Travel insurance can insure you against a variety of complications during travel, including trip delay, trip cancellation and medical issues. But not all policies are the same, and coverage can vary not only across providers, but across policies themselves.

Let’s take a look at travel insurance that covers COVID-19 quarantine, plus the best COVID-19 travel insurance.

What does travel insurance usually cover?

Travel insurance can provide peace of mind on vacation, especially when things go awry. There are varying types and levels of coverage for travel insurance, though these are common inclusions for travel insurance policies:

There's a lot of variety when it comes to travel insurance and the types of incidents it’ll cover. The cost of your premium will vary according to the plan you select.

Cancel for Any Reason insurance allows you to cancel your trip and recoup your costs, no matter why you’ve chosen to cancel.

Before buying travel insurance, check to see if any of your credit cards offer trip insurance . Some travel credit cards offer this insurance free of charge when you use your card to pay.

The Chase Sapphire Preferred® Card , for example, provides primary rental car insurance when you charge the car to your card.

The Platinum Card® from American Express , meanwhile, will cover trip cancellation, trip delay and lost luggage insurance. Terms apply.

There are no credit cards whose benefits include medical insurance. You’ll want to read the terms carefully to be sure that any travel insurance you purchase covers instances of COVID-19.

Finding insurance that covers COVID quarantine

In the beginning of the pandemic, many insurance policies covered losses related to COVID-19, including trip cancellation and medical issues.

Since then, some providers have chosen to exclude coverage for coronavirus-related issues. So you’ll need to search specifically for an insurance policy that covers COVID-19.

Fortunately, there are still insurance providers that’ll provide coverage in the event you’re affected by COVID-19, including:

Trip cancellation.

Trip delay.

Medical care/hospitalization.

Quarantine.

Several countries — like Thailand — actually require that you purchase this insurance before traveling.

If you already have an insurance provider in mind, take a look at their coverage options — and any available add-ons — to see if COVID-19 quarantines are covered.

If you aren’t already committed to an insurance company, sites such as SquareMouth, a NerdWallet partner, will allow you to search for and compare policy coverage from multiple companies at once.

You’ll be asked to provide a variety of information, such as your destination, dates of travel, age, and whether you’d like to be reimbursed for cancellations.

Once you’ve found a provider whose policy includes COVID-19 coverage, you’ll want to make certain that quarantine will also be reimbursed. Reading the policy in full will tell you; it can look something like this:

How much will COVID quarantine travel insurance cost?

The price of travel insurance is going to depend on many factors, including the length of your trip, your age, your destination and your coverage limits.

As you’d expect, the more comprehensive your coverage and the higher your limits, the more expensive your policy will be. The same is true if you’re heading out of the country for an extended period of time.

When comparing insurance policies, you’ll want to think about coverage limits as well as whether the insurance will provide primary or secondary coverage. Compare these factors against other policies to see which policy is the right fit for you.

Here’s a comparison between two different policies. Although neither of these requires a deductible, one of these is secondary (it pays out after other insurance) and caps out at $15,000 per person in emergency medical expenses.

The other is primary (it pays out before other insurance policies) and covers up to $100,000 per person in emergency medical expenses:

While $15,000 may seem like a lot of money, remember that this total includes all doctor visits, tests and medications — in addition to the cost of your quarantine stay.

If you’re staying somewhere expensive, those costs can quickly add up.

Although some communities, such as New York City, may provide cost-free isolation accommodation, others will leave you to fend for yourself.

Even if staying in a moderately priced hotel, five to 10 days (or more) of isolation can quickly run into the thousands of dollars. You’ll want to be sure your insurance policy can cover this.

I'm planning a lot of trips right now. But, I'm uncertain when and how some countries will reopen their borders to tourists. Even though I'll be fully vaccinated soon, I still may want to avoid pandemic hotspots.

As such, I'm only booking travel that is fully refundable or changeable . Specifically, I'm redeeming hotel points for stays and taking note of the date by which I must cancel to get my points back. When it comes to airfare, I'm booking award flights with programs that offer changes or cancellations with minimal fees.

But perhaps you want to book (or have already booked) a nonrefundable trip . In this case, you may wonder whether travel insurance will cover you if you need to cancel, reroute or postpone your trip. Unfortunately, travel insurance only covers trip cancellation or interruption for specific reasons and situations. And most travel insurance doesn't cover the primary reasons I'd want to cancel or interrupt trips in the COVID-19 era.

In this guide, I'll discuss what you need to know about trip cancellation and interruption travel insurance when traveling during the coronavirus pandemic.

What is trip cancellation and interruption insurance?

Trip cancellation and interruption insurance may provide reimbursement up to a specified amount per person per trip for eligible travel expenses or arrangements that must be canceled or interrupted. However, policies vary on the definition of eligible reasons for canceling or postponing a trip and the types of expenses reimbursed. And most policies will only provide coverage for set reasons.

Most trip insurance policies won't cover many of the reasons you might want or need to cancel or interrupt a trip during the coronavirus pandemic. In particular, most policies won't cover cancellation or interruption even if you can't travel due to border closures or specific entry requirements. Likewise, most policies won't cover cancellation or interruption if you choose not to travel -- even if your reason for not wanting to travel is primarily due to a CDC travel warning , elevated COVID-19 cases or specific entry requirements (such as a lengthy mandatory quarantine or PCR testing ).

However, some policies do cover cancellation or interruption if you are quarantined by a physician for health reasons or contract COVID-19 . In the following section, I'll take a closer look at some of the most popular policies.

Popular trip cancellation and interruption policies

There are two primary ways to obtain trip cancellation and interruption insurance:

  • As a benefit provided by your credit card when you charge eligible travel expenses to the card
  • As a benefit of purchased independent travel insurance

I'll discuss some of the most popular travel insurance policies and the coverage each may provide for coronavirus-related cancellation or interruption in the following sections.

Chase credit cards

Various Chase credit cards offer trip cancellation and interruption protection, including the Chase Sapphire Reserve and Chase Sapphire Preferred Card. To get coverage on these cards, you must charge all or a portion of the trip to your credit card account and/or the rewards program associated with the account.

The trip cancellation and trip interruption benefit offered by these cards may provide reimbursement for eligible travel expenses charged to your account up to $10,000 per covered person and up to $20,000 per trip. However, only select losses that result in cancellation or interruption of a trip are eligible for coverage.

In particular, the primary pandemic-related reasons Chase trip insurance may cover your trip cancellation or interruption are:

  • Accidental bodily injury, loss of life or sickness experienced by you or your traveling companion which prevents you or your traveling companion from traveling on the trip
  • Quarantine (due to health reasons) of you or your traveling companion imposed by a physician or by a competent governmental authority having jurisdiction

However, the insurance offered by these two cards explicitly excludes trip cancellation or interruption for the following reasons:

  • A pre-existing condition or any other event that occurs or commences before the initial deposit date or booking date of the trip
  • Your disinclination to travel due to an epidemic or pandemic

So, you definitely won't be covered by Chase credit cards if you decide not to travel because of an epidemic or pandemic. And even if you contract COVID-19 or need to enter quarantine, your Chase card's insurance may not cover you if the insurance administrator considers the coronavirus pandemic (and not your sickness or quarantine) as the event that caused your trip cancellation or interruption. After all, some travel insurance excludes trips booked after the coronavirus pandemic became a known event.

It's undoubtedly worth filing a Chase trip cancellation claim if you contract COVID-19 or a physician or government authority requires you to quarantine (and either of these events prevents you from traveling on a trip). But, I wouldn't assume I'd get any coronavirus-related trip cancellation and interruption insurance when booking new trips with my Chase cards.

American Express cards

Select American Express cards , including The Platinum Card® from American Express, the Delta SkyMiles® Reserve American Express Card, the Marriott Bonvoy Brilliant® American Express® Card and The Business Platinum Card® from American Express, offer trip cancellation and trip interruption insurance of up to $10,000 per covered trip and $20,000 per eligible card per 12-consecutive-month period.

To be covered, you must charge the full amount of your round-trip travel on a common carrier to your card. However, you can pay with a combination of your card and accumulated points on your card. And you can also pay with a combination of your card and redeemable certificates, vouchers, coupons or discounts awarded from a frequent-flyer program or similar program.

Amex's guide to benefits provides the following definitions of trip cancellation and trip interruption:

Trip cancellation means the cancellation of travel arrangements when the eligible traveler is prevented from traveling on a common carrier for a covered trip on or before the departure of the covered trip. Trip interruption means the interruption of the covered trip either on the way to the point of departure or after departure of the covered trip.

However, Amex's trip cancellation and interruption insurance only covers specific cancellation or interruption reasons. So, as is noted in Amex's trip cancellation insurance COVID-19 notice , you'll likely only get coverage in connection with COVID-19 if you contract COVID-19 or a physician requires you to quarantine for health reasons related to COVID-19 on or before the departure of your trip. Any other reason for trip cancellation or interruption in connection with COVID-19 likely ineligible for coverage.

Purchased travel insurance

Many of the most popular travel insurance providers have published coverage alerts and frequently asked questions pages that provide information about COVID-19 claims. So, if you have travel insurance with a specific provider or are looking to buy travel insurance with a particular provider, you should read any information that the provider has about the coronavirus pandemic and understand how that affects your insurance.

Of course, most independent travel insurance covers more than just trip cancellation and interruption. But, in this section, I'll focus on trip cancellation and interruption. I particularly took a closer look at the information AIG Travel Guard , Allianz Travel Insurance , Seven Corners and World Nomads have posted online about coronavirus-related claims. Here's what I found by looking at this information:

  • AIG Travel Guard : Policies purchased on or after March 11, 2020 don't cover any losses due to COVID-19 quarantine. However, suppose you contract COVID-19 before departure. In that case, you may be eligible for trip cancellation benefits (if you have a confirmed and documented diagnosis and are medically unable to travel at the time of departure due to COVID-19). Likewise, if you contract COVID-19 while traveling and have a confirmed and documented diagnosis, you may be eligible for trip interruption benefits.
  • Allianz Travel Insurance : Plans typically exclude losses for epidemics, including COVID-19. However, you can buy new plans with an epidemic coverage endorsement. And, for a limited time, Allianz is accommodating claims for trip cancellation and trip interruption if you of your traveling companion or family member becomes ill with COVID-19 either before or during your trip.
  • Seven Corners : COVID-19 is treated similarly to other sicknesses. So, you may be eligible for trip interruption coverage if a legally qualified physician determines that you or your traveling companion must interrupt the trip due to sickness. Likewise, you may be eligible for trip interruption or cancellation coverage if you or a traveling companion would be forced into mandatory quarantine for medical isolation by a physician or government (excluding at your return destination city or in any other place following the end of your trip).
  • World Nomads : Policies purchased by U.S. residents may provide coverage for trip interruption if you contract COVID-19 while traveling. You may also be eligible for trip cancellation coverage for pre-paid non-refundable travel expenses if your doctor advises you that you are unfit to travel on your scheduled departure date due to being diagnosed with COVID-19.

None of the policies provide reimbursement if you decide not to travel, even if your reason is because of an epidemic at your destination. And, although Seven Corners may allow you to cancel or interrupt a trip due to mandatory quarantine for medical isolation at your destination, your inability to enter a country or state isn't an eligible reason for trip cancellation or interruption.

However, some providers offer "cancel for any reason" trip insurance . If you purchase this type of policy, you can cancel your trip for any reason -- including deciding not to travel because of an epidemic -- and get back some of the prepaid, forfeited, nonrefundable payments or deposits for your trip. If you're booking nonrefundable travel that you aren't sure you'll be able to take, you should consider purchasing cancel for any reason insurance.

My travel insurance doesn't cover me, what now?

As you can see, most travel insurance policies won't cover you if you decide not to travel due to pandemic hot spots or mandatory quarantine or can't travel due to border closures or restrictions. So, if your travel insurance won't cover you, here are some other ideas to consider.

Call the airline or travel provider

If your travel insurance doesn't cover your canceled or interrupted trip, contact your airline and other travel providers. After all, the airline may have travel waivers that allow you to cancel, delay or change your flights with waived fees -- and sometimes with any fare difference waived as well.

Even if the airline doesn't have a travel waiver in place, you may find an agent who is willing to waive fees -- especially if you have elite status with the airline. But, you may need to wait until closer to your travel dates before the airline offers any waivers or alternate options.

Visit a second destination

Suppose the airline won't offer any solutions, but you don't feel comfortable visiting your original destination. In that case, you could book a second round-trip to a different destination and nest it within your original trip. Of course, you'll want to allow enough buffer between your tickets to reduce the chance that you'll misconnect in either direction.

You may also want to ask the airline how much it would cost to add another leg or two to your original ticket, as then you'd be considered a transit passenger at your original destination. And, as a transit passenger, you may be able to avoid entry restrictions at your original destination.

Consider the risk

If you can visit your destination, you may want to take an unemotional, logical look at your trip and the risks you're likely to face. Determine your risk based on your destination, activities at your destination, immunity , age, potential repercussions of becoming ill and possible consequences of testing positive during your trip . Some travelers are specifically avoiding international travel as long as the U.S. requires testing before return , as they don't want to risk needing to quarantine outside the U.S.

Forfeit the trip

If you've run out of options and still don't want to travel, you can try to recoup as much of your costs as possible. For example, you may be able to retain some value by canceling, paying a change or award redeposit fee. But you may lose nonrefundable costs.

Bottom line

Travel insurance can be helpful. But don't expect much coverage from trip cancellation and interruption insurance if you want to cancel or interrupt your trip for most pandemic-related reasons.

Sure, many insurance policies will provide benefits if you contract COVID-19 or are mandated to quarantine for medical isolation. But, you won't get assistance if you can't travel or don't want to travel due to more common reasons such as entry requirements, quarantine requirements or pandemic hot spots. As such, for the foreseeable future, I recommend booking fully refundable travel or purchasing cancel for any reason travel insurance if you aren't sure you'll take a trip.

Q:  What happens if I test positive for COVID-19 and need to cancel the day before my cruise? A:  Like with any illness diagnosed and treated by a medical professional, if you test positive for COVID-19 after purchasing the plan, you will be eligible to receive a refund to your original form of payment. If you do not have an official medical diagnosis, but feel unwell, you will receive a Future Cruise Credit.

Q:  What if my spouse gets COVID-19 right before the cruise and we can no longer sail? A:  You will be eligible to file a claim for a cash refund if you, your traveling companion or a member of your immediate family is diagnosed and treated for COVID-19, which causes you to cancel your cruise.

Q:  What if my mother, who isn’t sailing with us, gets COVID-19, and we may have been exposed? A:  Princess Vacation Protection has you covered! Even if they are not traveling with you, if someone in your immediate family is diagnosed and treated for an illness, you may request a refund to the original form of payment. If they are ill but are not diagnosed or treated, you may call us and receive a Future Cruise Credit for the cruise fare.

Q:  What if I find out I was exposed to someone either on my way to my cruise or during the cruise, and I end up being isolated in my cabin for part or all of the voyage? A:  PVP provides reimbursement for the days you are confined to your cabin if the confinement is ordered by a medical professional.

Q:  Does PVP cover my medical expenses if I get COVID-19 during my cruise? A:  Yes, PVP provides reimbursement for eligible medical expenses for any illness, including COVID-19.

Q:  What if I want to cancel my PVP coverage? How does that work? A:  PVP is refundable until final payment is due. For new bookings made within final payment, the PVP plan cost is only refundable within 10 days of purchase provided the person hasn't departed on the trip nor filed a claim.

Q:  Am I protected by my Princess Vacation Protection plan if my mother, who is not traveling with me, develops pneumonia and I have to cancel my trip to care for her? A:  Yes! Princess will refund the cancellation fees (up to the full value of the cruise vacation) if you have to cancel your trip because of a specified reason such as illness, injury or death of yourself, a traveling companion or either of your immediate family members.

Q:  If I have a heart condition and am on continuing medication, am I protected if my heart condition flares up during my vacation? A:  Yes! Princess Vacation Protection provides reimbursement for conditions that are stable during the 60-days prior to purchase, so if you have had no changes in your health (even if you are on continuous unchanged medication), you would be protected.

Q:  What if my condition did change prior to purchasing Princess Vacation Protection? A:  Rest assured, emergency evacuation benefits are still provided in the event of a medical emergency. Also, if you cancel your cruise vacation and do not qualify for cash reimbursement due to an unstable medical condition prior to purchase, you will have the benefit of cancel-for-any-reason protection from Princess.

Q:  If my pet becomes ill and I have to cancel my trip as a result, will Princess Vacation Protection reimburse me? A:  Although you would not qualify for cash reimbursement, as an added feature for purchasing Princess Vacation Protection, Princess will provide a credit toward a future cruise equal to 75% of the cancellation fees imposed (100% if Platinum Vacation Protection is purchased) if you choose to cancel for an ineligible reason. MOSCOW — Schools, shops and restaurants in the city of Moscow will close next week, municipal officials said, in one of the tightest lockdowns in the Russian capital since the pandemic began, a measure made necessary by low vaccination rates.

Russia’s coronavirus response, like that of many other countries, has seesawed between strict controls and lax enforcement of mask-wearing and vaccination rules. Russia pivoted again this week toward tighter lockdowns as reports of new cases and Covid-19 deaths climbed.

Reported cases are up 33 percent over the last two weeks, reaching more than 32,400, according to a New York Times database . And on average over the past week, 983 people a day died from the coronavirus ; on some days that number exceeded 1,000. Only Russian-made coronavirus vaccines are available in the country, and many people are hesitant to get them. Russia’s vaccination rate of 33 percent of the population is lower than the global average of 37 percent and far behind the rates in most of Europe.

To combat the surge in cases and deaths, President Vladimir V. Putin on Wednesday declared a countrywide “nonworking” week from Oct. 30 to Nov. 7 , extending a regular fall holiday by several days. The restrictions the city of Moscow announced on Thursday were even stricter.

Mayor Sergey Sobyanin ordered schools and nonessential businesses to close two days sooner, starting on Oct. 28. He said in a statement posted online that the spread of the virus had gone “by the worst scenario” and that Moscow would soon be setting daily records for new cases.

Early in the pandemic, Moscow locked down more strictly than many Western countries did. Residents were prohibited from leaving their apartments other than to shop at grocery stores or pharmacies, attend medical appointments or walk their dogs.

The city also lifted many restrictions this year before other European countries did on a wide scale, a move that became a point of pride. Mr. Putin contrasted Moscow’s bustling restaurants with the continuing lockdowns abroad.

Over the summer, Moscow imposed, but then dropped, a requirement that patrons be vaccinated to eat in some restaurants or to visit bars. Enforcement of mask-wearing rules in Russia is generally lax.

The Moscow city government has ordered people aged 60 and over to stay home for four months and told businesses to have at least 30 per cent of staff work from home amid a surge in COVID-19 cases and deaths in Russia.

The new rules take effect from October 25, it said in a statement.

On Tuesday Russia reported 1,015 coronavirus-related deaths, the highest single-day toll since the start of the pandemic, as well as 33,740 new infections.

The Kremlin has ruled out a new nationwide lockdown like the one early on in the pandemic that dealt a heavy blow to the economy and sapped President Vladimir Putin's popularity.

Daily death tolls have been surging for weeks and topped 1,000 for the first time over the weekend amid sluggish vaccination rates, lax public attitudes toward taking precautions and the government's reluctance to toughen restrictions.

Russia boasted about becoming the first country in the world to authorise a coronavirus vaccine in August 2020 — even though it was only tested on a small scale — and proudly named the shot Sputnik V after the world's first satellite, to underline the country's scientific achievements.

While extolling Sputnik and three other domestic vaccines developed later, Russia's state-controlled media chafed at the perceived flaws of Western-made shots, a controversial message that many observers saw as feeding public doubts about vaccines.

Amid widespread vaccine hesitancy, authorities have raised pressure on medical workers, teachers and public servants to get the shots, but the uptake has remained sluggish.

The government coronavirus task force said on Monday that about 45 million Russians, or 32 per cent of the country's nearly 146 million people, had been fully vaccinated.

Mr Putin has underlined the importance of broad vaccination, but emphasised that it should remain voluntary.

Authorities have opened vaccination locations in shopping malls and other facilities outside clinics and tried to encourage people to get the shots with lotteries, bonuses and other incentives, but all those efforts have failed to accelerate take-up.

Mr Putin's spokesman Dmitry Peskov said the government had done everything to make vaccines easily available but added that it should have been more proactive in its efforts to encourage vaccination.

"Obviously, more should have been done to explain the lack of alternative to vaccination," Mr Peskov said.

Asked if the government could authorise imports of foreign vaccines to help boost uptake, Mr Peskov said vaccine scepticism was not limited to domestic shots.

Russia-EU talks on mutual recognition of coronavirus vaccines have dragged on without an end in sight.

While resisting a nationwide lockdown, the Kremlin has told authorities across the country to decide on restrictions depending on the local situation.

Many of Russia's 85 regions have already restricted attendance at large public events and limited access to theaters, restaurants and other venues.

Some have made vaccination compulsory for certain categories of public servants and people older than 60.

Russian case numbers a threat to Europe

On Monday St Petersburg joined other Russian cities in ordering attendees at conferences and sports events to prove their vaccination status via a digital code system from November 1.

Those rules will be expanded on November 15 to make the codes mandatory at cinemas, theatres, museums and gyms, and on December 1 they become mandatory at restaurants, cafes and some stores.

The city has reported the nation's second-largest number of new infections after Moscow.

The government task force has registered a total of more than 8 million infections and its official COVID-19 toll ranks Russia as having the fifth-most pandemic deaths in the world following the United States, Brazil, India and Mexico.

However, state statistics agency Rosstat, which also counts deaths in which the virus wasn't considered the main cause, has reported a much higher pandemic death toll — about 418,000 people with COVID-19 as of August.

Based on that number, Russia would be the fourth hardest-hit nation, ahead of Mexico.

Martin McKee, a professor of European public health at the London School of Hygiene and Tropical Medicine, said the COVID-19 situation in Russia was extremely worrying, not only for Russia but for the rest of the world.

He said even high rates of vaccination elsewhere in Europe wouldn't be able to prevent the virus from being reimported from Russia, particularly if any worrisome new variants emerge.

"Until we have control of the virus everywhere, there's a risk of importation and the pandemic will not be under control," he said, describing the spread of the virus in Russia and the Baltic states as a threat to Europe.

Starting Monday, April 8, 2024, people in British Columbia will have access to a free spring COVID-19 vaccine booster to maintain and extend protection for themselves and their loved ones against serious illnesses from COVID-19.

The spring COVID-19 booster will be available throughout the province at pharmacies, regional health-authority clinics, some primary-care offices, and community health centres. This spring vaccine remains free and available for everyone six months and older.

Given the decreased rates of viral respiratory infections in the community, additional infection prevention and control measures implemented in fall 2023 in health-care settings are no longer required at all times. People are still encouraged to wear masks in health-care settings as appropriate, to continue to cover coughs and stay away from others when feeling sick. Health-care workers will continue to wear appropriate personal protective equipment, such as masks and respirators, in accordance with their point-of-care risk assessments.

While the peak of the respiratory illness season has passed, the SARS CoV-2 virus continues to circulate at lower levels in the community. The protection from infection that COVID-19 vaccines provide decreases over time, particularly in older people, so a spring COVID-19 vaccine booster will ensure people stay protected.

Aligned with advice from the National Advisory Committee on Immunization, B.C. health officials recommend the following people consider receiving an additional dose of the XBB.1.5 COVID-19 vaccine:

  • adults 65 and older;
  • Indigenous adults 55 and older;
  • adult residents of long-term care homes and assisted-living facilities (including those awaiting placement); and
  • individuals six months and older who have been diagnosed as clinically extremely vulnerable (a CEV 1 or CEV 2 condition).

B.C.’s COVID-19 spring program is focused on providing vaccines to those most at risk of severe COVID-19 illness or complications, and people who have not yet received an updated vaccine dose. However, anyone who feels they would benefit from an additional dose of the XBB.1.5 COVID-19 vaccine can contact the call centre at 1 833 838-2323 to book an appointment or walk into a pharmacy where the vaccine is available. Vaccine appointment capacity is available in all regions.

The 2023-24 respiratory illness immunization campaign was launched to the general population on Oct. 10, 2023. As of end of day March 31, 2024, B.C. had administered 1,457,246 doses of COVID-19 vaccines and 1,562,023 doses of influenza vaccines.

  • Invitations will start going out on Monday, April 8, 2024; reminders will be sent out to those who have not yet received an XBB.1.5 dose and to those who fit the listed categories.
  • There are approximately 3.9 million people in B.C. who have not received a COVID-19 XBB.1.5 vaccine.
  • B.C. has more than 900,000 doses of Moderna and Pfizer COVID-19 vaccines (adult, infant and pediatric) in stock and approximately 13,000 doses of the Novavax COVID-19 vaccine in stock for those requiring a non-mRNA vaccine.

Background: The epidemiology, clinical course, and outcomes of patients with coronavirus disease 2019 (COVID-19) in the Russian population are unknown. Information on the differences between laboratory-confirmed and clinically diagnosed COVID-19 in real-life settings is lacking.

Methods: We extracted data from the medical records of adult patients who were consecutively admitted for suspected COVID-19 infection in Moscow between 8 April and 28 May 2020.

Results: Of the 4261 patients hospitalized for suspected COVID-19, outcomes were available for 3480 patients (median age, 56 years; interquartile range, 45-66). The most common comorbidities were hypertension, obesity, chronic cardiovascular disease, and diabetes. Half of the patients (n = 1728) had a positive reverse transcriptase-polymerase chain reaction (RT-PCR), while 1748 had a negative RT-PCR but had clinical symptoms and characteristic computed tomography signs suggestive of COVID-19. No significant differences in frequency of symptoms, laboratory test results, and risk factors for in-hospital mortality were found between those exclusively clinically diagnosed or with positive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) RT-PCR. In a multivariable logistic regression model the following were associated with in-hospital mortality: older age (per 1-year increase; odds ratio, 1.05; 95% confidence interval, 1.03-1.06), male sex (1.71; 1.24-2.37), chronic kidney disease (2.99; 1.89-4.64), diabetes (2.1; 1.46-2.99), chronic cardiovascular disease (1.78; 1.24-2.57), and dementia (2.73; 1.34-5.47).

Conclusions: Age, male sex, and chronic comorbidities were risk factors for in-hospital mortality. The combination of clinical features was sufficient to diagnose COVID-19 infection, indicating that laboratory testing is not critical in real-life clinical practice.
