Non-gonococcal urethritis (NGU), inflammation of the urethra, is the most common sexually transmitted illness in men. Urethritis can be infectious or non-infectious. Infectious causes almost always sexually transmitted. Sexually transmitted urethritis is traditionally divided into 2 categories: gonococcal urethritis and non-gonococcal urethritis (NGU). This activity describes the etiology and pathophysiology of non-gonococcal urethritis and highlights the role of the interprofessional team in its management.

  • Introduction

Non-gonococcal urethritis (NGU), inflammation of the urethra, is the most common sexually transmitted illness in men. Urethritis can be infectious or non-infectious. Infectious causes almost always sexually transmitted. Sexually transmitted urethritis is traditionally divided into 2 categories: gonococcal urethritis and non-gonococcal urethritis (NGU). [1]  Since almost all urethritis patients have penile discharge, gonococcal urethritis can easily be distinguished from NGU by looking for gram-negative diplococci on a gram stain. If diplococci are identified, the urethritis is gonococcal; if no diplococci are identified, the diagnosis is classified as NGU. [2]

The most common cause of NGU is Chlamydia trachomatis , which accounts for 15% to 40% of NGU cases. [2]  The second most common cause is Mycoplasma genitalium , which accounts for 15% to 20% of NGU cases; although widespread testing for mycoplasma is not available. [3]  Less common causes of NGU include infection with Trichomonas vaginalis , herpes simplex virus, Epstein Barr virus, and Adenovirus . Enteric bacteria is an uncommon cause of NGU that is typically seen in males who practice insertive anal intercourse. In almost half of all cases of NGU, an etiology is not identified. [2]

  • Epidemiology

Urethritis is the most common sexually transmitted infection in men with gonorrhea and chlamydia representing the primary pathogens. The Center for Disease Control and Prevention (CDC) does not publish rates of urethritis as a syndrome. The CDC instead breaks down sexual infection rates down by etiology. Chlamydia is the most commonly reported sexually transmitted infection in the United States with over 1,500,000 new cases in 2016. Rates of new diagnoses continue to rise annually with an estimated increase of 4.7% from 2015 to 2016. Trends show that rates of chlamydia in females are 2 times higher than that of males. Rates are highest among adolescents and young adults. Racial disparities exist, and blacks are 5.6 times more commonly affected with chlamydia when compared to whites. Gonorrhea cases are more commonly seen in males, and there were an estimated 468,000 new cases of gonorrhea in 2016. Like with chlamydia, infections rates are higher in adolescents and young adults; the African American race is also disproportionately affected.

  • Pathophysiology

The most common cause of NGU is chlamydia. C .  trachomatis typically infects columnar epithelial cells at mucosal sites. It replicates in host cells eventually causing cell death. [4]  Other causes of NGU are less well described.

  • Histopathology

Microscopic evaluation of NGU reveals polymorphonuclear leukocytes without the presence of intracellular gram-negative diplococci on gram stain.

The most common cause of NGU, C. trachomatis, is an intercellular bacterium. The cell wall in C. trachomatis lacks peptidoglycan. The absence of peptidoglycan makes C. trachomatis not visible on Gram stain and makes beta-lactam antimicrobials ineffective. [5]

  • History and Physical

Patients present with complaints of dysuria, pruritus, penile burning, and urethral discharge. Urethral discharge can be purulent, mucoid or watery. Discharge in NGU is sometimes scant and only noted as crusting at the meatus or staining of the underwear.

Not all men with urethritis have symptoms, and over 40% of cases of all cases of NGU are asymptomatic. [6]

Patients who present with regional lymphadenopathy, constitutional symptoms, or vesicles may have urethritis from HSV NGU.

Physical examination may reveal penile discharge. Differentiating between gonococcal and non-gonococcal urethritis is difficult based on clinical examination of discharge. NGU tends to be mucoid or clear, compared to gonococcal urethritis which is more commonly purulent; however, this generalization is not reliable.

Men with urethritis can also present with fever, testicular pain, testicular swelling, sore throat, rectal pain, or rectal discharge. Differential diagnoses of these symptoms would include other infectious processes such as epididymitis, pharyngitis, or prostatitis, and these should be considered accordingly.

Conjunctivitis can accompany urethritis caused by gonorrhea, chlamydia, or Adenovirus .

Some patients with NGU can also present with reactive arthritis, which is characterized by urethritis, uveitis, and arthritis. This constellation of symptoms is typically associated with the HLA-B27 gene. [1]

Urethritis can be diagnosed with any of the following:

  • Mucoid, mucopurulent, or purulent discharge on examination
  • Gram stain of urethral secretions demonstrating equal to 2 white blood cells (WBC) per oil immersion field
  • Positive leukocyte esterase test on urinalysis or microscopic examination of sediment from a spun first-void urine demonstrating equal to 10 WBC per high-power field

The diagnosis of NGU requires the exclusion of gonorrheal infection. This can be achieved with a gram stain that lacks gram-negative diplococci or by any negative gonorrhea test (culture, nucleic acid testing, among others). [7]

In men at high risk for infection, if symptoms are present but none of these criteria are met, a presumptive diagnosis can be made, and empiric treatment started. These patients should be treated with a drug regimen for both chlamydia and gonorrhea. [8]

Patients should also have a nucleic acid amplification test (NAAT) performed for chlamydia, gonorrhea, and trichomoniasis. In urethritis, NAAT testing is recommended over culture because of its high sensitivity and specificity. NAAT testing for M. genitalium exists but is not widely available. [1]

  • Treatment / Management

Empiric treatment for urethritis should be started at the time of diagnosis, before results of testing for a specific etiology. Treatment should be based on whether the gonococcal infection has been evaluated by microscope:

Presumptive Treatment of NGU with Microscope

Recommended Regimens

  • Azithromycin 1 g orally in a single dose, or
  • Doxycycline 100 mg orally twice a day for 7 days

Azithromycin and doxycycline are both highly effective for the treatment for chlamydial urethritis. Urethritis associated with M. genitalium responds better to azithromycin, although azithromycin resistance is increasing.

Presumptive Treatment of NGU without Microscope

When Gram stain is not available gonorrheal infection cannot be ruled out. These patients should be treated with a drug regimen effective against both chlamydia and gonorrhea.

  • Ceftriaxone 250 mg IM

Recurrent or Persistent NGU

Recurrent or persistent symptoms can be common following treatment for NGU with the recommended regimen. Patients should first be evaluated for adherence to prior treatment and re-exposure to disease. If they continue to have evidence of urethritis (discharge on exam, elevated WBC on a microscope, or leukocyte esterase on urinalysis) therapy should be directed towards M. genitalium. M. genitalium is the most common cause of persistent or recurrent NGU.

  • If azithromycin was not used for initial exposure:
  • Azithromycin 1 g orally once  
  • If azithromycin was used for initial exposure
  • Moxifloxacin 400 mg orally daily for 7 days
  • In either instance, metronidazole 2 g orally once should be given in areas where T. vaginalis is prevalent. [1]
  • Differential Diagnosis

The differential diagnosis of patients presenting with dysuria includes:

  • Prostatitis
  • Epididymitis

Typically, patients with another diagnosis present with other clinical findings; however, any of these conditions may also co-exist with urethritis. [9]

  • Treatment Planning

Partner management is recommended for patients with urethritis regardless of whether specific etiology is identified. Sexual partners within 60 days should be referred for complete STI evaluation and treatment with the same treatment regimen as the original patient. [1]

NGU is generally a self-limited disease and resolves, even without therapy, with no complications in most cases.

  • Complications

Complications for NGU in men are rare but include epididymitis, prostatitis, abscess formation, and reactive arthritis. One percent to 2% of males with NGU develop epididymitis. One percent to 2% develop conjunctivitis. Urethral stricture or stenosis is possible after NGU due to post inflammatory scar formation but is rare.

Complications in women are more common than in men (10% to 40%). Women with urethritis can potentially develop pelvic inflammatory disease, infertility, and ectopic pregnancy. [10]

Children born to mothers with chlamydia urethritis can potentially develop complications including conjunctivitis, iritis, and pneumonia. Routine treatment of all newborns with antibiotic eye ointment has significantly decreased the incidence of these. [11]

Chlamydia urethritis can also cause lymphogranuloma venereum, a genital ulcer disease which presents as lymphadenitis or lymphangitis and can lead to lymphatic obstruction, strictures or fistulas. [1]

  • Deterrence and Patient Education

Patient education should include the following:

  • Patients should abstain from sexual activity for 7 days, starting after the completion of therapy for the patient and all partners. [1]
  • Patients taking doxycycline should be advised to reduce sun exposure, as doxycycline can cause photosensitivity. [12]
  • Patients taking metronidazole should be advised to avoid alcoholic beverages.
  • Pearls and Other Issues

Urethritis is typically a sexually transmitted infection with most cases caused by gonorrhea, chlamydia, and mycoplasma.

Typical presentation includes dysuria and penile discharge.

Diagnosis is based on positive discharge on an exam or more than 2 WBC seen on microscopy or presence of leukocytes on urinalysis  (or 10 WBC on first-void urine catch).

Initial empiric treatment should be started at the time of diagnosis. Treatment for NGU when gonococcal infection can be rule out based on microscope examination includes azithromycin 1 g orally once or doxycycline 100 mg twice per day for 7 days. If a gonococcal infection cannot be ruled out treatment should add ceftriaxone 250 mg intramuscularly once to cover for gonococcal infection.

Patients should be instructed to avoid sex for 1 week and until partners are treated. Partners should be tested and treated for urethritis.

  • Enhancing Healthcare Team Outcomes

Patients with urethritis may present to the emergency department, urgent care clinic, or primary care provider. It is important for these healthcare professionals to be aware of the cause of urethritis and its treatment. In many cases, the treatment is empirical until the laboratory has confirmed the diagnosis.

The Marble Mountains are five mountains made of limestone and marble, about 9 kilometers (5 miles) south of Da Nang city proper. The cluster of hills is a well-known pilgrimage site with beautiful peaks, vast networks of tunnels, gorgeous caves, and impressive temples making for a wonderful place to explore.

History of the Marble Mountains

There is a beautiful myth surrounding the Marble Mountains: according to this myth, when a dragon came out of the ocean at Non Nuoc beach it laid an egg. This egg hatched after a thousand days and nights, and a girl came out of it. The fragments of the shell were left behind, and these were what became the hills of the five mountains.

The five mountains have been named after the five elements: Kim (metal), Thuy (water), Moc (wood), Hoa (fire) and Tho (earth). Núi Cẩm Thạch, the Vietnamese for the Marble Mountains, is literally 'Five Elements Hill'. The one peak that is open for climbing to the top of is Thuy.

Tam Thai Pagoda, at the top of Thuy, is nominally 400 years old: it has been rebuilt and repaired many times.

Xa Loi Pagoda (built 1956), at the top of the elevator, was South Vietnam's Buddhist headquarters. It has Buddhist statues, a ceremonial hall, and a bell tower. The tower is 28 meters (92 feet) high, with six sides and seven stories representing seven emotions, which are to be overcome like climbing the floors of the pagoda in Buddhism.

The incongruous concrete and glass elevator, looking like a pale skyscraper attached to the hill at the top, was built in 2011.

Today, they are a popular tourist destination because the views are so amazing from the top and there's lots to see along the way.

From caves with ancient Buddha statues inside and beautiful Hindu and Buddhist art, to beautiful ponds with greenery, and pagodas and temples along the way, there is just so much to see for the entrance ticket of just 40,000 Vietnamese dong.

The grottoes' carvings show depictions of religious scenes carved out of marble, and impressive statues.

It is best to visit Da Nang and the Marble Mountains between January and June, when it is less rainy. This is because Da Nang has a tropical monsoon climate and it rains a lot between September and November.

April and May are probably the best months to visit, as January and February can be a little chilly still, especially in the mornings or evenings.

As you're likely to want to combine the Marble Mountains with a visit to one of Da Nang's many amazing beaches, April and May is also when the temperature is best for this (through to August, although closer to the end of summer big rainstorms become more likely). Is it Hard to Climb up the Marble Mountains?

The climb up is quite steep, so it's a tough walk to get up, hence we suggest you only attempt this if you are fit.

There are a total of 156 steps (about the equivalent of a 12 story building) leading to the top of Thuy (the peak of the mountain cluster named after the water element). At a leisurely (careful) pace, recommended due to unevenness and slipperiness after rain, 20 minutes is sufficient for the fit.

A map costing 15,000 dong is useful when walking up to find the sights and as a souvenir.

It is approximately a 3-or-4-hour trip in total, there and back from Da Nang, and not recommended to do with very young children, because it gets steeper the closer you get to the top.

There is a lift near the entrance that you can take up, although it doesn't go all the way to the very top, costing an additional 20,000 Vietnamese dong each way, if you decide to take it.

Clothing: Make sure you are wearing comfortable shoes, and light, breathable clothing if you are visiting in the summer (April through to the end of August). While we recommend starting your visit early in the day when it is not yet extremely hot, it will get warmer throughout the day.

If you are visiting on a rainy day during the rainy season (between September and December), make sure you're also wearing shoes with good grip, as some of the steps can get a little slippery. Bringing along an extra layer is also a good idea during this time when temperatures are lower, especially at the top of the peak. If you start off early it will also be a little cold when you start your hike.

Bring a torch if you have one to explore the inside of the caves, and plenty of water for your journey, as well as a snack.

You'll find children along the way selling souvenirs and offering tours. They can be quite persistent, so make sure you've packed some small change if you're looking to get a souvenir or would like to be shown around by them.

It can also be helpful to be wary of the local monkeys that roam the mountains. They can be a little aggressive as they sometimes do get food from the visitors, so please do not feed them and don't get too close to them.

The Marble Mountains are only about 20 minutes by road outside Da Nang in Ngu Hanh Son District of Da Nang.

Bus 1 from Da Nang Bus Station and the Coco Tour Bus (pink) stop a few minutes walk from the Marble Mountains' entrance.

For the more adventurous, motorbikes can be rented in Da Nang.

Address for taxis/drivers: 2 Huyen Tran Cong Chua, Hoa Hai, Ngu Hanh Son, Da Nang, or just say Núi Cẩm Thạch if your Vietnamese is passable. Ask the taxi driver to use the meter or use a ride booking app. Drivers wait at the entrance ready to take visitors back to the city.

Our guide lele is a wealth of information, Lele is very professional and very attentive to our needs. Lele is amazing. Lele got everything spot on. It probably helps that Guilin is a brilliant place to visit so Lele has great material to work with but that doesn't take anything away from how much Lele helped make it a great trip.

Our guide for Beijing was super knowledgeable and experienced and able to help us to achieve as much as we wanted within the time given. We had a fun time guided by him as he is also super humorous and you can see how he interacts with the vendors and people whom he comes by. Thank you for a very enjoyable time in Beijing and accommodating to all our needs!

Our China Highlight guide. Michael, was attentive, thoughtful and knowledgeable. He narrated many interesting historical events about Chengdu while touring around the city and having afternoon tea with us. He was thoughtful to provide us with snacks during long hikes at Leshan or walks around the city.

She was very flexible and added extra time when we needed it and we felt extremely well taken care of. She also chose the best restaurants for us,

Our tour guide Helen, was excellent, she was very kind, professional and passionate for her work and she also loves Pandas! She will take you to take the best panda photos and to know more about Chengdu city. Our tour was great, she took us to all our destinations always with the best spots: Temples, pagodas, famous streets, theaters, pandas...you name it! Everything was great.

He picked up our pre-booked boat/other excursions tickets so we were able to avoid all the long lines and chaos. He is knowledgeable of the places we visited, courteous, fun to travel with and well-versed in Chinese classics.

Tom is the guide that will take you to where no other guide will. We pushed for the experience and Tom and the team delivered more than what we could have ever asked for. His English speaking ability and his Chinese history knowledge is second to none.

Located on the route of Da Nang- Hoi An, Marble Mountains Vietnam, or Ngu Hanh Son is a complex of 5 limestone mountains rising on the coastal sand: Kim Son, Moc Son, Thuy Son, Hoa Son, and Tho Son. Ngu Hanh Son Mountain has majestic landscapes with ancient mossy rocks and many unique cultural and historical architectural works.

3. How to visit the Marble Mountains Danang?

The distance from the center of Da Nang city to Ngu Hanh Son is only about 7km so you can choose to travel by motorbike, bus, or taxi.

Motorbike: You can follow 2 roads to Ngu Hanh Son: Vo Nguyen Giap – Truong Sa – Non-Nuoc. Or Le Van Hien – Non-Nuoc. Travel time by motorbike is about 15 minutes.

Bus: You can choose the number 1 route from Da Nang to Hoi An, depart from Da Nang City Central Bus Station and get off at Marble Mountains Da Nang . The time between trips is 20 minutes. Taxi: Some of the taxis in Da Nang are the Han River Taxi, Mai Linh Taxi, and Huong Lua Taxi.

4. What to visit at the Mable Mountains Da Nang?

4.1. mable of water.

Thuy Son or Marble of Water is also called Tam Thai mountain because there are 3 peaks located on three floors like the three stars of Tam Thai. In  the 5 mountains, Thuy Son is the most beautiful and largest mountain that many tourists choose to visit and explore. To reach the top of the mountain, you can choose to climb over a hundred steps. Another way is to take an elevator with a transparent glass chamber with a two-way ticket of 40,000 VND.

Come here not only to admire the beautiful panoramic city of Da Nang from above but also you can visit many beautiful temples and caves.

4.2.  Linh Ung Pagoda

When coming to Marble Mountains Da Nang Vietnam , Linh Ung Pagoda cannot be overlooked on Thuy Son mountain. Placed in a solemn position in the middle of the temple is a 10m high Buddha statue with two sides worshiping Lady Buddha Bodhisattva and Bodhisattva Dia Tang. This place does not only attracts many tourists who love spiritual tourism. But also where you can immerse yourself in a peaceful place at Buddha’s door and away from the bustling city.

4.3. Tam Thai Pagoda

Tam Thai pagoda is also a large pagoda that many tourists choose to visit.  The pagoda has a 3 door gate and according to Buddhist culture, the main gate is only for the monks while the right gate is for women and the left is for men. Over time and in war, the pagoda has been renovated many times but still retains many ancient Buddhist architectural features.

4.4. Huyen Khong Cave

Huyen Khong Cave is the most beautiful scenery among the caves in Ngu Hanh Son. The cave is open-air and has an extremely unique structure with circular arches to the outside, so it is always filled with light. Not only come to visit, but visitors also come here often to immerse themselves in the sacred space to feel calm and relaxed.

4.5. Tang Chon Cave

Tang Chon Cave is one of the most beautiful Marble mountain caves that you must visit on Mable of Water. Inside the cave, there are a lot of smaller caves where you will admire the masterpieces of nature such as The big Buddha statue doing meditation; The standing Buddha statue teaching and blessing people. The Holy chess table and as well as the place where the Champa Kingdom worships the God Shiva in Hinduism.

4.5.  Hades Cave

Am Phu Cave or Hades Cave is one of the major caves in the Ngu Hanh Son cluster. This is a natural cave with mysterious dark features and many nooks deep into the ground that seems to be going to the gates of hell. Therefore, people here have long recreated many punishments under the underworld in caves with the purpose of reminding and waking up people to live better.

4.6.  Non-Nuoc Mable carving village

Non-Nuoc stone carving village is a destination many tourists visit when visiting Ngu Hanh Son. Coming here, you will admire the stone sculptures made by extremely talented hands of Non-Nuoc artisans. You will feel a bit of the love of the craftsmen here when you witness the process of “transformation” from lifeless stones, through each drill, meticulous strokes to become a finished product.

Come here, you can buy some pretty figurines as gifts for relatives and friends. Remember to bargain to buy it at a cheaper price!

4.7.  Non-Nuoc Beach

Located near the Marble Mountains , Non-Nuoc beach attracts visitors with its pristine beauty, tranquility, and freshness. 5. Marble Mountains entrance fee and opening hours

To visit the Marble Mountains of Vietnam , you have to buy entrance tickets. The Marble Mountain price costs 40,000 VND roughly 2 USD for a person. If you would like to take the elevator to go up, it costs 15,000 VND/per person. Children under 10 years old are free. The Marble Mountain opening hours are from 7 am to 6 pm every day of the year and even on New Year as well.
