Depending on the reason you chose, you may be asked to provide some / the following information:

  • Dependent visas   You will be asked the surnames and given names of the primary applicant. 
  • Application Receipt/Petition Number  Used when the trip purpose is for CW/E2C, H, K, L, O, P, R, T, or U visas. 
  • Principal Applicant’s Company Name  Only used if the applicant is not the principal applicant, and the trip purpose is Treaty Trader/Investor (E1, E2).
  • Date of Arrival in U.S.   In Format: DD-MM-YYYY
  • Arrival Flight (if known)  
  • Arrival City 
  • Date of Departure from U.S.   In Format: DD-MM-YYYY
  • Departure Flight (if known)  
  • Departure City  
  • Provide the locations you plan to visit in the U.S.  
  • Address Where You Will Stay in the U.S.:   Write the address where you will stay in the United States. If you are staying with your relative, such as a son, daughter, son-in-law, or daughter-in-law, write their address. It can also be a hotel where you might be staying. You don’t have to necessarily stay at that address for your entire trip to the U.S. “Any hotel” is not a proper answer to this question. If you are a student (F1 visa), this response might include your expected dormitory address, or if this is not known, your International Advisor/University campus address. If you are employed in the U.S. (H1B or L1) and maintain an apartment in the U.S., please provide that address and telephone number.  This should be the address where you can actually be contacted.  If you are crossing the border for shopping, enter the store’s or hotel’s street address, city, and state that you normally visit.

How to fill out the Cuba D’Viajeros Travel Form

Getting ready to travel to Cuba? Make sure you follow our steps to complete the D’Viajeros travel form before you travel. The form must be completed and shown at the airport before and after travelling to the island and requires passengers to input data such as their passport information, travel plans, and customs declarations.

Filling D’Viajeros form is mandatory for each passenger, and although it can be a bit confusing, we’re hoping our easy guide will help you fill it in without issues!

Step 1: Go to this site https://dviajeros.mitrans.gob.cu/inicio

You can only fill in this form 48hrs prior to departure so don’t even try beforehand!

The first screen you’ll see looks like this:

Step 2: Select your language

If you don’t speak Spanish then click Top Right Hand ‘Lenguaje’ tab for the dropdown language menu and choose Ingles/ English which will instantly translate the page for you!

Step 3: Click on ‘Form Request’ to start the process

A form will appear on the page.

Step 4: Fill in your personal data

Please note mandatory fields in RED. When you’ve done this, click the button that says ‘NEXT’.

Step 5: Migratory information

Fill in ‘Flight information’ and ‘Arrival Airport’. If you’re arriving in Havana, it’s Aeropuerto Internacional Jose Marti (HAV)

Step 6: Sanitary Information

Fill in information ‘Sanitary Information’ including ‘Vaccination information’. If you haven’t been vaccinated you should have a negative PCR test (although currently not mandatory for entry to Cuba. Please refer to UK GOV’s Cuba entry requirements here )

Step 7: Select your vaccination type from the drop-down menu

Make it easy for yourself and just select La Habana 🙂 When it comes to Municipalities , choose Habana Vieja or Plaza de la Revolucion for accommodation in the Vedado area.

  • If you’re staying in a Casa Particular choose House or room for rent and add the name and address we have given you for your first night’s stay.
  • If you’re staying in a Hotel choose Hotel , then select a hotel from the drop-down menu

Step 10: Customs Information

There is no requirement to declare what you are bringing unless you are bringing more than $5,000 in cash or miscellaneous non-personal items worth more than $1,000.

Please make sure you tick the boxes asking whether you have any unaccompanied baggage AND if you have anything to declare to Customs.

Nothing confusing here….

Make sure you view PDF and print it or save it.

With other QR codes, you get sent a copy via email, but NOT so with Cuba! This is why it’s critical that you’ve saved a copy. I print a version AND put a copy in my phone wallet just in case….

Create a Certificate of Rent Paid (CRP)

If you own or manage a rental property and rent living space to someone, you must provide a CRP to each renter if either of these apply:

  • Property tax was payable in 2023 on the property.
  • The property is tax-exempt, but you made payments in lieu of property taxes.

Renters will need the CRP to apply for the Renter’s Property Tax Refund. You must give each renter a CRP by January 31, 2024.

Subscribe to Landlords CRP email updates . 

All residential property owners and managers can now use e-Services, our online filing and payment system, to create Minnesota Certificates of Rent Paid (CRPs).

Landlords are not required to use e-Services to create 2023 CRPs. However, landlords will be required to use e-Services to create 2024 CRPs.

To help determine which account type to use, visit e-Services Account Types for CRPs . 

Individuals

To sign up, go to the e-Services login page and select Create a username below the username fields.

To create a username, individuals are required to provide:

  • Your Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN)
  • Refund or Amount Due from your Minnesota Individual Income Tax return or a Property Tax Refund return filed within the last 3 years

All business users have access to the CRP system in e-Services. If you currently create CRPs using e-Services, you will be able to access and import your data in the same way as previous years. This update includes a new step-by-step process which will be available to all users.

You can create CRPs for one or more properties by importing your data from a file in e-Services. Importing your CRP data from a spreadsheet allows you to create CRPs for multiple addresses using one file. To learn more about formatting your file to import, visit Certificates of Rent Paid File Formats .

If you choose to manually enter your data, each street address must be created as a separate submission. You can enter multiple units as part of the same submission if they share the same address.

Landlords are not required to use e-Services to create 2023 CRPs. For 2023 CRPs can use a certified software product. See Certificate of Rent Paid Software .

Landlords will be required to use e-Services to create 2024 CRPs. You will still be able to export your data from a software product and import it into e-Services, but you will not be able to create a CRP in another way. 

Rent in a Motel or Hotel

Individuals who stay in a motel or hotel can qualify to receive a CRP if it is their permanent residence and if the stay is 30 consecutive days or more. Line 1 of the CRP must be for rent only, not for any services. Often, motels and hotels are unaware of the requirement to fill out the CRP for the renter. The renter may contact the department to request a Rent Paid Affidavit (RPA) to take the place of the CRP if the hotel or motel refuses to complete a CRP.

Campgrounds, Travel Trailers, and Campers

Individuals who reside in campgrounds, travel trailers, and campers do not receive a CRP, even if they rented there for extended periods. This is true even if they do not have a permanent residence elsewhere. Sales tax is paid on such units, not property tax. Also, landlords do not issue CRPs for land rented for a trailer (unless it is considered a permanent situation involving a mobile home park).

In November 2024, you will receive a Notice of Proposed Property Tax for Taxes Payable in 2025 from your county treasurer.

By state law-as an owner of rental housing-you are required to:

  • mail or hand deliver a copy of the notice to your tenants; or
  • post a copy of your notice in a conspicuous place on the premises occupied by your tenants.

The purpose of providing copies or posting the notice is to notify tenants of proposed property tax increases or decreases on the building, which may affect their rent payments.

You must post, mail or hand deliver a copy of your Notice of Proposed Property Tax to your tenants by: 

  • November 27, 2024; or
  • no later than three days after you receive the notice, whichever is later.

If You Want Your Notice to be Mailed to a Different Address

While the Notice of Proposed Property Tax is normally mailed to the same address as the property tax bill, owners of rental housing may call or write their county treasurer to request the notice be mailed to a different address.

Calculations for Health Care Facilities

Health care facilities have special requirements for completing CRPs for their residents. 

Do not provide a CRP if a resident’s entire care is paid under any of these assistance programs:

  • Supplemental Security Income (SSI)
  • Minnesota Supplemental Aid (MSA)
  • Medical Assistance (MA or Medicaid)
  • General Assistance Medical Care (GAMC)
  • Housing Support (formerly Group Residential Housing or GRH)

Note: If assistance programs pay only part (or none) of a resident’s care, you must issue a CRP.

For each qualifying resident:

  • On line B, enter the amount of Housing Support (formerly GRH) payments you received for rent on the resident's behalf. This amount should not be included on Line 1.
  • If the resident lived in the residence for part of the month, prorate the rent ($930 or less) for that month.
  • Enter the result on line 1.
Example: A renter lived in adult foster care from January 1 through June 10. The renter had five full months and one partial month in the unit. Five Full Months $930 x 5 months = $4,650 One Partial Month $930 x (10 days / 30 days) = $310 Total Rent $4,650 + $310 = $4,960 (round to nearest dollar)

You must include all payments made under a rental agreement, in exchange for the right to stay at the assisted living facility. Do not include medical services in total rent paid on the CRP.

We will accept any reasonable method for determining what portion of rent consists of medical services. If there are specific costs for medical services included in the lease agreement, exclude those amounts from line 1 of the CRP. Otherwise, include the full amount of rent paid on line 1.

  • Multiply the daily (per diem) rate by the number of days billed during the year.
  • Subtract the annual Medical Assistance spend-down amount.
  • On line B, enter the amount of Housing Support (formerly GRH) payments you received for rent on the resident’s behalf. This amount should not be included on Line 1.
  • If the resident paid less than $600 a month, use the lesser amount.
  • If the resident lived in the facility for part of the month, prorate the rent ($600 or less) for that month.
Example: A renter lived in the intermediate care facility from January 1 through June 10. The renter had five full months and one partial month in the unit. Five Full Months $600 x 5 months = $3,000 One Partial Month $600 x (10 days / 30 days) = $200 Total Rent $3,000 + $200 = $3,200
