Primary Care

At Mobile Physician Services, our customized care teams provide patients with both comfort and familiarity as they work with a dedicated primary care provider and care coordinator to improve their health. Our physicians and staff take a proactive approach to preventive care, chronic disease management, and chronic illness support right where you live.

Each home visit includes an in-depth examination and individualized treatment plan, which is monitored and adjusted through routine follow-up visits. The primary care provider will deliver your ongoing care and will recommend to you specialty services as needed.

  • Annual Wellness visits : This wellness visit allows your primary care provider to create or update your personalized prevention plan. This visit includes a review of your medical and social history related to your health and may include counseling about preventive services. This plan may help you to prevent or reduce the chances of future illness based on your current health and risk factors.
  • New Illness Exams : When a new symptom or ailment arises, call us. Early indications of not feeling well could be a clue that you may be getting sick. A symptom in one part of the body may also be a sign of a problem in another part of the body. Moreover, unrelated symptoms that might seem minor on their own, could be warning signs of a more serious medical disease or condition. The new illness exam can be very brief or more detailed depending on your concerns and the provider’s findings.
  • Follow-up Care: Involves a regular medical checkup, which may include a physical exam and laboratory testing. Follow-up care checks are a proactive way of assessing the potential for and preventing health problems from returning after treatment of a disease has ended or an illness has seemingly passed.
  • Referral for Specialty Care : Referrals are the link between primary and specialty care. The referral coordination includes the documentation of patient care activities, the transfer of information, the inter-provider communication itself, and the integration of care services to the patient. Mobile Physician Services is a multi-specialty practice so many of these specialty referral services can be made seamlessly with little inconvenience or disruption to the patient and caregivers regular routines.
  • Medication Management: Medication management is a treatment structure that ensures our patients are receiving optimal therapeutic results from their prescription medications, both in the short and long term. Our team’s goal is to mitigate medication noncompliance and monitor all prescriptions treatments so that drug interactions complement one another for the most optimal outcome for our patients.

The provider may be a physician, advanced practice nurse or a physician assistant. A dedicated care coordinator will also be assigned for each patient to help arrange comprehensive services and assist patients and their caregivers.

  • Online Patient Portal: You and your designated caregiver, if you choose, will be able to connect with your provider through a convenient, safe and secure environment which allows access to your health records and a way to communicate with our staff in a timely manner.

Our psychiatry team specializes in the diagnosis and management of mental, emotional and behavioral disorders. They carefully evaluate each patient to develop an individualized treatment plan to improve the patient’s overall mental and physical well-being.

  • Depression : Depression is a common and often serious medical illness that negatively affects how you may feel, the way you may think, and how you may act. Depression can cause feelings of sadness, despair and hopelessness, which may lead to a loss of interest in activities you once enjoyed.
  • Anxiety : Intense, excessive, and persistent worry and fear about everyday common situations. The worry or anxiety could make you feel fatigued, irritable, and interfere with your regular sleep habits.
  • Dementia : An overall term that is used to describes a collection of symptoms related to an individual’s decline in memory or other thinking skills. It may be severe enough to diminish a person’s ability to perform everyday common activities. The effects of dementia can negatively influence your memory, thinking and social abilities.
  • Phobias : A phobia is when you experience excessive panicking or an irrational fear reaction to a situation. If you have a phobia, you may experience a deep sense of dread or fright when you encounter the source of your fear. The fear may be a certain place, situation, object, animal or even another person.
  • Behavioral disorders : Attention deficit, hyperactivity, bipolar, learning, defiant or conduct disorders are all examples of complaints that may have a detrimental impact on a person’s interpersonal relationships with family, friends, and co-workers.

Our team can provide behavioral counselling and medical therapy, when appropriate, to help a patient feel better about themselves and to assist them with better coping and managing their condition.

Our wound care specialists have been trained in the attention and treatment of all types of acute and chronic wounds. They have skill and experience in wound debridement and wound care procedures – managing chronic, non-healing wounds and infections, with a demonstrated care that fosters healing… right in the patient’s own home.

We specialize in serving homebound patients who may also be bed-bound or have difficulty in walking or moving around. As a result, immobility compression sores develop at pressure points on the body when the weight of an immobilized individual rests continuously on a firm surface, such as a mattress or wheel chair. Often these same patients are on oxygen or have high-risk medical conditions which makes it an even more challenging and stressful effort for them to travel to a doctor’s office for an appointment. Thus, the necessity for in home care and treatment.

Wounds that benefit from specialized wound care techniques include:

  • Diabetic foot wounds and ulcers
  • Post-surgical wounds
  • Traumatic wounds caused by injury
  • Arterial and vein stasis caused by lack of circulation
  • Immobility pressure sores. (Bed sores from stillness)

We work closely with home health agencies to provide ongoing care and monitoring of patient’s wounds.

House Call Medical Bag

Our board-certified podiatrists treat foot pain, wounds, and more. Treatments may include but are not limited to treating conditions of the lower extremities which could hinder mobility.

  • We will review each patients’ medical history to evaluate the condition of the feet, ankle or lower leg
  • Carry out a diagnosis on the feet and lower legs through examination and medical tests
  • Order physical therapy when deemed necessary
  • Treat wounds of the lower extremities using various wound care modalities. This may include debridement to improve the healing potential
  • Promote prevention, health & well being, the treatment and management of the foot and related problems, disability, deformity, and the pedal complications of chronic diseases for the elderly
  • Prescribe and fit prosthetic appliances such as diabetic shoe inserts and evaluate for bracing if necessary
  • Refer patients to other specialists for treatment, including conditions such as diabetes or arthritis
  • Advise patients on ways to prevent future leg problems and increase speed of recovery
  • Monitor the recovery progress of patients to determine the need for change in treatment

Podiatry

We know it’s not always easy to leave home to get the care you need for your eyes. Our optometrists bring state-of-the-art diagnostic eye equipment and technology to your home – making it much easier for you to get the vision care you need.

During a visit, your doctor will exam each eye for signs of serious issues such as glaucoma, cataracts, macular degeneration, and detached retinas, among other conditions.

Receiving regular eye exams regardless of the state of your vision can help detect serious eye problems at their earliest stages ─ when they are most treatable. During an eye exam, your doctor will observe and evaluate the health and condition of the blood vessels in your retina, which can be good indicators of the health of your blood vessels throughout the rest of your body.

  • Comprehensive eye exams : This exam goes beyond a simple vision screening. A comprehensive eye exam includes a host of tests in order to do a complete evaluation of the health of your eyes and your vision.
  • Annual retina exams : A retinal exam allows your doctor to evaluate the back of your eye, including the retina, the optic disk and the underlying layer of blood vessels that supply the retina.
  • Eyeglass fittings : A prescription works best when your eyeglasses are properly fitted. Improper fitting may cause pinching, distorted vision, headaches, and even dizziness. Our doctors will make sure your prescription lenses and frames are working together for you.
  • Diabetic eye exams : Diabetes does not have to lead to vision loss. Taking an active role in managing your diabetes can go a long way in curbing later complications. Regular eye exams, good management of your blood sugar and blood pressure, along with early intervention for vision problems can help prevent vision loss caused by diabetic retinopathy. Retinopathy is caused by damage to the blood vessels of the light-sensitive tissue at the back of your eye.
  • Refractions: This test is given as part of your routine eye examination. It is often referred to as a vision test. This test assists your eye doctor in measuring you for the exact lens prescription you will need.
  • Eye injuries: Eye trauma could be the result of a sudden blow to the eye. It may cause the eye to suddenly compress and retract which could cause damage to your eye and the surrounding tissue. Even if the injury may seem minor, all eye injuries should be furthered examined by a doctor for possible more serious and underlying injuries.
  • Eye infections: The most common eye infection is conjunctivitis, also known as pink-eye. An eye infection can happen in almost any part of your eye, including your eye lid, cornea and optic nerve. Symptoms of eye infections may include redness, itching, swelling, discharge, pain, or problems with vision. Always consult with your doctor before treating, as recommended actions are contingent on the cause of the infection.
  • Low-vision exams: A low vision exam is different from a normal eye exam. This functional-vision assessment determines how specific visual impairments affects your ability to perform everyday activities. The exam’s results assist your doctor in prescribing management tools and medications to better enhance and manage your remaining vision.

A pain management specialist is a provider with advanced training in diagnosing and treating pain. Our pain management specialists treat pain stemming from a variety of different causes, whether it’s neuropathic pain or headache, or the result of injury, a surgical procedure, cancer or another illness.

Palliative care is an approach to the holistic care of patients, including family and caregivers, to improve the quality of their lives after the diagnosis of a chronic debilitating disease or life-limiting illness that may cause a host of complaints.

Palliative care can begin at diagnosis and continue to be offered while the patient is continuing active treatment through different phases of their life limiting condition. Palliative care is for any patient with a chronic illness who is experiencing a decreased quality of life because of symptoms related to their illness or treatment, like renal dialysis, oxygen therapy or chemotherapy. The care is provided by a specially-trained team of doctors, nurse practitioners, physician assistants, and other specialists who work together to provide an extra layer of support to the patient and their caregivers.

Palliative care can help in symptom control including not only pain, but nausea, weakness, shortness of breath, fatigue and weight loss at any time during their diseases, not only at the end of life.

HouseCalls is a yearly health and wellness visit that happens in the privacy of your very own home. No driving to an appointment. No sitting in a waiting room.

Here’s how it works.

Our HouseCalls team finds a time that’s convenient for you and schedules the visit. A health care practitioner, a physician’s assistant, medical doctor or nurse practitioner, comes to you at the scheduled time. The clinician completes a health evaluation, including a physical exam and other important screenings, then YOU decide what happens next.

You can discuss questions to ask your doctor at your next appointment, review instructions from your last doctor’s appointment or chat about other health concerns you haven’t had a chance to talk to your doctor about. Whatever you need.

The visit takes about 45-minutes to an hour – plenty of time to answer your important health questions.

Your practitioner will even send a summary of your HouseCalls visit to your doctor.

And unlike those other home deliveries – there’s no additional cost for a HouseCalls visit, it’s a feature of your medical plan. If you’re in good health or you already see your doctor regularly – you might not think HouseCalls is for you, but it is. Think of it as an extra layer of care – valuable one-on-one time you don’t always get in the doctor’s office.

What to expect during a HouseCalls visit

During a HouseCalls visit, you’ll meet with the same types of professionals you’d see in a doctor’s office — a licensed physician or nurse practitioner. They’ll answer your health-related questions, perform a physical exam and offer a health screening in the comfort of your own home. If you choose, your family members, loved ones or caregivers are welcome to sit in on this visit and ask additional questions. A typical visit will range from 45 minutes to a full hour.

The results of your exam and screening will be forwarded to your primary care provider (PCP) and you’ll receive a summary of your visit in the mail. 

During your visit, here are some typical topics you'll cover

  • An overview of your medical history and prescription medications
  • Conversations about overall well-being, like how to help you stay active, sleep well and manage stress
  • Tips on to help you avoid trip hazards in your home
  • Community resources and support

Open the door to the convenience of a medical visit in your home

Meet with licensed medical staff on your schedule, in the convenience of your home. Once they arrive, talk about health concerns at your pace (the visits are focused on you and you alone). Use the results to help coordinate care with your doctor. 

  • Be prepared to provide a urine sample during your visit—drink fluids in advance
  • Wear shoes or slippers that are easy to remove (your feet will be checked)
  • Know where to find medication bottles so they are ready to review 
  • If applicable, have blood pressure readings available for review
  • If you have diabetes, please provide blood sugar test results, as well as your blood glucose meter 

Remember, at the end of your visit, you’ll receive a personalized checklist of topics to discuss with your doctor at your next appointment. Plus, you and your doctor will receive a mailed summary of your visit.

In 1930, house calls were standard practice for physicians, accounting for approximately 40 percent of patient visits. By 1950, this number had fallen to 10 percent, and by 1980, only about 1 percent of patient visits happened in the home. Who Qualifies for a Housecall Visit?

Patients with difficulty leaving home due to a mobility limitation, patients with difficulty leaving the home due to cognitive impairment, patients who require the use of special transportation to leave the home, patients with an inability to leave the home and for whom leaving the home requires taxing effort, patients requiring the assistance of another person to leave the home, patients for whom leaving the home is medically contradictory due to the patient's medical condition, accepted insurance.

What is a house call doctor?

Today, many doctors are bringing back the old practice of visiting patients in their homes. With house calls, older adults don’t have to go through the stress and difficulty of getting to the doctor’s office. And neither do you.

Another bonus is that these doctors usually spend more time with patients. You won’t have to fit all your questions into a 15 minute visit.

Do they accept Medicare?

Yes, many house call doctors accept Medicare , private insurance, and sometimes Medicaid . It usually costs the same as a regular office visit.

But every house call doctor is different, so make sure you understand their fees and accepted insurance plans before making an appointment.

How to find a house call doctor

Some large health care systems like Kaiser Permanente or the VA have programs that include home visits by doctors and nurses. For example, Kaiser’s home-based palliative care program includes house calls.

Even your older adult isn’t part of a large health network, it’s worthwhile to ask your older adult’s doctor if they have home doctor visit programs.

The American Academy of Home Care Medicine’s provider directory is another way to locate a house call doctor in your area.

You can also use Google to search for “house call doctor” + your city or county or “home doctor visit” + your city or county (don’t include the quotation marks).

What to look for in a house call doctor

Before booking an appointment, make sure you understand the doctor’s services, fees, and billing.

Questions to ask:

  • Do you specialize in treating seniors, people with Alzheimer’s or dementia , or those with  multiple chronic conditions ?
  • Do you accept Medicare , Medicaid , or my older adult’s private insurance plan?
  • If we want, can we also keep my older adult’s primary care doctor?
  • Will you communicate with my older adult’s existing doctors and specialists so their care is coordinated?

A HouseCalls visit is a no-cost yearly health check-in for members enrolled in an eligible UnitedHealthcare health plan.

Will a HouseCalls visit affect my insurance premium or benefits?

No. A HouseCalls visit is available to eligible UnitedHealthcare plan members and is not intended to imply the recipient has a specific condition or disease. The visit does not take the place of your regular visits to your primary care provider (PCP). Participation in this program does not affect your health care premium or coverage.

What should I expect during a HouseCalls visit?

During a HouseCalls visit, you’ll meet with the same types of professionals you see in a doctor's office — a licensed health care practitioner. They’ll help answer your health-related questions, perform a medical exam and select health screenings in the comfort of your home. If you choose, your family members, loved ones or caregivers are welcome to sit in on the visit and ask questions. A typical visit is up to a full hour.

The results of your exam and screening will be sent to both you and your primary care provider (PCP).

Who will come to my home for the HouseCalls visit?

You'll meet with a licensed health care practitioner who may be a nurse practitioner, physician assistant or medical doctor. Just like the professionals you see in your regular doctor’s office, our staff are state licensed and maintain national certification. We take the extra step of performing background checks on our clinical staff to provide additional assurance for our members.

How do I know if I am eligible for a HouseCalls visit?

It’s always a good idea to double-check eligibility with your health plan. Call the phone number on your member ID card to confirm.

Can my family members take advantage of a HouseCalls visit?

If your family member is enrolled in an eligible health plan, they can also have a HouseCalls visit at no additional cost. Each eligible member must schedule their own HouseCalls visit.

How can HouseCalls help if I already see a doctor?

  • HouseCalls supplements your doctor’s care with an extra set of eyes looking out for your health between regular doctor’s visits.
  • Your in-home health care practitioner will help you by supporting existing relationships with doctors — and can also help you establish relationships with new care resources if needed.
  • Doctor’s appointments can sometimes feel rushed, and you may not know what to ask. HouseCalls health care practitioners can provide you with a checklist of important questions and topics specific to your needs to discuss with your doctor at your next visit. Yes, we understand the importance of prompt healthcare. We make every effort to offer same-day appointments whenever possible. We do our best to arrive within one to two hours of your booking.

Do we offer house calls on weekends or evenings?

Yes, we understand that your schedule may be busy during weekdays. We offer flexible scheduling, including weekends and evenings and try our very best to accommodate your preferred times. Our medical house call services are available seven days a week. We’re here to provide you with personalized healthcare when it’s most convenient for you.

Do you provide service to my place of business or hotel?

Yes, ConciergeMD is an at-your-door service. We will come to your office, home or hotel 7 days a week, 365 days a year.

What should I expect during a House Call visit?

During a house call visit, you’ll meet with the same types of professionals you see in a doctor’s office — a licensed health care practitioner. After introductions, your provider will perform a thorough history and physical examination appropriate for your specific complaints and will then recommend the appropriate treatment options. After all of your questions are answered, instructions are given, your provider will depart.

How does a house call differ from a regular doctor office visit, Urgent Care Facility, or an Emergency Room?

We bring excellent, quality healthcare to you and on your schedule. You are seen in the comfort of your home, hotel or office and at a convenient time for you. You never have to wait again in an overcrowded waiting room. You will not be sitting next to sick patients for an extended period of time. We can treat the same illnesses that you would normally go to see in a primary care physician’s office. Our fees are affordable and often less expensive that one would pay at an Urgent Care Center or an Emergency Department.

Do you provide medications?

Our providers carry injectable medications and can prescribe medications to your local pharmacy for pick up.

What if the physician decides my illness requires additional testing and/or treatment?

We can provide all additional testing (e.g. x-rays, EKG, ultrasound, etc.) in the comfort and convenience of your home or hotel suite, and can provide referrals to any specialty if needed.

Do you draw labs?

We do offer a variety of laboratory tests and studies that can be completed in the home, hotel or workplace. Blood work can be drawn and delivered to local laboratories when necessary.

Do you offer X-ray and ultrasound services?

X-ray and ultrasound services are available to be brought to you upon request. A certified x-ray technician brings a portable x-ray machine to your home, hotel room, workplace, or wherever you are. The x-ray images are processed on site, and films are reviewed by your house call physician or a board certified radiologist. Copies of the images may be provided to the patient upon request. Various portable ultrasound studies are also available. Additional studies, including CT or MRI may be completed at a radiology center of your choice.

Will you care for the home-bound, or elderly, as a primary care doctor?

Yes, with pleasure, under most circumstances.

If I am experiencing an emergency, should I book a house call?

Concierge MD is not an Emergency Room replacement and we do not treat patients experiencing chest pain, shortness of breath, head trauma, loss of consciousness, or other life-threatening emergencies, as these conditions may require immediate hospitalization. If you are having a medical emergency, please go directly to the E.R. or dial 911 immediately.

Do you accept health insurance?

Although most insurance companies will reimburse a portion of the visit or apply the amount to your deductible if it has not been met at this time, we do not accept insurance in the traditional sense. We will provide you with the documentation that can be submitted to your health insurance company for reimbursement. You may be eligible for reimbursement through your “out-of-network” option of your PPO or HMO plan. We do not guarantee any reimbursement.

May I utilize your services while I still see my regular physician with whom I have a long term relationship?

Yes! We understand the tremendous value in long standing Doctor Patient relationships. We will confer with your physician and coordinate your care together.

In 1999, recognizing the needs of such elders and their families, we created the MedStar House Call program - MedStar Total Elder Care to provide full medical and social services that help elders remain in their homes with dignity. These teams now serve both Washington, D.C., and Baltimore, MD.

The MedStar House Call Program - MedStar Total Elder Care is nationally recognized for the quality and outcomes of our care. We serve patients at home with a team of geriatricians, nurse practitioners (NPs), social workers, office nurses, and coordinators. We make routine and urgent house calls (in-person or via telehealth with video or audio-only phone visits). Our medical staff is also available by phone 24/7 for urgent issues. We provide access to state-of-the-art hospital and specialty care. Additionally, our physician team follows our patients if and when they are admitted to the MedStar Washington Hospital Center.

If House Call patients require hospitalization, and 911 is not needed, our team can arrange transportation and admission to MedStar Good Samaritan or MedStar Washington when beds are available.

To qualify for the MedStar House Call Program, patients must:

Be 65 years or older and have difficulty getting to the doctor’s office

Have Medicare, Medicaid, or another participating insurance plan

Stop seeing their previous primary doctor and agree to have us take on that role following the first visit

Live in a qualifying ZIP code

Our office is open 8:30 a.m. to 4:30 p.m., Monday - Friday.

As in most doctor's offices, your main health insurance (such as Medicare) covers 80 percent of House Call visit fees, and any secondary insurance covers the other 20 percent. The patients who do not have any secondary insurance are responsible for the 20 percent co-pay.

Is the House Call Doctor Coming Back?

While uncommon, house calls can be appealing to doctors, patients and the health care system. 

When Simon Jacobsen’s aging mother needed to see a doctor, she first had to descend the stairs – a journey that once left her with a broken neck. After that, Jacobsen and his father carried her down themselves, but their method was hardly graceful.

“We were just a couple of gorillas doing the best we could,” says Jacobsen, a 49-year-old who lives ​in the District of Columbia. Meanwhile, the care his mom received was fragmented, time-consuming and physically and emotionally exhausting for the entire family.

“It just got to a point where we realized we couldn’t move this person, but we also believed that she didn’t belong in a nursing home,” Jacobsen says.

So Jacobsen’s father called Dr. Ernest Brown​, a 47-year-old ​family physician in the District of Columbia who runs a house call-only independent practice. Until Jacobsen’s mom’s death about five years later, Brown served as her primary provider, coordinating with other doctors for more specialized care, driving her to the hospital himself and eventually setting up a type of in-house hospice care . Some weeks, he visited every day.

“I think this is care that everybody deserves,” Jacobsen says.

For Brown – who's tough to miss in blue scrubs with "House Call Doctor" embroidered on the back and a classic black medical bag in his hand – one of the most rewarding aspects​ of his job is being a part of the community. “I feel like I belong,” he says between calls last week, while sipping a cappuccino at a local Turkish restaurant – whose staff he treats​.

Brown cares for many hotel guests, including visiting heads of state and tourists, as well as low-income, elderly people whom he sees pro bono. He also treats airline employees, hotel concierges and a wide circle of friends – nearly all within a few mile radius of his apartment. "I really don't have to go but a few blocks to do what I do – provide care," he says.

Brown is a rarity among an already small cohort of doctors. Only 13 percent of family physicians surveyed made regular house calls in 2013, and only 3 percent made more than two per week, according to the American Academy of Family Physicians . Even fewer run practices like Brown’s that are exclusively house calls, says AAFP's president, Dr. Robert Wergin, a family physician in Milford, Nebraska, who goes on a couple house calls each month.​

Still, the historic practice of seeing patients in their homes may be making a comeback, says Dr. Justin Davis​, a primary and urgent care doctor in San Francisco who typically sees a few house call patients every day in addition to treating people in his private practice.

“When I first started [making house calls] 10 years ago, it was even more unheard of,” he says. “Now, there’s a paucity of it in actual practice, but there’s a lot of talk about it. It’s becoming more in people’s consciousness.”

A Personalized Experience

The concept of a doctor coming to a patient rather than a patient going to a doctor is hardly revolutionary. In 1930, about 40 percent of doctor-patient interactions were through house calls, but by 1980, the rate was down to 1 percent, according to a Clinics in Geriatric Medicine article .​ “It used to be the norm, and the norm was decency,” Jacobsen says. “[Dr. Brown’s care] is an act of incredible decency that seems so foreign to so many people now because they’re just not used to it.”

From a patient’s perspective, summoning a doctor to your house has plenty of appeal: No waiting rooms , traffic or skipped work. No dragging a sick and tired body out of bed to see a doctor whose prescription is to go back to bed. No feeling like a number.​

Patients are looking for "a personalized experience,” Davis says. “They want to feel like they’re with somebody who knows about them, or at least cares about them, and can give them time and really listen to them.”

For doctors, aspects of the practice are attractive too: more satisfying patient interaction and the ability to see how patients live. “There’s nothing like going into the home saying, ‘This is what their environment looks like,” Wergin says. “If there’s food that’s old on the counters, if it’s just in disarray, that tells me … that maybe they’re going to need more help.”

The health care system also seems to benefit. A study last year in the Journal of the American Geriatrics Society found that Medicare costs among 722 elderly patients in a house call program were $8,477 less​ on average per person than costs among a group that didn't receive house calls over two years​. The house call group also had 9 percent fewer hospitalizations, 10 percent fewer emergency department visits and saw specialists 23 percent less.

While house call patients in the study saw generalists more than twice as often as the other group, those visits still saved the health care system money since specialists' care is usually more costly. For Brown, ​the chance to prevent illness is key. He can track the origin of a restaurant worker's sickness, for instance, because he knows the employee's lifestyle and colleagues. He can also predict who else may get infected – and then try to prevent it. "I can do more here than I ever could in a clinic," Brown says.

What’s the Hold Up?

Despite the benefits, there are logistical barriers to making house calls more widespread among physicians. And of course, not all medical issues should be handled at home. "Sometimes the office setting is good because then you have your whole team there," not to mention more equipment, Wergin says.

When house calls are appropriate, physicians who make them can take the hit in their paycheck, since they're predominantly paid for the number of patients they see and the number of procedures​ they perform. Doctors who travel to patients’ homes simply can’t pack in nearly as many visits as doctors who have patients come to them. And, while most insurance companies do reimburse for house calls, they'll only do so if there's a medical reason why the patient can't or shouldn't visit an office instead.

For some house call doctors, it's easier to deal outside of insurance networks altogether. Davis, for one,charges new patients a minimum of $250 for an office visit and $450 for a house call, which they can pay out of pocket or submit to their insurance company for reimbursement. With this model, he sees anywhere from four to 18 patients a day. A primary care doctor in an office-based practice, on the other hand, might be expected to see up to 40, he says.

Brown also eschews insurance. "Anytime an insurance company comes in, I get hives," he says. He charges patients on a sliding scale based on the care provided, length of the visit and the patient’s financial situation. And no patients are charged until they’re healthy. As Brown told an inquiring German hotel guest whose wife was bedridden with either food poisoning or gallstones, "I charge [patients] when they get better."

Brown’s setup pays his bills and satisfies him, but it wouldn’t work for all doctors interested in making home visits. “While it can certainly be financially rewarding for physicians to give up their current way of doing things or go out of their practice and sort of strike out on their own, doing this can be very daunting,” Davis says.

But as the Affordable Care Act is implemented, that prospect may be a little less so. The law penalizes hospitals for high readmission rates – something house calls can help avoid – and shifts Medicare from a volume-based, fee-for service system to one that also depends on quality of care, Wergin says. ​

“That’s where telemedicine – and maybe even home visits on occasion – come in,” he says. Wergin predicts an increase in virtual house calls through technologies like Skype , while Davis expects to see more nurse practitioners and physician assistants offering house calls at a lower price point than physicians.

“It’s difficult to say how that will pan out, but I do think there’s a definite trend toward expanding health care,” he says. “Physicians and patients alike are both looking for a better health care experience, and I think this is definitely one of the avenues you’ll see more of.”

Please call or email to request an appointment. Appointments are scheduled 1-2 weeks in advance according to when a doctor will be seeing patients in your area. We will call you to confirm a one to two hour appointment window for the doctor’s arrival at your home.

Emergency medical care is provided free to all foreign nationals in case of life-threatening conditions that require immediate medical treatment. You will be given first aid and emergency surgery when necessary in all public health care facilities. Any further treatment will be free only to people with a Compulsory Medical Insurance, or you will need to pay for medical services. Public health care is provided in federal and local care facilities. These include 1. Urban polyclinics with specialists in different areas that offer general medical care. 2. Ambulatory and hospitals that provide a full range of services, including emergency care. 3. Emergency stations opened 24 hours a day, can be visited in a case of a non-life-threatening injury. It is often hard to find English-speaking staff in state facilities, except the largest city hospitals, so you will need a Russian-speaking interpreter to accompany your visit to a free doctor or hospital. If medical assistance is required, the insurance company should be contacted before visiting a medical facility for treatment, except emergency cases. Make sure that you have enough money to pay any necessary fees that may be charged.

Travelers need to arrange private travel insurance before the journey. You would need the insurance when applying for the Russian visa. If you arrange the insurance outside Russia, it is important to make sure the insurer is licensed in Russia. Only licensed companies may be accepted under Russian law. Holders of a temporary residence permit or permanent residence permit (valid for three and five years respectively) should apply for «Compulsory Medical Policy». It covers state healthcare only. An employer usually deals with this. The issued health card is shown whenever medical attention is required. Compulsory Medical Policyholders can get basic health care, such as emergencies, consultations with doctors, necessary scans and tests free. For more complex healthcare every person (both Russian and foreign nationals) must pay extra, or take out additional medical insurance. Clearly, you will have to be prepared to wait in a queue to see a specialist in a public health care facility (Compulsory Medical Policyholders can set an appointment using EMIAS site or ATM). In case you are a UK citizen, free, limited medical treatment in state hospitals will be provided as a part of a reciprocal agreement between Russia and UK.

Pharmacies can be found in many places around the city, many of them work 24 hours a day. Pharmaceutical kiosks operate in almost every big supermarket. However, only few have English-speaking staff, so it is advised that you know the generic (chemical) name of the medicines you think you are going to need. It is standard practice for expats to visit private clinics and hospitals for check-ups, routine health care, and dental care, and only use public services in case of an emergency. Insurance companies can usually provide details of clinics and hospitals in the area speak English (or the language required) and would be the best to use. Investigate whether there are any emergency services or numbers, or any requirements to register with them. Providing copies of medical records is also advised.

Moscow hosts some Western medical clinics that can look after all of your family’s health needs. While most Russian state hospitals are not up to Western standards, Russian doctors are very good.

In a case of a medical emergency, dial 112 and ask for the ambulance service (skoraya pomoshch). Staff on these lines most certainly will speak English, still it is always better to ask a Russian speaker to explain the problem and the exact location.

Ambulances come with a doctor and, depending on the case, immediate first aid treatment may be provided. If necessary, the patient is taken to the nearest emergency room or hospital, or to a private hospital if the holder’s insurance policy requires it.

Rare visit by House speaker to campus escalates tension at Columbia

“Get off our campus!” one student yelled. “Go back to Louisiana, Mike!” someone shouted.

NEW YORK — House Speaker Mike Johnson and his Republican colleagues were met with boos, laughs and pro-Palestinian chants after parachuting into one center of the roiling protest movement against Israel’s war against Hamas: Columbia University in New York City.

Johnson and a group of GOP lawmakers landed on campus — where tensions are high between the university administration and students who have erected pro-Palestinian encampments — and demanded that Columbia’s president, Nemat “Minouche” Shafik, resign for failing to quickly dismantle the encampments and, in their view, for not doing enough to ensure that Jewish people on campus feel safe.

Around 4 p.m. Wednesday, the Louisiana Republican — who just shepherded through Congress a $26 billion aid package for Israel, including $9 billion in humanitarian help to Gaza and elsewhere — appeared on the steps of Columbia’s stately library, which looks out over the student encampments. Signs of a campus on edge were all over: A dozen New York police officers stood guard outside the school’s big black gates on Broadway. Bike racks strung with yellow police tape cordoned off some of the sidewalk.

“I am here today joining my colleagues and calling on President Shafik to resign if she could not immediately bring order to this chaos,” Johnson said. “As speaker of the House, I’m committed today that the Congress will not be silent as Jewish students are expected to run for their lives and stay home from their classes hiding in fear.”

A crowd of students, swelling as Johnson and his colleagues began speaking, intermittently laughed and yelled that they couldn’t hear the congressman or his colleagues. The students booed the speaker, chanted in support of Palestine, including “Free Palestine,” “Stop the genocide” and “ From the river to the sea ,” a phrase that some say constitutes antisemitic speech.

“Enjoy your free speech,” Johnson rejoined, sounding uncharacteristically irritated.

As Johnson wrapped up, the students renewed their boos and began to chant, “Mike, you suck!”

House Republicans have long accused elite colleges and universities of skewing left and pursuing a “woke” agenda that tramples on parental rights. But the antiwar outbursts on campuses across the country that began shortly after Hamas’s Oct. 7 attack on Israel — and the rise of antisemitism on college campuses, according to the Anti-Defamation League — are now oft-repeated targets of Republican criticism. GOP lawmakers are seeking to slash federal funding for universities and have hauled university officials to Capitol Hill to answer questions such as whether “calling for the genocide of Jews” would violate their schools’ code of conduct.

“If these campuses cannot get control of this problem, they do not deserve taxpayer dollars,” Johnson said. “We’ll continue to work on legislation to adjust this at the federal level. This Congress — and I genuinely believe there’s bipartisan agreement on this — will stand for what is good and what is right.”

House Democrats descended Monday onto Columbia’s campus to express outrage over antisemitic harassment of Jewish students on and around campus. They included Jewish Reps. Josh Gottheimer (N.J.), Dan Goldman (N.Y.), Jared Moskowitz (Fla.) and Kathy Manning (N.C.).

The lawmakers’ pleas were not as forceful as Republicans’, who left no room for distinction between those targeting Jewish students and those peacefully protesting the Israel-Gaza war . But the Democrats were adamant about the need to protect students with backgrounds like theirs.

“Imagine trying to study for finals at Columbia, while people outside the library are calling for your death,” Gottheimer said at a news conference following their walk through campus. “To the administrators at Columbia and beyond, here are our demands: Stop the double talk and start acting. Discipline harassers. Restore civility on this campus. Encourage peaceful, constructive, civil dialogue. Every student has a right to be safe on campus.”

Johnson’s remarks came after he met with Jewish students at Columbia University, shared a meal with the university’s Rabbi Yuda Drizin and briefly met with Shafik before the news conference with three New York House Republicans and House Education and the Workforce Chair Virginia Foxx (R-N.C.). Johnson said Republicans met with Shafik and other top officials and left the meeting believing that they had “not acted to restore order on the campus.”

Asked whether he believes the National Guard should be sent in to restore order on college campuses across the country, Johnson said, “If this is not contained quickly, and if these threats and intimidation are not stopped, there is an appropriate time for the National Guard.”

Johnson also said he would call President Biden to inform him about what he saw on campus and “demand that he take action. There is executive authority that would be appropriate.”

The speaker’s visit marks the first time the top representative in the U.S. House has visited a college campus amid ongoing protests that have led to tense exchanges between pro-Palestinian and Jewish students. More than 100 people on Columbia’s campus were recently arrested and charged with trespassing, with several students who took part in the protest facing suspension just weeks before year’s end. Shafik called on the New York Police Department, whose officers arrived in riot gear, to arrest protesters just one day after she and other Columbia leaders told Congress she would make changes aimed at ending the harassment of Jewish students. The school also announced it would start a hybrid learning model for the rest of the year.

Neither Johnson nor Congress has any power to force a university president’s resignation. White House press secretary Karine Jean Pierre on Wednesday declined to weigh in on whether Shafik should resign, telling reporters, “Columbia’s a private institution. We’ve been very consistent here about not commenting on personnel matters.”

House Republicans who visited Columbia with the speaker made clear they would follow through with punishing colleges and universities if the protests are not controlled.

“The inmates are running the asylum,” Foxx said. “The [Education and Workforce] committee will pursue every possible avenue to create a safe learning environment for Jewish students.”

Rep. Michael Lawler (R-N.Y.) was much more forceful in his rebuke of students, acknowledging that he too wants Palestinians to be free “from their oppressor, Hamas,” and characterized any students who support the terrorist organization as “an absolute abomination.”

“If you are a protester on this campus, and you are proud that you’ve been endorsed by Hamas, you are part of the problem,” fellow N.Y. Rep. Anthony D’Esposito (R) said.

House Republicans have been pummeling the heads of elite university institutions for months, using them as a punching bag to make a broader point about how out-of-touch elite institutions are with normal Americans. Johnson has previously invited Jewish students to meet with him in the Capitol, and he has often allowed them to tell their stories of being under attack at school during news conferences.

At a December hearing, the interrogation by House GOP Conference Chair Elise Stefanik (R-N.Y.) of Harvard President Claudine Gay over whether antisemitic remarks should be protected under free speech went viral.

The hearing led to a bipartisan call on Capitol Hill to denounce or demand the resignation of leaders at Harvard, the University of Pennsylvania and the Massachusetts Institute of Technology for their responses, which were deemed out of touch. Penn President Liz Magill and Gay both resigned amid public outcry.

The hearing launched further investigations by the committee and continual hearings to combat antisemitism on college campus, ending in Shafik’s Capitol Hill testimony last Wednesday.

The Israel-Gaza war is also contentious among House Democrats, with liberals clashing with some Jewish colleagues early on in the war. That prompted Democratic leadership to attempt to keep attacks from becoming personal. Over the weekend, 37 liberals voted against sending $14 billion in aid to Israel over concerns that humanitarian aid would not reach Gaza, joining 21 Republicans who did not support the measure over spending concerns.

It’s just as complicated on Columbia’s campus. Basil Rodriguez, 23, argued Wednesday that Johnson and any lawmaker who backed sending aid to Israel is responsible for the deaths of thousands of Palestinians.

“I would urge him to reckon with his own positionality in the United States government and how the U.S. has been sending weapons that are falling in Gaza,” said Rodriguez, who is participating in the student encampment on campus. “He is directly complicit in this genocide unless he is a vocal advocate for it to stop.”

For Jewish student Spencer Davis, 19, the situation at Columbia is more nuanced than many of its critics have portrayed.

A member of a joint program between the Jewish Theological Seminary and Columbia, he was in the crowd watching Johnson speak. He said he feels safe on campus but understands why others do not and that his roommate booked a last-minute flight home over safety concerns. Davis said people have thrown things at members of his Jewish fraternity.

Still, Davis said, he believes the protests have been largely peaceful and questioned the motivations of politicians such as Johnson who have decried the encampment and Columbia’s leadership. “I think that a lot of Republican congresspeople are using this opportunity to further their culture war against liberal institutions like Columbia,” Davis said. “I think it has less to do with protecting Jewish students and more to do with their agenda, and they’re using Jewish students as pawns.”

Visit the Cathedral of Christ the Savior

This impressive Moscow Cathedral is the seat of the Russian Orthodox Church. It is a must see if you visit Moscow in 2 days and clearly one of the things to do in Moscow. The Cathedral of Christ the Savior was first built in 1883 in memory of Russia’s victory over Napoleon’s Grand Army. Then in 1931 Stalin ordered its destruction. It was then rebuilt again (identically) only in 2000.

The old Krasny Oktyabr factory: one of the coolest things to do in Moscow

If you cross the Moskva River by a pedestrian bridge which is located just in front of the Cathedral of Christ the Savior, you will enjoy a beautiful view of the city and at the same time you can discover Balchug Island. Furthermore, if you want to visit Moscow in 2 days, you can include this island in your itinerary between the cathedral and Gorky park. Here is the old confectionery factory Krasny Oktyabr, which has gradually turned into a fashionable micro-district. There are then some elements of street art, cafes and restaurants and some Moscow bohemian side. At the end of the island you can see a gigantic 98-meter-high monument dedicated to the Russian reforming tsar Pierre The Great.

Good to know! You can find on this island are the trendiest nightclubs in Moscow. On weekends, there are taxi caps after midnight so there are so many people. On the other hand, if you go there in winter and during the day, the island is quite empty and less interesting to see.

Gorky Park is one of the TOP places to visit in Moscow, because it allows you to better understand the life of the locals and their rhythm. In fact, it’s a huge entertainment park. For example, in winter there is a giant ice rink and in summer – free dance or yoga lessons, sandy beaches for playing volleyball, an outdoor cinema. So, like the locals, have a Stakantchik (ice cream or cooked corn), and enjoy the atmosphere of the place: that’s one of the interesting things to do in Moscow to discover the city.

Sparrow Hill and Moscow State University

The Sparrow Hill, Vorobiovy Gori in Russian, is the highest point in Moscow. It is rather known to Russians, but less to travelers. A nice view on Moscow opens from the hill, and in particular on the Luzhniki Stadium. In addition, on the hill itself is the Moscow State University: an impressive skyscraper from the Soviet era.

  • How to get there? By bus T7 (35 min) from Oktyaborskaya station, near Gorki Park. By metro (Vorobiovy Gorki station) + climb the hill on foot. On foot along the Moskva along the Gorky Park (1h30) + climb in funiculars.

Good to know! It is possible to cross the Moskva river by funicular. We actually tested it and it was pretty cool! That is one of our favorite things to do in Moscow!

  • Opening hours. Daily: 10: 00-20: 00, except Thursday: 11: 00-21: 00. Closed on Mondays. The boxes close an hour before closing.

Walking on Arbat Street

All Russians know Rue Arbat. So, walking on Arbat street is one of the things to do in Moscow. However, after the years, little by little it became very touristy. This is a pedestrian street only. There are souvenir shops, restaurants and cafes there, but it is no longer the most authentic neighborhood in the city.

Moskva-City is Moscow’s business center, much like Paris’s Defense district. The skyscrapers of Moskva-City are among the tallest in Europe: 373 meters high! Very nice place to see at dusk.

Good to know! You can admire a nice view of Moscow City from the docks of Tarasa Shevchenko. It’s especially beautiful in the evening with all the lights on.

Bunker 42, one of the most unusual things to do in Moscow

Bunker 42 is a secret military complex which was to be used by the Soviets in the event of a nuclear attack: a space of 7000 m² 65 meters underground!

What to visit in Moscow in 6 days or more?

There are still so many places to see, because Moscow is a big megalopolis and there is always something exciting to do there. For example: the ancient Kolomenskoye village or the WINZAVOD contemporary art center .

If you are interested in history and want to see Russian cities on a rather “human scale”, it would certainly be interesting for you to discover the cities of the Golden Ring . For example, it is very easy to get to Sergey Posad from Moscow (less than 2 hours in train). Visiting the Golden Ring is one of the best things to do in Moscow if you are staying more than a 5-6 days.

Businesswoman and reality TV star Kim Kardashian visited the White House Thursday to meet with Vice President Kamala Harris about criminal justice reform, an issue for which Kardashian has long been an advocate.

Harris announced a new rule from the Small Business Administration that lifts some restrictions on loans for people with criminal records.

"Making this available, reducing and eliminating that restriction is going to mean a lot in terms of second chances and the opportunity for people to excel," Harris said.

"Is it not the sign of a civil society that we allow people a way to earn their way back, and give them the support and resources they need to do that?" Harris said.

The event comes the day after President Biden used his clemency authority to pardon 11 people who were convicted of non-violent drug offenses and commute the sentences of five others. The White House says these individuals received disproportionately long sentences and would have been given shorter sentences under current laws and policies.

Four of the pardoned individuals attended the event with Harris and Kardashian Thursday.

"I am super honored to be here to hear your stories today," Kardashian said. "I think it's so important to share them an amplify them because there are so many people that are in your position that can use the inspiration."

Kardashian was instrumental in elevating the issue during the Trump administration , advocating for pardons and meeting the former president and his son-in-law and adviser Jared Kushner.

The White House says so far, Biden has commuted the sentences of 122 individuals and granted pardons to 20 who committed non-violent drug offenses.
