At least 21 dead after tour bus plunges off overpass near Venice, Italian officials say

A bus carrying foreign tourists including Ukrainians crashed near the Italian city of Venice when it fell from an elevated street Tuesday, killing at least 21 people and injuring 18 others, authorities said.

Four of the injured were in serious condition following the accident in the Mestre borough, on the mainland opposite the historic old city of Venice, said Renato Boraso, a Venice city official. Two of the dead were children, Venice prefect Michele Di Bari said.

Boraso confirmed that some of the victims are Ukrainians, and said the bus was bringing tourists to a camping site.

Venice Mayor Luigi Brugnaro wrote on X, formerly Twitter, that the scene of the crash was “apocalyptic” and that he had already declared the “city's mourning" for the “numerous victims” who were on the bus.

According to local media, the bus fell a few meters before crashing close to Mestre's railway tracks, where it caught fire. Emergency crews were on the scene.

Premier Giorgia Meloni expressed her “deepest sorrow” after the crash.

After the crash, the tour bus rolled from a bridge on the N1 freeway into a river below, said ER24, whose paramedics were on the scene.

"Three people were found deceased by the roadside and 16 down by the river — all were declared dead on arrival," ER24 said in a statement.

"One patient, of the 69 passengers confirmed transported to hospital for various injuries, has since died," it said.

Police divers were searching a river that runs along the highway "for people who might have been swept away," the regional transport department said.

It was not immediately clear how many people the tour bus was carrying.

ER24 said police were investigating the circumstances surrounding the crash and that heavy rains had fallen in the area where it happened.

At least 21 people were killed and 18 were injured when a bus crashed and fell off an overpass near Venice on Tuesday evening, local officials said.

The vehicle broke through the flyover’s guardrails before plunging around 30 feet, local media reported. It landed near railway lines in the Mestre district, where Venice connects to the mainland. The bus caught fire, although the blaze was quickly extinguished, a city official said.

“An apocalyptic scene, there are no words,” Venice Mayor Luigi Brugnaro wrote on X, formerly known as Twitter, adding that he declared a citywide state of mourning.

It wasn’t immediately clear what caused the accident. Renato Boraso, a Venice city councilor, said the 40-year-old Italian driver had “years of experience.” Boraso told Sky Italia television that the driver may have been ill before the crash.

The victims included Ukrainian and German tourists, Boraso said. The bus apparently was on the way to a campground.

Photos of the crash site showed the bus upside down, partly crushed and emitting a plume of smoke. Emergency responders, including firefighters, climbed into the overturned vehicle using ladders and shining flashlights, and carried away victims. Alarms blared and a helicopter flew overhead, videos from the scene showed.

“There are many deaths, too many,” one firefighter said while walking away from the scene, local newspaper Corriere Del Veneto reported. Two children were reportedly among the dead.

Italian Prime Minister Giorgia Meloni tweeted her “deepest condolences” for the victims of the accident. She said she was in close contact with top officials in Venice.

Likewise, Italian President Sergio Mattarella called the mayor of Venice to express his condolences for “the very serious tragedy,” according to a statement on social media from the office of the president.

The incident echoed the disastrous 2018 bridge collapse in Genoa that sent vehicles plunging almost 150 feet during heavy rain, killing 43 people. That tragedy was caused by deteriorating infrastructure, as a more than 200-yard section of the bridge broke apart. Dozens of people, mostly executives and engineers responsible for the bridge’s upkeep, have since faced charges of manslaughter and undermining transport safety.

Nearly 90 people have been killed or injured after a tour bus fell off a bridge into a swollen river when it crashed into a security van.

ER24, the private ambulance company that went to the scene, said police were investigating the circumstances of the crash, which happened around 8pm.

Limpopo was one of seven South African provinces to be hit by widespread flooding in recent days, as the La Nina weather phenomenon hits the country.

President Cyril Ramaphosa declared a state of disaster yesterday, with his office saying homes and vehicles had been flooded and basic infrastructure had been lost.

It was the second time Mr Ramaphosa had made such a declaration in less than a week.

He also invoked the disaster law on February 9, in reaction to a series of crippling energy shortages that have resulted in daily rolling power cuts.

The department said of those hurt in the crash, “ten passengers are reported to have sustained serious injuries, 16 moderate, and 35 minor injuries. Passengers with injuries were transported to Makhado, Elim, and Siloam hospitals for medical attention.”

The cause of the accident is still under investigation.

Heavy rains have been falling in the area.

Deadly crash between "Ride the Ducks" vehicle, bus in Seattle

SEATTLE -- An amphibious tour vehicle and a charter bus carrying foreign college students collided on a busy Seattle bridge Thursday, killing four people and sending dozens to hospitals in a wreck that scattered crumpled metal and broken glass on the road.

Seattle Fire Chief Harold Scoggins said 12 people were in critical condition and many others had minor injuries. The crash also involved two passenger cars.

Mayor Ed Murray said foreign students were on the charter bus, and efforts were being made to contact consulates. He had no other details.

North Seattle College confirmed to CBS Seattle affiliate KIRO-TV that 45 students and staff from its international program were in the crash.

School spokeswoman Melissa Mixon said in a statement that the uninjured students and staff were taken back to campus, where counselors were on hand. Mixon said the school is trying to determine more information.

Witnesses described hearing a loud screech before the wreck and then seeing injured people either lying on the pavement or wandering around in a daze.

Jahna Dyer, a registered nurse, said she was walking across the Aurora Avenue bridge when she came upon the scene, a mess of jumbled metal and glass.

Some victims were lying on the road, while others milled about seemingly in shock and falling down, she said.

Dyer jumped a railing separating the sidewalk from the roadway and helped stabilize an injured man's neck. She said she also helped a woman who had a cut lip and glass in her eye.

"She was holding my hand and saying thank you," she said.

John Mundell said he was at the south end of the bridge when the crash occurred.

"We could hear the screech and twisted metal. It was surreal," he said, adding he saw what appeared to be a few dozen people on the ground. "I wanted to try to help. I felt helpless."

One of the vehicles involved was an amphibious, military-style bus operated by a tour company called "Ride the Ducks." The tours are known for exuberant drivers and guides who play loud music and quack through megaphones as they lead tourists around the city.

In a statement, Ride the Ducks of Seattle offered condolences to the victims' loved ones.

"Our company is fully cooperating with all law enforcement agencies and the first responders who are at the scene now trying to help the victims, get them prompt medical attention and eventually return traffic in the area to normal," the company said.

Seattle Fire Lt. Sue Stangl said emergency crews were quick to arrive at the scene and encountered several victims.

"When (firefighters) arrived a lot of people were running at them, obviously saying people needed help," Stangl said.

Northwest Hospital spokeswoman Karen Peck said her facility is one of at least six Seattle hospitals receiving patients. Three of the 15 patients being treated at Northwest were from one family, she said - men ages 60, 20 and 24.

Harborview Medical Center planned to treat up to 10 of the most seriously injured, spokeswoman Susan Gregg said.

The bridge was expected to be closed for hours as authorities investigated and cleared the wreckage, Stangl said.

The Aurora Avenue bridge carries Washington Highway 99, one of Seattle's two primary north-south arterials, over Lake Union. It has three lanes in each direction and no barrier separating the north and southbound lanes.

While there was no immediate word about the cause of the crash, some noted the safety of the amphibious boats has been questioned before. Lawyer Steve Bulzomi represented a motorcyclist who was run over and dragged by a Ride the Ducks boat that came up behind him at a stoplight in downtown Seattle in 2011.

"These are military craft that were never designed to navigate narrow city streets," Bulzomi said Thursday. "This is a business model that requires the driver to be a driver, tour guide and entertainer at the same time. It asks too much of the driver."

In 2010, a barge plowed into an amphibious vessel packed with tourists that had stalled in the Delaware River in Philadelphia.

The crash sent all 37 people on the duck boat into the river, but 16-year-old Dora Schwendtner and 20-year-old Szabolcs Prem never resurfaced. The Hungarians were visiting the United States through a church exchange program; their families filed wrongful-death lawsuits.

A tug operator, Matt Devlin, eventually pleaded guilty to involuntary manslaughter in the crash. Devlin acknowledged the accident was caused largely by his continuous use of a cellphone and laptop computer while he was steering the barge.

In July, the family of a woman struck and killed by an amphibious tourist boat in Philadelphia filed a wrongful-death lawsuit.

Attorneys for Elizabeth Karnicki's family allege the May 8 accident, which occurred during rush hour, was due in part to "huge blind spots" on the Ride The Ducks vehicle.

A tour bus rolled off a bridge and into a creek in Ketchikan, Alaska on Wednesday. 

Officials confirmed the crash caused no injuries as luckily no passengers were on board and the driver was unharmed.

The tour bus rolled backwards over a guardrail, off a bridge, and into a creek after the bus lost power.

The vehicle is owned by Alaska Coach Tours.

After the accident, a barrier was placed along the road across where the guardrail, and the road was temporarily closed to through traffic.

Officials say the bus was driving up Venetia Avenue toward Cape Fox Lodge when it lost power.

The bus then rolled backwards down Venetia and crossed over Park Avenue, over the sidewalk, and crashed through the railing along the road above the river.

The bus is still located in the creek, and workers are working on removing the bus.

The US Coast Guard Marine Safety Detachment was informed about the bus in the creek following the incident.

Ketchikan Creek, the scene of the incident, is an anadromous fish stream. The world-famous Alaskan salmon are currently heading upstream to spawn new generations of fish.

Pollution like oil and gas in these streams can damage fragile ecosystems.

US Coast Guard officials said that crews did not spot any evidence of oil or gas discharge and that the Marine Safety Detachment will continue to monitor the area.

Mechanical failure in 'Ride the Ducks' crash caused by maintenance, oversight issues

The National Transportation Safety Board said the cause behind a crash involving an amphibious Duck vehicle and a tour bus that killed five people was a mechanical failure that stemmed from maintenance-related problems and a lack of oversight years before the crash.

In NTSB’s assessment announced Tuesday, they reiterated that the driver was not at fault, but a mechanical failure.

According the board, the mechanical failure was related to a vehicle part called the Duck 6 axle housing design.

The company over Ride The Ducks Seattle, Ride The Ducks International (RTDI), was not registered with the Highway Traffic Safety Administration as a manufacturer. Therefore, the administration could not address defects.

According to the NTSB, RTDI was aware of defects on some of its vehicles and attempted to correct it with modifications to the vehicles, but the modifications were poorly executed.

In its summary, the NTSB said  Ride the Ducks of Seattle lacked protocols to ensure maintenance was completed properly and that RTDI should be registered as manufacturer so it can address defects through a federal recall program.

Safety issues included in the board’s report were:

Failure by unregistered vehicle manufacturer (RTDI)  to properly fix a safety defect under the recall process. Lack of adequate oversight of APV maintenance and failure to conduct safety repairs as recommended

Last month, the investigative review board released still frames showing the moments just before the September 2015 crash.

The pictures showed the Duck vehicle veering across the double yellow lines and then tearing into the tour bus that was taking 50 North Seattle College students on a tour of the city.

The amphibious Duck vehicle tore a 19-foot gash into the bus. Five international students were killed and dozens of others were hurt.

The NTSB had been investigating since the crash occurred and released transcripts from its work.

The driver of the Duck, Eric Bishop, described losing control , feeling the steering wheel lock up, and then veering sideways.

He and the driver of the tour bus both were cleared after investigators looked at drug tests and cellphone records.

Earlier this year, Utilities and Transportation Commission approved a $380,000 settlement against Ride the Ducks for 159 acute or critical motor safety violations. Other violations were related to record-keeping.

Since the Ducks have returned to the roads, they no longer travel over the Aurora Bridge and have a second crew member to give the tour so that the driver will not have to take their attention off the road.

"Since the NTSB began its investigation, we have cooperated with and assisted in every way possible. The organization has done a remarkable job in parsing mountains of information and developing its final report.
"While the investigation by the NTSB is comprehensive as to the facts of the accident, we believe it is important to point out underlying issues that are critical to a complete understanding of the nature of the accident.
"For example, the report clearly concludes that Ride the Ducks International (RTDI) was a vehicle manufacturer, subject to very specific rules and regulations, including dramatically heightened responsibilities for warning operators of safety issues. Had the company done its duty as prescribed by the regulations, we believe this tragic accident would have never occurred.
"I think it is also noteworthy that the NTSB called into question RDTI design and manufacturing. The board called the design 'inherently flawed' and the welds it made prior to delivering the vehicle to Ride the Ducks Seattle 'poorly executed.'
"In fact, Ride the Ducks Seattle has said that the service bulletins issued by RTDI were often problematic, citing issues that had previously corrected, or offering changes that had not been designed or reviewed by engineers or the appropriate experts.
"Ride the Ducks Seattle has voluntarily taken many of the steps the NTSB discussed today, and the company welcomes working to refine and implement the additional safety enhancements discussed in the hearing.
"Ride the Ducks Seattle has gone above and beyond what the NTSB or Washington State's UTC has asked of it in working to become the safest commercial fleet in operation. Ride the Ducks Seattle has received the UTC's highest rating, and is setting the standard for the industry."

Records: Speed likely factor in fatal Arizona tour bus crash

A tour bus on its way to the Grand Canyon was speeding when it flipped on its side, killing one passenger while injuring dozens, according to an investigative report that stops short of drawing conclusions about the cause of the crash.

Four dozen people from across the country were on the bus operated by Las Vegas-based Comedy On Deck Tours when it veered into a dirt embankment, over ruts and rocks, and hit Joshua trees on Jan. 22. At one point, it rode the face of a small hill and was airborne before coming back on to the road and flipping on its side, according to records.

The passengers suffered injuries ranging from abrasions and ankle sprains to fractures, blunt force trauma and broken ribs. Shelley Ann Voges from Booneville, Indiana, was partially ejected and died. Her husband and son, who recently had moved west, also were on the trip.

The bus was heading to Grand Canyon West, about 2.5 hours from Las Vegas and outside the boundaries of the national park. The tourist destination is on the Hualapai reservation and is best known for the Skywalk, a glass bridge that juts out 70 feet (21 meters) from the canyon walls and gives visitors a view of the Colorado River 4,000 feet (1,219 meters) below.

The Mohave County Sheriff’s Office released the crash report, body camera footage, photographs and 911 call recordings to The Associated Press in response to a public records request. The office didn’t respond to additional questions about the report Wednesday.

Passengers told authorities that it appeared the driver possibly had fallen asleep and was driving too fast. The records also call into question whether the automatic engine brake was engaged as the bus traveled on a curve and slightly downhill.

Mohave County sheriff’s Sgt. Dave Coffin noted speed limits of 45 mph posted along the road. The driver, Gary Griep, told authorities he was going no more than 40 mph, but the records say the GPS on the bus put the speed at 62 mph.

Griep said the engine brake, which helps slow and control the bus, was engaged, but two other bus drivers who were on tours in the area that day and stopped at the crash site said that didn’t appear to be true.

The crash report also said there were no visible marks on the tires or in the embankment to indicate heavy braking or steering for at least 700 feet (213 meters). Only when the bus hit a raised embankment did it veer from its path, the report said.

The investigation has not been turned over to prosecutors, said Cara Engstom of the Mohave County Attorney’s Office. The sheriff’s office is awaiting the results of a toxicology test on Griep and an autopsy report on Voges.

Passengers who had to crawl out of a rooftop hatch, the back door or large windows said they thought Griep might have fallen asleep. Griep told authorities he got at least eight hours of sleep the night before, doesn’t drink alcohol and wasn’t under the influence of any drugs. He said he sometimes coughed so hard his head hung low but he always kept his eyes on the road.

He attributed the crash to gusty winds that the National Weather Service said were 13 to 19 mph at the time.

Griep picked up passengers at hotels along the Las Vegas Strip that morning, took them to breakfast and the Hoover Dam and then headed toward the Grand Canyon Skywalk. As he came around a corner, he said the wind pushed the rear of the bus off the shoulder and into the embankment.

“Once it was in, I was fighting to get it back out, but once I did, it rolled over,” he said.

He told authorities he knew the road well and had made the trip hundreds of times. In his 20 years of driving buses, he said he never had another accident. He hung up abruptly Wednesday when reached by the AP.

Voges husband, Hubert, told authorities that passengers were tossed around the bus in the moments before it flipped, struggling to hold on, and screamed and cried. He said the trip was on the “bucket list” for his family, and he and Shelley Voges planned to return home the following day. He declined Wednesday to say more by phone.

Friends have said Shelley Voges, 53, was a sweet and compassionate woman.

Sheriff’s officials noted that all of the seat belts were in the locked position and didn’t appear to be worn. The driver was wearing a seat belt.

Two lawsuits have been filed against Comedy On Deck Tours over the fatal crash.

Four surviving passengers allege negligence on the part of the tour bus operator and Griep. The company has denied the allegations, according to court records, and didn’t return a call Wednesday from the AP.

Justin Zarcone, a Las Vegas attorney who represents the tour bus company in one of the lawsuits, declined comment Wednesday.

Shuttle bus plows into group near Honolulu pier leaving 1 dead and nearly a dozen injured

HONOLULU ( KNHL /Gray News) - One person is dead, and 11 others were injured Friday after a shuttle bus reportedly lost control near a Honolulu cruise ship terminal.

Authorities said a bus driver dropping off passengers near the terminal appeared to hit the gas instead of the brake and ended up pinning several people against concrete barriers.

Honolulu first responders said a 68-year-old woman was taken to the hospital in critical condition, but she later died.

Four people in their 50s and 60s were seriously injured with a man in his 70s showing stable vital signs was also taken to the hospital.

Six other adults ranging in age from their 40s to 70s suffered minor injuries and declined to be taken to the hospital.

First responders said they were called to Pier 2 at the ship terminal at about 10:20 a.m.

Honolulu police said the collision happened in a private parking lot.

According to police, the 57-year-old driver was dropping off the passengers when he was told that the vehicle was moving forward. He jumped into the driver’s seat to stop it but pressed the gas pedal instead of the brake.

Witnesses said the chaotic scene unfolded in seconds.

HONOLULU -- One person died and multiple people were injured when a shuttle bus collided with pedestrians and concrete barriers at a Honolulu cruise ship terminal, authorities said Friday.

The crash occurred when the shuttle bus driver mistook the gas pedal for the brake, Honolulu police said in statement.

The driver had dropped off customers at Pier 2 when bystanders noticed the bus was moving forward, police said. The driver jumped in the driver's seat and attempted to stop the vehicle when he stepped on the gas, police said.

One pedestrian, a 68-year-old woman, died. Paramedics took four others in their 50s and 60s to the hospital in serious condition. They also took a man in his 70s to the hospital in stable condition.

Paramedics evaluated and bandaged six others who declined transportation to the hospital, said Honolulu Emergency Medical Services spokesperson Shayne Enright.

Police said speed does not appear to be a factor in the crash. It's unknown if drugs or alcohol were involved, police said.
