patient recruitments are increasing with globalization and might be open to abuse. Thus, we examined the ethical implications of this practice from a critical point of view.

Consequences of Physicians Acting as MTFs

The role of physicians in guiding clients to make the best decisions for their care is a long-held practice. However, knowledge asymmetry has been exploited by physicians to increase their financial gain in several situations, which have led to increased rates of cesarean section births and other unnecessary procedures consequent to information imbalance ( 21 , 22 ). Supplier-induced demand (SID), which results from this knowledge asymmetry, has been well-studied for decades. Changes in policies that favor a particular system have been observed to concomitantly lead to a shift and increased delivery of the services promoted by the physicians who offer these services to the patients ( 21 , 23 ). The consequences of physicians trying to maximize their profits is of significant concern as medical tourism is growing and physicians are approached to become MTFs ( 24 ).

The engagement of physicians in Nigeria to recruit medical tourists raises ethical concerns, which could lead to abuse if not addressed. There are concerns that physicians are being given financial inducements based on the number of patients they refer to medical tourism centers in different countries ( 20 ). These financial inducements could becloud the judgment of these physicians, who may be referring patients for procedures that are not necessary or that could ordinarily be performed within the country, negatively impacting the health system and causing undue hardship for the medical tourist who has to pay huge bills out of pocket. The model of economic benefit to the referring physician, directly linked to the amount the physician is able to generate for these institutions, is a reason for concern. Besides the economic hardship that such decisions may cause to the patient, there are also other social and medical consequences to which the patients are unduly exposed. This includes the psychological effect on the patient who has to travel to an unknown country for (un)necessary care, the possibility of infections that are not easily susceptible to antibiotics in his/her local environment, and the challenge of follow-up after surgical procedures performed outside the country. Engagement of physicians to refer patients to a specific medical tourist destination is a significant conflict of interest that needs to be appropriately reviewed by the authorities.

The absence of a regulatory framework for medical tourism is a major drawback to the MTF profession, which challenges the norm ( 2 ). MTFs view their ethical responsibility to a potential client as being equal to that of the client’s physician ( 2 ). Many MTFs interviewed in an earlier study identified the potential conflict of interest between their business objectives and the good of the patient ( 2 ). Those interviewed were non-clinicians who were in the business for a financial gain, but who were also concerned about the practice of the growing unregulated profession. The trend of physicians doubling as MTFs, as promoted by MTF agencies, raises a significant conflict of interest. Without appropriate checks, medical tourism will be susceptible to SID. MTFs have been signing liability waivers with their clients in case of adverse outcomes. Should a physician referring a patient wish to do likewise, it would be deemed unethical.

Most physicians in developed countries oppose medical tourism because of the ill effects that may arise as a result of these exposures ( 2 ). Likewise, few studies that have been published on the topic in developing countries also antagonize the trend of medical tourism because of negative effects occasionally observed in the patients and the overall effect on the health system ( 3 , 13 ). However, physicians acting as MTFs can be a strategy to get their individual buy-in and thereby alleviate their antagonism through direct engagement and financial inducements. Such a step would constitute a moral corruption of the physicians.

The role of a physician in advising a potential medical tourist can often be complex ( 25 ). Notwithstanding, many patients do not seek medical advice before traveling for medical tourism. In Australia, only 40% of medical tourists consulted a physician before embarking on a medical tourism journey ( 10 ). When they did, the physicians were sometimes the last point of call after a potential medical tourist had already made plans for care abroad. In such a situation, it would be necessary for the physician to critically appraise the need for travel and, while maintaining the autonomy of the patient, provide advice on the pros and cons of making such a trip. The involvement of physicians as MTFs may help address some issues that have been raised by Crozier and Baylis ( 25 ) in having a better perspective on the type of care available to their patients at distant health facilities, which are otherwise unavailable in their home countries. However, the conflict of interest must be appropriately managed.

While medical tourism needs to be better regulated by authorities in departure countries, there is a need for health care providers and health system in these countries to focus on increasing the quality of services provided to clients at their practices. It will help improve their confidence in the health system and foster utilization of locally provided health care services ( 8 ).

Increasing use of the internet and social media for marketing health care services by MTFs and the engagement of physicians as MTFs has created a new ethical challenge in medical tourism departure countries. Having physicians as MTFs is a major moral and ethical issue and the absence of a legal framework makes such a professional relationship open to abuse. There is an urgent need for the regulation of medical tourism agencies and physicians who act as MTFs in Nigeria and elsewhere in order to protect patients from abuse. Proposals for control of this practice may include making it a crime for a physician to refer a patient to a foreign health facility for financial gain and the enactment of legislation making it possible to sue a provider over the quality of care at a referral center. Departure countries also need to invest more resources into their health care systems and develop more skilled human resources in order to discourage medical tourism.

Reversing medical tourism in Nigeria

With the increasing population of Nigerians exiting the country to seek medical treatment abroad and the huge impact on the nation’s economy, it has become a matter of urgency to revamp the health sector to edge the ugly trend.

Tens of thousands of Nigerians travel every year to the United States of America, (USA), United Kingdom, (UK), India, Thailand, Turkey, France, Canada, Germany Malaysia, Singapore, Saudi Arabia and China, among other countries for treatments ranging from kidney transplant, open heart or cardiac surgeries, neurosurgeries, cosmetic surgeries, orthopedic surgeries, eye surgery and other health conditions, to the extent of delivering babies over there.

It is estimated that Nigeria loses about $1.3 billion to medical tourism yearly, which has caused a huge burden on the nation’s economy.

The inadequacy of Nigeria’s health system has been traced to the thriving foreign medical tourism due to poor service delivery, poor funding, unavailability of human capital, poor infrastructure and absence of specialist services, among other flaws.

And according to the World Health Organisation (WHO), Nigeria is currently ranked 187 out of 190 countries in the health index.

While the country is suffering from the alarming rate of medical tourism, brain drain has also been linked to the ugly trend as the best physicians are leaving the country to seek better paying job and working environment abroad, especially the US and Canada where they would be respected.

The Secretary of the Board Trustees, Nigerian-American Medical Foundation (NAMF), Dr. Adeyinka Shoroye in an interview with The Guardian said according to the a count done by the foundation with the American Medical Association (AMA) in 1995, Nigerian doctors by birth in the US was about 1,200, noting that by April 2018, another count was done, which showed an increase of 4,500.

He stressed that human capital; infrastructure, poor quality of education and funding have contributed immensely to the poor state of the country’s health system.

“Misplaced priorities, no proper funding for health care budget, no education, in many universities we don’t have enough faculty for medicine the teaching staff are inadequate and they are overstretching the system to get the best training, this is human life and medicine.

“Basically, people are leaving the country because tertiary care is not doing very well, it has failed due to poor funding and the educational standard has gone so bad in the last three decades, the effect is what we are seeing.

“Failed primary healthcare, uncoordinated tertiary health care, corruption and mismanagement, of course, we are all seeing the effect now. Those who can afforded treatment go to Europe, North America or South Africa which has a very good infrastructure and those that can not afford it stay in the country. People are leaving the country, because they want to get better healthcare at the super-tertiary level. We all know the massive outflow of doctors who left the country in the last two decades practices somewhere in the North America and the whole idea is, even those who don’t want to come back home, if there is a way for them to come in and voluntary, especially the area people are leaving for tourism, which is the super-tertiary care, then we can fill that gap, which is the whole idea of reversing medial tourism,” he said.

He recalled that during the days of the former president of Nigeria, Gen. Yakubu Gowon, the National Health Development Plan I was developed, adding that funding in the universities was sufficient, which included the medical schools, which are Ahmadu Bello University (ABU), University of Nigeria Nsukka (UNN), University of Lagos, University of Ibadan, University of Ife, that produced excellent graduate, according to him, who later left the country.

Shoroye lamented that with the way the population is growing, policy makers at the local, state or federal level should be intensify efforts at addressing the issue.

“Nigeria’s population is a scary phenomenon, we are not preparing for the future, and we are now in the second decade of the 21st century, 2018. By the years 2050, the UNDP is saying Nigeria’s population will be the number three in the world. Right now China is 1.4billion people, India is 1.3billion, US is 325million, Indonesia is 270 million, Brazil is 210million, Pakistan is 207million, with the figures we are having now Nigeria is 198milllion, which is almost 200million; we are getting close to Pakistan, we are number seven and we will overtake Pakistan, which is very close as well as Brazil, Indonesia and US, that means by 2050 Nigeria will be the third most populous country globally.

“And we are not planning infrastructure, education and human capital. Infrastructure can even be the number two, you can have any fine structure but what about the people, the human capital that will be able to sustain… so those are the things we are looking at, it may not be in our own time but let us look at fairness to the next generation coming that it is a challenge they would have to face. Population is ticking time bomb, even if the population is growing are people sitting down to look at statistics and planning well for the future, even the state of the economy.”

He added that most of the countries in the world with the best human indices in healthcare have high standard of living, which has led to the lowest maternal and infant mortality and very high life expectancy.

Proffering solution to the ravaging problems, Shoroye said NAMFI is working towards strengthening the primary health care in the country from the local government wards, making it stronger and very effective, adding that once that is done, the secondary and tertiary healthcare level would have lesser problem.

“If we make the primary health care stronger there will be less problems for the secondary and tertiary health care level. Of course there is super-tertiary which is what we are dealing with in this foundation.

“The human capital is a luxury and we are all glad that this is a golden opportunity to tap into that. Yes they don’t live here, some might not want to come back, but at least, they can be volunteers and rotate periodically. If we have 12 experts in one particular narrow sub-specialty, they can pick a month each, come home two or three weeks do consultation, it might not be the best, but it is better than nothing.

He stressed that Nigeria accounts for the highest burden of non communicable diseases, adding “The figures we have now from WHO, state that the developed world has a 47-per cent increase in the last few years, in the developing country 60 per cent increase and that is the problem, we are not just battling with the old problem, we have made some strides in communicable and infectious diseases, remunerations have made big difference in the last century, but in this 21st century one of the challenges we have now is the NCDs, and what ways are we going to screen and address them, it is at the primary health care level, at least that is the challenge we are going to face for a while in the next generation of this part of the world and we are not taking them seriously.”

He lauded the federal government’s effort in revitalizing 10, 000 PHCs across the country urging the current policy makers to take it serious, adding “if you strengthen primary care, it will become less problem for secondary and tertiary health care level and also for the super-tertiary, but when you have ignored that one, from uncontrollable hypertension to kidney transplant and now they are looking for funds to travel to do transplant.

“The problem start from prevention which could be done at the primary care level by strengthening it, so it is laudable, but there are other things behind the scene that we need to be aware of between the kind of living of the people with those indices.”

On what NAMFI intends to do to stem medical tourism in Nigeria, Shoroye said the foundation would invest in human capital by engaging the Nigerian doctors in the diaspora to render medical service here in Nigeria, either periodically by medical missions or volunteers, adding that it would help the country for a long time.

“We are hoping that we can be able to attract them to come and work here. We have a huge human capital in front of us to be able to move forward, which is a big plus,” he added.

On sustainability, he said, “it is not going to be easy, it is a huge population, at least we have to start somewhere, even if it is just a single referral centre, it is a good beginning, we can’t solve everything in one day, but it is better than doing nothing because it is easy to start something and stop, but is it better to sustain it. These are going to be driven by data and we will get there.”

“We had this 10-year master plan for infrastructure. The challenges are there. One thing we have carefully done is this donor mentality, looking for foreign aid, we are not going to be looking for funding from any foreign bodies, and we have the wherewithal to put things in place. The 10-year master plan, hopefully by 2024, it would be achieved.”

Nigerians spend $11 billion on medical services abroad in 10 years

Over the past 10 years, Nigerians have spent the sum of $11.01 billion on health-related services abroad. This is according to a review of the Central Bank’s balance of payment data.

It is important to evaluate the effect of the huge capital flight recorded by Nigeria over the years, as it goes a long way in determining the level of our foreign reserve viz-a-viz exchange rate. According to the data, Nigerians spent $11.01 billion on health expenses abroad between 2011 and the first quarter of 2021, recording the highest in 2019 with $2.56 billion foreign health expenses, and the least in 2016 at $17 million.

Quality health is a fundamental right of all Nigerian citizens and a basic necessity for survival and productivity. This makes the health sector a crucial component of the economy; one that ideally, should attract massive investment. Unfortunately, Nigeria’s health sector bleeds from multiple besetting problems, one of which is underfunding. Averagely, the sector has received 4.72% of the country’s fiscal budget in the last 10 years.

Although wealthy Nigerians are both willing and financially capable of seeking quality healthcare services abroad, it is costing the nation a small fortune and further compounding the FX pressures that Nigeria grapples with. While Nigeria records significant capital flight to fund health-related services abroad, it records no inflow for reciprocal services in Nigeria as the nation’s health sector leaves much to be desired in terms of service delivery, availability of adequate medical infrastructure and sufficiency of skilled professionals.

Nigeria’s healthcare problem

According to the Nigerian Sovereign Investment Authority (NSIA), Nigerians spend an average of $1 billion annually on medical tourism for a range of healthcare needs. 60% is reported to be across four key specialities: oncology, orthopaedics, nephrology, and cardiology.

A PwC survey of Nigerians found that more than 90% of respondents associated advanced healthcare delivered in Nigeria with “low quality.” Each new report of poor clinical care has reinforced the belief among the population that Nigerian healthcare is substandard and should be avoided whenever an alternative can be found.

In terms of contribution to economic growth, Nigeria’s health sector, valued at N484.76 billion in real terms as of 2020, only accounted for a meagre 0.69% of the economy.

A cursory look at the country’s budget allocation to the health sector revealed that only 4.05% of the national budget was allocated to the federal ministry of health in 2021, 76% of which was budgeted for recurrent expenditure, while capital expenditure took the remaining 24%.

The data also shows a systematic decline in the proportion of allocations to the health sector over the past 10 years, as it declined from 5.58% in 2011 and 6.03% in 2012 to 4.05% in 2021.

While Nigeria’s healthcare system continues to record massive drawbacks, countless Nigerian citizens are excelling in the medical profession abroad. Two cases in point are Nigerian-born medical doctor, Professor Iyalla Elvis Peterside who was named as one of America’s best physicians for the year 2020 by the U.S National Consumer Advisory Board. Similarly, Dr Onyema Ogbuagu a U.S-based and Nigerian born doctor was one of the scientists who discovered the Pfizer vaccine for the covid-19 disease.

Why do Nigerians go abroad for healthcare?

In 2019, Nigeria’s health minister, Osagie Ehanire while representing President Buhari at the second National Health Summit of the Nigerian Medical Association (NMA) faulted the poor attitude of health workers across the country stating that it is a major reason for the growing culture of seeking medical care abroad.

He opined that Nigerians have lost confidence in the country’s health system because of the poor service delivery in Nigerian hospitals. Despite alluding to the fact that lack of equipment and medical technical expertise has contributed to the capital flight, he admitted that the attitude flaws of healthcare practitioners contribute the most.

“If we must be honest to ourselves, we must admit that even more than lack of equipment and medical technical expertise, poor housekeeping and sanitation and often poor and disrespectful attitude of health workers and also a perceived lack of confidentiality are damaging the image and reputation of the health system and also the public confidence in it,” he said.

Earlier this year also, Senator Aishatu Dahiru Ahmed representing Adamawa Central at the red chamber cited under-funding, weak facilities and infrastructure, poor motivation of health workers, low budget, weak accountability, conflicts with the political structure of the states and industrial strike actions as major problems bedevilling the Nigerian health sector.

She also mentioned that an average of 20,000 Nigerians travel each year to India for medical-related services due to the absence of a solid healthcare system at home.

In a chat with a Nigerian healthcare worker, Mrs Emilola Eniodunmo, she addressed the lack of adequate facilities and low morale on the part of the health workers as a major reason for the poor delivery of healthcare services in the country.

She, however, stated that there are good healthcare service providers in the country, for patients who can afford premium care.

“In some notable private hospitals in Nigeria and even state-owned hospitals, we have executive rooms for patients, VIP treatment room, where special treatment is being given to patients for the right amount,” she said.

She also stated that the dearth of modern medical equipment and technical expertise to manage certain severe disease conditions was a major reason for patients’ choice of seeking medical help outside the country.

Meanwhile, according to the Global Health Workforce Alliance of the World Health Organisation, Nigeria’s healthcare service sector is affected by a number of factors, including:

  • Lack of public and private sector coordination;
  • Commercial pressures in the private sector that lead to poor quality work;
  • Work environments that contribute to low motivation, less-than-optimal productivity, high attrition – especially in rural areas; and
  • Lack of planning based on staffing projection needs, resulting in an overproduction of some categories of health workers at the expense of others.

Bottom line

Evidently, Nigeria’s healthcare sector needs adequate funding to compete with its counterparts abroad and attract foreign investments and patronage. Besides this, the nation also needs to incentivise the medical profession to stymie the ongoing brain drain in the sector as it is no secret that Nigerian medical professionals are leaving the country in droves to seek better working conditions abroad.
