Make the most of your doctor visit

A visit with your health care provider is a good time to share health concerns and ask questions. Preparing ahead for your appointment can help you get the most from your time together.

When you see your provider, be honest about your symptoms and lifestyle habits . Ask questions to make sure you understand. Taking an active role in your health can help you get the best care possible.

How to Prepare

Before your visit, jot down your questions and concerns. You may want to ask things like:

• Am I due for any screening tests?
• Should I keep taking this medicine?
• What could be causing my symptoms?
• Do I have other treatment options?
• Should I worry about my family medical history?

Also be sure to write down all medicines, vitamins, and supplements you take. Include over-the-counter medicines and herbal supplements as well. Bring this list with you to your appointment.

If you are having symptoms, write down details before the visit.

• Describe your symptoms
• Describe when and where they appear
• Explain how long you have had symptoms and if they have changed

Put the notes in your purse or wallet so that you do not forget to bring them. You can also put the notes in your phone or in an email to your provider. Writing things down makes it easier to remember details at the time of your visit.

If you need support, invite a friend or family member to come with you. They can help you understand and remember what you need to do.

Make sure to have your insurance card with you at the time of your visit. Tell the office if your insurance has changed.

What to Share and ask

What you do and how you feel can affect your health. Here are some things you want to share.

Life changes. These may include:

• Job changes
• Family changes, such as death, divorce, or adoption
• Threat or acts of violence
• Planned trips outside of the country (in case you need shots)
• New activities or sports

Medical history. Go over any past or current health conditions or surgeries. Tell your provider about any family history of disease.

Allergies. Tell your provider about any past or current allergies or any new allergy symptoms.

Medicines and supplements. Share your list at your appointment. Tell your provider if you are having any side effects from your medicines. Ask about special instructions for the medicines you take:

• Are there possible interactions or side effects?
• What is each medicine supposed to do?

Lifestyle habits. Be honest about your habits, your provider will not judge you. Alcohol and drugs can interfere with medicines or cause certain symptoms. Tobacco use puts you at risk for a number of health problems. Your provider needs to know about all of your habits in order to best treat you.

Symptoms. Share your notes about your symptoms. Ask your provider:

• Which tests might help to find the problem?
• What are the benefits and risks of tests and treatment options?
• When should you call your provider if your symptoms do not improve?

Prevention. Ask if there are screening tests or vaccines you should have. Are there any lifestyle changes you should make? What can you expect for results?

Follow-up. Ask your provider when you should schedule more appointments.

After Your Visit

Your provider may want you to:

• See a specialist
• Have a test
• Take a new medicine
• Schedule more visits

For the best results, follow your provider's instructions. Take medicines as directed, and go to any follow-up appointments.

Write down any new questions about your health, medicines, or treatment. Continue to keep a record of any symptoms and all of your medicines.

When to Call the Doctor

You should contact your provider when:

• You have side effects from medicines or treatments
• You have new, unexplained symptoms
• Your symptoms get worse
• You are given new prescriptions from another provider
• You want the results of a test
• You have questions or concerns

One of the scariest things as a foreign language speaker is having to go to the doctor when you are sick. If you’re not sure how to talk about common medical problems in English, I’m here to help. I’m not a doctor, but I am an English teacher. So let’s look at 15 common problems you might be facing and how to use them in a sentence. Whether it’s a scratchy sore throat or the nagging ache in your muscles, knowing how to comfortably use these words in English will help you the next time you have to go to the doctors.

Head and neck

• Part of Speech : Noun
• Meaning : Pain in your head or neck
• Example Sentence : “I can’t focus on my work because I have a terrible headache.”
• We can only ever use the word ‘headache’ as a noun. But, we can separate the two words and use ‘ache’ as a verb. For example: ‘My head is aching.’ Or: ‘My head aches.’

Sore Throat

• Meaning : A scratchy pain or discomfort in your throat, often combined with a cough
• Example Sentence : “Drinking hot tea with honey can help soothe a sore throat.”
• We can say that we have a sore throat or that we’re getting a sore throat. But, we can separate the words and change the order for example: ‘My throat is sore.’ Or: ‘My throat is feeling sore.’
• Part of Speech : Verb/Noun
• Meaning : To push air out of your lungs quickly and with force while making a noise
• Example Sentence : “I can’t stop coughing; I think I might be coming down with a cold.”
• In this example sentence cough is used as a verb. But we can also use it as a noun. We could say for example: ‘I have a cough.’ Or maybe: ‘I am coming down with a cough.’

Common Cold

• Meaning : A viral infection causing symptoms like sneezing, a runny nose, congestion, and a cough
• Example Sentence : “Rest and plenty of water can help you recover quickly from the common cold.”
• We don’t need to use the word ‘common’ when we talk about a cold. For example we could just say: ‘I stayed home from work yesterday because I had a cold’. It’s important to remember that the word cold can also mean the opposite of hot. We use that word with the verb to be or to feel. For example: ‘I am cold, I’m going to get a jacket.’ But, when we are talking about the common cold, or being sick, we use the verb to have or to get or even to come down with. For example: ‘I think I’m coming down with a cold, I can’t stop sneezing and sniffling.’

Muscles and joint s

Muscle aches and pains

• Meaning : Discomfort or soreness in your muscles
• Example Sentence : “After a strenuous workout, I always have muscle aches and pains.”
• We don’t often use the full phrase ‘muscle aches and pains’. Instead we might say for example: ‘I’m feeling a bit achey.’ Or: ‘My muscles are a bit sore today.’
• Meaning : Pain or discomfort in your back
• Example Sentence : “If you lift weights that are too heavy you can end up with back pain.”
• Again, back pain isn’t a super common phrase to use verbally. We might instead say: ‘My back has been hurting me a lot recently.’
• Meaning : Discomfort or soreness in your joints, such as the knees, elbows, or shoulders
• Example Sentence : “Changes in the weather sometimes cause my grandmother to experience joint pain.”
• Again, the more common, everyday way we talk about joint pain is: ‘My knee is feeling really achey.’ Or even simpler: ‘My knee hurts.’

Upset Stomach

• Meaning : Feeling a pain, uneasiness, or discomfort in your stomach, often with the urge to vomit or go to the toilet
• Example Sentence : “Eating too quickly can give you an upset stomach.”
• We can use this phrase with the verb to have. For example: ‘I have an upset stomach after eating all of that candy.’ But we can also switch the order order to say: ‘My stomach is a bit upset tonight.’ When we have an upset stomach we also can say we have a ‘stomachache’.
• P art of Speech : Noun
• Meaning : The feeling of wanting to vomit
• Example Sentence : “The strong smell of fish gave her a sense of nausea.”
• Nausea is a noun but we more commonly use the adjective form: nauseous. For example we’d say: ‘I feel really nauseous I think I’m going to lay down.’ If we’re speaking even more casually and less politely we might just say: ‘I feel like I’m going to throw up.’
• Meaning : A burning sensation in your chest, often caused by stomach acid
• Example Sentence : “Eating spicy food before bedtime can sometimes give you heartburn.”
• This phrase always has to stay together as a noun. For example it’s not possible in this context to say: ‘My heart burns.’

Constipation:

• Meaning : Difficulty pooping or infrequent bowel movements
• Example Sentence : “Constipation can cause a lot of problems and be very painful.”
• We would more often use the adjective version of this word. For example we might say: ‘I’ve been constipated for a few days and now my stomach is really hurting.’
• Meaning : Loose or very watery poops.
• Example Sentence : “Food poisoning can cause diarrhea.”
• We normally use the verb to have with diarrhea. For example someone might say: ‘I need a bathroom right now I feel like I’m going to have diarrhea.’

Mental Health

• Meaning : Difficulty falling or staying asleep
• Example Sentence : “Stress and anxiety can cause people to experience insomnia.”
• The word insomnia isn’t super common outside of a doctor’s office. Instead we might say: ‘I’ve been having trouble sleeping.’
• Meaning : Excessive worry, fear, or nervousness
• E xample Sentence : “Public speaking causes a lot of people to experience anxiety.”
• The adjective form of this word is also very commonly used. Someone might say: ‘I get really anxious when I have to take an exam in school.’
• Meaning : Persistent feelings of deep sadness and a loss of interest in almost anything
• Example Sentence : “Depression can be very debilitating, causing people to be unable to get out of bed.”
• This is another word that we often use in its adjective form. The word is ‘depressed’. For example people might say: ‘I’ve been really depressed the past week and I haven’t wanted to do anything much.’

Example dialogue

Doctor: Good morning! How can I help you today?

Patient: Hi, I’ve had a persistent cough, and I’ve also been feeling feverish for a few days.

Doctor: I see. And have you experienced any shortness of breath or chest pain along with the cough?

Patient: No chest pain, but yeah, I do feel a bit short of breath sometimes.

Doctor: Okay. Any recent travel or exposure to someone with similar symptoms?

Patient: No travel, but my coworker did have a bad cough last week.

Doctor: Got it. I’ll take a look at your throat and listen to your lungs. You might have a respiratory infection. Given the shortness of breath, I want to rule out any complications. I’ll prescribe an antibiotic to address any bacterial infection and recommend over-the-counter cough medicine to ease your symptoms. Make sure to rest, stay hydrated, and monitor your temperature. If your symptoms persist or worsen, please come back for a follow-up.

Title: from words to numbers: your large language model is secretly a capable regressor when given in-context examples.

Abstract: We analyze how well pre-trained large language models (e.g., Llama2, GPT-4, Claude 3, etc) can do linear and non-linear regression when given in-context examples, without any additional training or gradient updates. Our findings reveal that several large language models (e.g., GPT-4, Claude 3) are able to perform regression tasks with a performance rivaling (or even outperforming) that of traditional supervised methods such as Random Forest, Bagging, or Gradient Boosting. For example, on the challenging Friedman #2 regression dataset, Claude 3 outperforms many supervised methods such as AdaBoost, SVM, Random Forest, KNN, or Gradient Boosting. We then investigate how well the performance of large language models scales with the number of in-context exemplars. We borrow from the notion of regret from online learning and empirically show that LLMs are capable of obtaining a sub-linear regret.

Concerns about eye discomfort appear to rise after solar eclipse

Google searches about “hurt eyes” spiked Monday afternoon, just after many U.S. communities experienced the total solar eclipse. 

The searches suggest some people in the sun’s path were worried they’d glanced at it too long. 

It’s a valid concern, eye experts said. Looking at the sun without protective equipment can harm your vision, and complaints of eye issues have been documented after past eclipse events. However, cases of long-term damage after eclipses aren’t common.

In addition, hurting eyes aren’t the best indicator of a severe problem: Injuries from “solar retinopathy,” when light injures retinas, occur without immediate pain. 

Two main types of injuries are possible from looking at the sun — a burn to the outside of the eye and damage to nerve tissue within. 

“You can get a little bit of a burn to the surface of the eye, or what we call solar keratitis,” said Dr. Daniel Lattin, an ophthalmologist at Nemours Children’s Health in Jacksonville, Florida. “You can get sort of a burn to that cornea, and that’ll cause redness and tearing and those sorts of symptoms. That should resolve on its own, within a day or two, without any sort of permanent damage.”

That type of injury is rare, and it’s more commonly associated with climbers who spend time at high altitudes without proper eye protection, said Dr. Russell Van Gelder, an ophthalmologist at University of Washington Medicine and the director of the Karalis Johnson Retina Center in Seattle.

“It’s pretty hard to get that eclipse gazing; you need a fair amount of exposure,” Van Gelder said. 

Vision symptoms are more likely and worthy of concern. 

“If you have symptoms of a blind spot, wavy lines, floaters or blurry vision, that could be solar retinopathy after this eclipse, and you need to be seen right away,” said Dr. Luxme Hariharan, chief of ophthalmology at Dayton Children’s Hospital in Ohio. 

Hariharan said symptoms of solar retinopathy typically develop in the 24 hours after sun exposure. The condition isn’t associated with pain. 

“It’s not a pain sensation,” Van Gelder said. “The retina is devoid of the sense of pain or temperature. The retina can’t feel when it’s injured.” 

Lattin said there’s no treatment for solar retinopathy, though some patients’ eyes will recover over time.

Limited data suggests prolonged problems aren’t common. Only about 100 patients reported “eclipse-related retinopathy” after the 2017 total solar eclipse, according to a technical report published by the American Astronomical Society . Children and young adults were the most likely to be affected, according to the report, which based its estimates on informal survey data. 

Van Gelder said no national registry keeps track of such injuries. 

After the 2017 eclipse, he said, his clinic treated a half-dozen patients in Seattle who had eye complaints. He treated two of them directly, both of whom experienced partial recoveries. 

“If people are having pain, it’s probably not anything significant,” Van Gelder said. “If they have vision issues, they should be seen.” 

Experts said it doesn’t take long to damage the eyes after you gaze at the sun. 

“It can take as little as one to two seconds where you’re looking at it unprotected if you’re not using the glasses,” Hariharan said. “The problem is when the moon is blocking it in totality and it’s cloudy, you think it’s safe to look at, and people will stare at it for longer.”
